Community-integrated noncommunicable disease service models: lessons from China.

BACKGROUND: Integrated health services are advocated for primary health care to address non-communicable diseases (NCDs), especially in low- and middle-income countries (LMICs). However, evidence of care delivery models and means of scaling up is limited. This study examines community-integrated NCD service models in China, providing evidence on the steps that China took to reorient health services towards primary health care to address the NCD epidemic. METHODS: A systematic review identified and included 20 studies (from 3959 records screened from five databases) conducted in various region

Individual, community and health systems factors influencing time to notification of tuberculosis: situating software and hardware bottlenecks in local health systems.

BACKGROUND: Despite several global interventions, tuberculosis (TB) remains a leading cause of death affecting millions of people globally. Many TB patients either have no access to quality care or go undetected by national health systems. Several multilevel factors account for under-detection of persons with TB. This study sought to explore patient-related software, community and health systems software and hardware factors influencing time to notification of TB in Lusaka District, Zambia. METHODS: This was an exploratory qualitative case study that adopted a software and hardware lens of co

Prenatal and infancy home visitation by nurses: recent findings.

This article describes a 20-year program of research on the Nurse Home Visitation Program, a model in which nurses visit mothers beginning during pregnancy and continuing through their children's second birthdays to improve pregnancy outcomes, to promote children's health and development, and to strengthen families' economic self-sufficiency. The results of two randomized trials (one in Elmira, New York, and the second in Memphis, Tennessee) are summarized, and an ongoing trial in Denver, Colorado, is briefly described. Results of the Elmira and Memphis trials suggest the following: The progra

Beyond the Clinic Walls: Nurse-Led Socialised Care and Health Equity for Vulnerable Populations in Australia.

AIM: This paper critically examines the Australian College of Nursing's proposed socialised healthcare model and the potential of nurse-led socialised care as a paradigm shift for addressing the intersection of medical need and social distress among vulnerable populations experiencing homelessness, domestic violence, poverty, and other disadvantage. APPROACH: This is a position paper. It offers a theoretical commentary and critical reading that situates the proposed socialised healthcare model within frameworks of social determinants of health, health equity, and socially responsive care deli

Portable Negative Pressure Wound Therapy: A Case Study of a Patient Treated in the Community in Jersey.

The aim of this case study is to describe the authors' experience with the use of portable negative pressure wound therapy (pNPWT) in a non-NHS community health care environment. In this context, wound care products are funded by the individual patient. Nurses struggle to reconcile best practice and current evidence-based care when cost implications limit treatment options. In this case, funding for pNPWT was provided by a local charity. This afforded the patient the opportunity to receive best-practice wound care, and significant wound healing, pain reduction and improved patient satisfaction

Nursing and midwifery education, practice, and issues in Israel.

Nurses deliver most of Israel's healthcare services, yet its nurse per population ratio is only 5.9 per 1000, one of the lowest in the developed world. For several years, the managers of the profession have been pushing to upgrade nursing and to move it into the academic sphere. The semiskilled qualification of the practical nurse has been eliminated and training programs for registered nurses are being transformed from diploma training to academic degree programs. A midwifery license is accessible only to registered nurses who take a further 1 year of advanced training and sit the State Midwi

Case report: Autism spectrum disorder as the basis of a feeding problem. A view from nursing care.

This case report presents the nursing approach to a 38-year-old woman diagnosed with restrictive-type anorexia nervosa (AN) since the age of 15 and autism spectrum disorder (ASD) at 35. The patient was admitted due to structured suicidal ideation with a high risk of acting on it, triggered by the experience of significant personal losses. A comprehensive assessment was conducted using Gordon's functional health patterns, identifying the following nursing diagnoses: ineffective health self-management, impaired social interaction, imbalanced nutrition: less than body requirements, and constipati

RESULTS: Integrating an immunization nurse

Exploring Moderation Dynamics: Callous-Unemotional Traits, Parental Practices and Conduct Problems in Juvenile Delinquency.

INTRODUCTION: Callous-unemotional (CU) traits, parental practices and conduct problems (CP) have been consistently identified across numerous studies as significant predictors of the severity and persistence of juvenile delinquency. AIMS: This study seeks to investigate the moderation dynamics underlying the relationships between these variables among Arab incarcerated youth. Specifically, the study aims to (1) explore whether parental practices moderate the relationship between CU traits and CP among incarcerated youth in Jordan and (2) examine the bidirectional moderating influence of CU tr

Association between adverse childhood experiences and burnout among Chinese female nurses.

BACKGROUND: Burnout is a public health issue. Adverse childhood experiences (ACEs) have been linked to mental health problems, but their impact on burnout is unclear. METHOD: A cross-sectional study was conducted using convenience sampling across seven hospitals in Zhejiang Province, China. ACEs were self-reported and categorized by number, type, and context. Burnout was measured using the Maslach Burnout Inventory-Human Services Survey, including status, severity, and dimensions such as emotional exhaus

Evaluating the PM ACTIVAS Nurse-Led Preventive Care Model for Community-Dwelling Older Adults: A Randomized Controlled Trial.

BACKGROUND/OBJECTIVES: Falls among community-dwelling older adults are a major public health challenge and a leading cause of functional decline. Although multifactorial fall prevention strategies are recommended, evidence on comprehensive nurse-led preventive care models in primary health care remains limited. This study evaluated the clinical effects of the PM ACTIVAS nurse-led preventive care model among community-dwelling older adults in routine primary health care. METHODS: This article reports the quantitative component of a broader mixed-methods research programme. A two-arm randomized

Advancing effective street outreach to people experiencing unsheltered homelessness: Concepts for public health nursing.

Homelessness, particularly unsheltered homelessness is a public health emergency in the U.S. Street outreach programs have demonstrated efficacy in connecting people to housing, initiating or retaining people in primary care, improving access to mental health services, and increasing treatment for substance use disorder. Effective outreach is generally characterized by trained, multidisciplinary teams that visit clients where they are, forming trusting relationships over time. Public health nurses working on outreach teams have opportunities to exercise their full scope and standards of practi

Substitution of doctors by nurses in primary care.

BACKGROUND: Demand for primary care services has increased in developed countries due to population ageing, rising patient expectations, and reforms that shift care from hospitals to the community. At the same time, the supply of physicians is constrained and there is increasing pressure to contain costs. Shifting care from physicians to nurses is one possible response to these challenges. The expectation is that nurse-doctor substitution will reduce cost and physician workload while maintaining quality of care. OBJECTIVES: Our aim was to evaluate the impact of doctor-nurse substitution in pr
