The insulin-like growth factor system: towards clinical applications.

Insulin-like growth factors (IGF-I and II) are important endocrine, paracrine and autocrine mediators of physiological growth. They promote cellular proliferation, survival and differentiation. Their effects are mediated mainly through the IGF-I receptor, but IGFs also bind to the IGF-II/mannose 6-phosphate and insulin receptors. IGF activity is modulated by a family of six high-affinity IGF binding proteins (IGFBPs); in most situations, IGFBPs inhibit IGF actions but they may also enhance them. Assays are now available for IGF-I, IGF-II and individual IGFBPs. IGF-I and IGFBP-3 assays have som

The influence of atropine dose on recovery from vecuronium-induced neuromuscular blockade.

To determine whether the dose of atropine affects the rate of neostigmine-induced recovery from vecuronium-induced neuromuscular blockade, the authors monitored isometric adductor pollicis mechanical activity in 36 anesthetized (thiopental, fentanyl, nitrous oxide) adult patients (ASA physical status 1 or 2). Once surgery was completed and twitch height had spontaneously regained 25% of its initial value, the patients were randomly allocated into three groups (A10, A15, A20; n = 12 in each group) according to the dose of atropine (10, 15, or 20 micrograms/kg) that was mixed with 40 micrograms/

Necrotizing Fasciitis in a Patient on Long-Term Intramuscular Interferon-Beta for Multiple Sclerosis: A Case Report.

CASE: Immunomodulatory injections are becoming common long-term treatments for neuromuscular diseases such as multiple sclerosis (MS), although they carry a risk of local site infection. We describe a case of a 57-year-old man who developed necrotizing fasciitis of the anterior thigh secondary to intramuscular (IM) injections of interferon-beta-1A for MS, ultimately developing septic shock and requiring serial debridements for source control. CONCLUSIONS: This is the first reported case of necrotizing fasciitis from chronic IM injections for MS and deserves particular attention because of the

[The central and peripheral components of respiratory muscle fatigue during inspiratory resistive loading in cats].

In 18 anesthetized cats, electrical activity of diaphragmatic nerve, external intercostal muscles and diaphragm, was monitored. Starting from a certain level of inspiratory load, the transdiaphragmatic pressure and the diaphragm EMG decreased, the efferent activity of diaphragmatic nerve remaining the same. This data suggests a peripheral character of diaphragmatic muscle exhaustion related to disorders in neuromuscular transmission. Further exposure to hard mechanical load entails a decrease in diaphragmatic nerve electrical activity, too, thus indicating a development of central exhaustion o

Intraoperative neurophysiological monitoring (IONM) in neurosurgery: A critical appraisal of established practices, ongoing controversies, and future trajectories.

Intraoperative neurophysiological monitoring (IONM) has evolved from a novel technique into an evidence-based standard treatment method for high-risk neurosurgical and spinal surgeries. Its effectiveness is based on two interrelated pillars: optimized multimodal monitoring, mainly including motor and somatosensory evoked potentials (MEPs/SSEPs) as well as electromyography (EMG), and total intravenous anesthesia (TIVA) combined with a precise neuromuscular blockade-based anesthesia protocol. IONM significantly reduces neurological function damage during surgeries for spinal deformities, intrame

The effect of methylprednisolone on the reversal time of rocuronium by sugammadex in the paediatric patient group: A randomised controlled trial.

BACKGROUND: Sugammadex is an effective agent for reversing aminosteroid neuromuscular blocking agents, but its effectiveness can be influenced by interactions with steroid-based drugs. Previous studies suggest corticosteroids may affect sugammadex effectiveness, but data on methylprednisolone in paediatric patients is limited. OBJECTIVE: This study aimed to determine whether methylprednisolone administration at induction affects the reversal time of rocuronium by sugammadex. DESIGN: Single centre, double blinded, randomised controlled trial. SETTINGS: Turkish tertiary care centre from Septe

Designing a Self-Powered Hydrogel-Based Wearable EMG Device.

Neuromuscular conditions arising from accidents, injuries, or genetic neurodegenerative conditions can significantly affect the patient's physical and mental well-being, and tracking their progression and treatment often requires continuous monitoring. Herein, we propose a self-powered IoT-enabled wearable electromyography (EMG) solution that integrates flexible solar panels embedded in everyday clothes. This study presents a self-powered wearable EMG system that integrates flexible solar cells, hydrogel-based electrodes, and a microcontroller-driven circuit. The solar cells deliver 1.763 W, 1

Variations of nociception level (NOL®) measurements during robot-assisted laparoscopic prostatectomy - a monocentric retrospective analysis.

BACKGROUND: Inadequate analgesia during anaesthesia is associated with a range of complications. While anaesthesiologists routinely monitor the depth of anaesthesia and neuromuscular blockade, no system currently in routine clinical use provides an objective assessment of adequacy of anti-nociception. Although various monitoring systems have been developed in recent years, their impact on the optimization of analgesic therapy remains uncertain. Moreover, the influence of perioperative surgical and non-surgical procedures and events on the measured parameters is not yet fully understood. Noneth

New perspectives in intraoperative facial nerve monitoring with antidromic potentials.

Electromyographic (EMG) activity recorded from the facial muscles in response to electrical and mechanical stimulation is the most popular method for continuous monitoring of the facial nerve during cerebellopontine-angle surgery. EMG recording is, however, extremely sensitive to the administration of neuromuscular blockers. An alternative technique for the continuous monitoring of the facial nerve [i.e., monopolar recording of facial nerve antidromic potentials (FNAPs)], is described. Ten subjects undergoing retrosigmoid vestibular neurectomy for Meniere's disease and 11 subjects operated on

Influence of the Pringle maneuver during partial hepatectomy on the neuromuscular block induced by intermittent and continuous dosing of rocuronium.

PURPOSE: The Pringle maneuver (PM) is a common procedure in hepatectomy that is known to interrupt drug elimination. The purpose of this study was to examine the influence of PM on the duration of action of rocuronium administered by intermittent bolus dosing, the continuous rocuronium infusion dose required for maintenance of a moderate neuromuscular block, and changes in plasma concentrations of rocuronium. METHODS: Twenty-seven adult patients undergoing partial hepatectomy with PM were enrolled in this study. The duration of action of 0.2 mg/kg rocuronium boluses (DUR), and the continuous

Train-of-four recovery after pharmacologic antagonism of pancuronium-, pipecuronium-, and doxacurium-induced neuromuscular block in anaesthetized humans.

Previous studies have suggested that the increased duration of action of long-acting neuromuscular relaxants may make their pharmacologic antagonism more difficult and, thus, increase the likelihood of residual block. This hypothesis was tested in healthy, adult humans who received a background of isoflurane/N2O/fentanyl anaesthesia. Study subjects were paralyzed with either pancuronium (N = 8), pipecuronium (N = 8), or the longer-acting relaxant, doxacurium (N = 8). Neuromuscular function was monitored, and, using a blinded, randomized study design, the relaxants were titrated to identify the

Evaluation of reliability of post-tetanic motor-evoked potential monitoring during spinal surgery under general anesthesia.

STUDY DESIGN: A prospective research. OBJECTIVE: Compare the reliability of post-tetanic motor-evoked potential (p-MEP) monitoring in the detection of motor injury during spinal surgery with that of conventional MEP (c-MEP). SUMMARY OF BACKGROUND DATA: Myogenic MEPs are sensitive to suppression by anesthetics and neuromuscular blockade. Recently, we reported a new technique for MEP recording, called "p-MEP" in which MEP amplitude can be enlarged by tetanic stimulation of peripheral nerve before transcranial stimulation in comparison with that of c-MEP. The purpose of this study is to compare
