Cerebellopontine Angle Surgery Assisted by Continuous Mapping of the Facial Nerve Via the Ultrasonic Aspirator.

BACKGROUND: Cerebellopontine angle (CPA) surgery carries the risk of lesioning the facial nerve. The goal of preserving the integrity of the facial nerve is usually pursued with intermittent electrical stimulation using a handheld probe that is alternated with the resection. We report our experience with continuous electrical stimulation delivered via the ultrasonic aspirator (UA) used for the resection of a series of vestibular schwannomas. METHODS: A total of 17 patients with vestibular schwannomas, operated on between 2010 and 2018, were included in this study. A constant-current stimulato

Update on Trigeminal Neuralgia.

PURPOSE OF REVIEW: To review current treatments for trigeminal neuralgia, with an emphasis on determining which patients may benefit from neurosurgical procedures. RECENT FINDINGS: A detailed history is the most helpful tool for diagnosing trigeminal neuralgia (TN) and predicting response to neurosurgical treatments. Patients with classic trigeminal neuralgia will describe severe, unilateral, intermittent facial pain that is triggered by innocuous sensory stimuli. In most cases, pain is caused by compression of the trigeminal nerve by a blood vessel near the brainstem. Magnetic resonance imag

Repair of Frontoethmoidal Encephalocele in the Philippines: An Account of 30 Cases Between 2008-2013.

BACKGROUND: Frontoethmoidal encephalocele is a congenital abnormality of the anterior skull base involving herniation of cranial contents through a midline skull defect. Patency of the foramen cecum, along with other multifactorial variables, contributes to the development of frontoethmoidal encephaloceles. Because of limited resources, financial constraints, and lack of surgical expertise, repair of frontoethmoidal encephaloceles is limited in developing countries. METHODS: Between 2008 and 2013 an interdisciplinary team composed of neurosurgeons, craniofacial surgeons, otolaryngologists, pl…

Two-color 3D-3D fusion of selective rotational cerebral angiograms: a novel approach to imaging in cerebrovascular neurosurgery.

BACKGROUND: Since its introduction, digital subtraction angiography has been considered the gold standard in diagnostic imaging for neurovascular disease. Modern post-processing techniques have made angiography even more informative to the cerebrovascular neurosurgeon or neurointerventionalist. Open neurosurgical procedures such as aneurysm clipping, extirpation of arteriovenous malformations, and extracranial-intracranial bypass remain important techniques in the armamentarium of a comprehensive cerebrovascular neurosurgeon. In-depth study of the anatomy of vascular pathology prior to and aft

Patient satisfaction after surgery for trigeminal neuralgia--development of a questionnaire.

OBJECTIVE: This project aimed to prepare a self complete patient satisfaction survey for patients who have undergone surgery for trigeminal neuralgia and then assess its reproducibility, validity and acceptability in one centre. METHODS: The questionnaire, for initial use in patients who had undergone posterior fossa surgery for trigeminal neuralgia, was designed after a systematic review of the surgical literature had been performed and discussions held at the US and UK Trigeminal Neuralgia Support group meetings. It underwent several changes after input from neurosurgeons, patients, copywri…

The Utilization of Palliative Care for Patients with Intracerebral Hemorrhage: A Narrative Review.

Intracerebral hemorrhage (ICH) is a neurosurgical ailment regarded to possess a high degree of mortality and long-term complications in patients. Despite the abundance of literature on the use of PC for neurological disorders, there remains a gap in the literature examining the use of this care specifically for patients with ICH. Herein, we conducted a narrative review of 30 included studies within the literature to examine the utilization of PC in patients with ICH. Through greater awareness, acknowledgement, and formalized training in PC skills for neurosurgeons, the needs of patients and th…

Biopsy of the Superficial Sensory Radial Nerve in the Distal Forearm: A Technical Note.

Neurosurgeons are frequently consulted for nerve biopsy. Common indications for nerve biopsy include vasculitis, peripheral neuropathy of unknown etiology, amyloidosis, and chronic inflammatory demyelinating polyneuropathy (CIDP). While the sural nerve is most commonly selected, a specimen from the superficial sensory radial nerve (SSRN) is often chosen when the pathology is confined to the upper extremities. The SSRN is typically exposed in the proximal forearm between the brachioradialis (BR) and extensor carpi radialis longus (ECRL) tendons. Here, we describe the technical details for biops…

Tailored frontotemporoorbitozygomatic (FTOZ) craniotomy for a large anterior clinoidal meningioma.

Large anterior clinoidal meningiomas are tumors that arise on the anterior clinoid often compressing and encasing the nearby neurovascular structures such as the carotid artery and the optic nerve. These remain as very challenging cases for neurosurgeons because of the issues concerning preservation of critical structures and gross total excision. In this video submission, we will show a case of a large anterior clinoidal meningioma through a tailored frontotemporoorbitozygomotic craniotomy with emphasis on anterior clinoidectomy and the different corridors that can be obtained by this particu…

Endoscopic clipping of an anterior communicating artery aneurysm.

BACKGROUND: Anterior communicating artery aneurysms are most prone to rupture. Surgically, they are conventionally being managed by a pterional approach. Some neurosurgeons prefer a supraorbital keyhole approach in select cases. Fully endoscopic clipping of such aneurysms is seldom described. METHOD: We clipped an antero-inferiorly directed anterior communicating artery aneurysm endoscopically via a supraorbital keyhole approach. The intraoperative aneurysmal rupture was also managed endoscopically. The patient made an excellent postoperative recovery without any neurological deficits. CONCL…

Brainstem anesthesia during removal operation of ventriculoperitoneal shunt - A case report.

BACKGROUND: Brainstem anesthesia is a transient loss of brainstem function usually associated with retrobulbar block and rarely seen by neurosurgeons. CASE DESCRIPTION: Here, we report a case of brainstem anesthesia during shunt revision operation in a 79-year-old woman. Local anesthesia administered at the end of surgery was thought to have infiltrated the subarachnoid space through a burr hole, causing prolonged unconsciousness and cranial nerves' impairment. Spontaneous resolution occurred during systemic support. CONCLUSION: As brainstem anesthesia may occur by leakage of local anestheti…

Nonopioid Postoperative Pain Management in Neurosurgery.

Neurosurgeons have sought to minimize the use of opioids in neurosurgery. Preoperative medical strategies include methadone and gabapentinoids. Intraoperative strategies include local anesthetic infiltration with bupivacaine, ropivacaine, and lidocaine; scalp block; steroids such as methylprednisolone, triamcinolone, and dexamethasone; ketamine; acetaminophen; ketorolac; liposomal bupivacaine; dexmedetomidine; and performing awake surgery. Postoperative strategies include continuous infusion pumps, wound catheters, and patient-controlled analgesia. Multimodal analgesia may be most effective, w…

Dysplasias in the Child's Spine.

Many of the skeletal dysplasias impact the spinal column causing scoliosis, kyphosis, stenosis, and/or instability at various segments of the spine. Understanding how a particular dysplasia is likely to manifest is important to ensure proper screening and treatment decisions. Equally important is understanding the natural history of specific spine abnormalities, such as awareness of the resolution of the cervical kyphosis common to infants with diastrophic dysplasia. Many patients with rarer dysplasias are best cared for by teams of providers including neurosurgeons, orthopedic surgeons, genet…
