Open Reduction vs Distal Femoral Replacement Arthroplasty for Comminuted Distal Femur Fractures in the Patients 70 Years and Older.

BACKGROUND: The ideal management of distal femur fractures in the elderly is unclear. Acute arthroplasty has the theoretical advantage of earlier mobilization. We examined the outcomes of patients 70 years and older who underwent open reduction internal fixation (ORIF) vs distal femoral replacement (DFR) for comminuted, intra-articular distal femur fractures. METHODS: A retrospective review of patients with AO/OTA classification 33C distal femur fractures treated with either ORIF or DFR was performed. Outcomes including all-cause reoperation, length of stay, fracture union, postoperative comp

Open reduction internal fixation has fewer short-term complications than shoulder arthroplasty for proximal humeral fractures.

BACKGROUND: Open reduction and internal fixation (ORIF), hemiarthroplasty (HA), and anatomic or reverse total shoulder arthroplasty (TSA/RTSA) are surgical treatment options for proximal humeral fractures (PHFx). Little is known about comparative complication rates. We aimed to determine whether ORIF for PHFx has fewer 30-day complications than HA and TSA/RTSA and to define independent risk factors for 30-day complications. METHODS: Patients who underwent ORIF, HA, or TSA/RTSA for PHFx between 2006 and 2013 were identified from the National Surgical Quality Improvement Program database. Poten

Risk factors for 30-day postoperative complications and mortality following open reduction internal fixation of distal radius fractures.

PURPOSE: To identify the incidence and risk factors for 30-day postoperative morbidity and mortality following operative treatment of distal radius fractures in a multicenter cohort. METHODS: We retrospectively queried the American College of Surgeons National Surgical Quality Improvement Program database for the years 2005-2011 for cases of closed distal radius fractures treated operatively with internal fixation. Patient demographics, comorbidities, and operative characteristics were analyzed. Thirty-day postoperative complications were identified and separated into categories of major morb

External fixation versus open reduction with plate fixation for distal radius fractures: a meta-analysis of randomised controlled trials.

BACKGROUND: Both external fixation and open reduction with internal fixation (ORIF) using plates have been recommended for treatment of distal radius fractures. We conducted a systematic review and meta-analysis of randomised controlled trials comparing external fixation to ORIF. METHODS: MEDLINE, EMBASE, and COCHRANE databases were searched from inception to January 2011 for all trials involving use of external fixation and ORIF for distal radius fractures. Eligibility for inclusion in the review was: use of random allocation of treatments; treatment arm receiving external fixation; and trea

Functional and radiologic outcome of open reduction and internal fixation of condylar head and neck fractures using miniplate or microplate system.

BACKGROUND: Although the appropriate management of condylar process fractures after miniplate or microplate fixation has been described, there has been no comparative analysis of these plating systems. METHODS: A retrospective review of patients who underwent open reduction and internal fixation (ORIF) of condylar head or neck fractures at our institution from January 2000 through August 2010 identified 70 patients. Of these, 38 were treated with microplates and 32 with miniplates. The primary functional and radiographic results were the maximal mouth opening and condylar bone resorption, res

Lengthening z-osteotomy of the fibula to correct persistent talar shift following open reduction internal fixation of ankle fractures.

BACKGROUND: In cases where ankle fracture union has been compromised by persistent syndesmotic diastasis following open reduction internal fixation, both external rotation and shortening of the fibula have been identified as prominent features. OBJECTIVE: This study reports a technique that uses a z-osteotomy to achieve both lengthening and internal rotation of the fibula to correct persistent talar shift following ankle fracture fixation. METHODS: Four patients with persistent talar shift following open reduction internal fixation for an ankle fracture received z-osteotomy of the fibula to

The long-term outcome of open reduction and internal fixation of stable displaced isolated partial articular fractures of the radial head.

BACKGROUND: Excellent long-term results have been reported for nonoperative treatment of stable isolated displaced partial articular (Mason 2) fractures of the radial head, suggesting that the role of operative treatment can be questioned. This investigation reports the long-term outcome of operatively treated Mason 2 radial head fractures. METHODS: Sixteen patients with stable displaced partial articular (Mason 2) fractures of the radial head not associated with fracture or dislocation of the proximal forearm were evaluated an average of 22 years (range, 14-30 years) after open reduction and

[Management of complex intra-articular distal radius fractures with open reduction and internal fixation with double dorsal locking plates].

AIM: In this prospective study we have analysed the clinical outcome of complex distal radius fractures that were reduced by a dorsal approach and fixed with 2 dorsal locking plates. METHODS: In the period from January 2006 to June 2008 60 patients with a distal intra-articular instable radius fracture, who were treated with the above mentioned procedure, were prospectively included in this study. All fractures showed signs of a complex and dorsal instability. X-rays and CT scans were taken preoperatively. Follow-up examinations and X-rays were conducted after 6 weeks, 6 months and 1 year. Th

Arthroscopic assessment of intra-articular distal radius fractures after open reduction and internal fixation from a volar approach.

PURPOSE: The volar approach with locked plating is a common treatment for intra-articular distal radius fractures. The purpose of this study was to arthroscopically assess the articular surface after internal fixation through the volar approach as a means to evaluate the ability of an extra-articular reduction to anatomically restore the joint surface. METHODS: Sixteen patients with intra-articular distal radius fractures were prospectively enrolled. A volar approach and internal fixation using a locked volar plate was performed. Using a visual analog scale (VAS), the fracture reduction was c

Delayed internal fixation of fractures of the neck of the femur in young adults. A prospective, randomised study comparing closed and open reduction.

We have compared the results and complications after closed and open reduction with ternal fixation in young adults with displaced intracapsular fractures (Garden grades III and IV) of the neck of the femur. We also studied the risk factors which influenced nonunion and the development of avascular necrosis (AVN). A total of 102 patients aged between 15 and 50 years was randomised to receive either closed or open reduction. Both groups were compared for age, gender, time to surgery and posterior comminution as well as for union and complications. Using univariate and multivariate analysis the

Locked fracture dislocation of the calcaneus treated with minimal open reduction and percutaneous fixation: a report of two cases and review of the literature.

Fractures of the calcaneus with associated locked dislocation of the posterior facet have been previously described. Two patients with a calcaneal fracture with a locked dislocation of a portion of the posterior facet were treated with minimally invasive open reduction and percutaneous screw fixation of the fragment with cannulated screws. Both patients had satisfactory reductions and healed the fractures without any soft-tissue complications. This technique can be a useful addition to the armamentarium of the surgeon treating these injuries, especially in the patient at high risk for wound co

Varus collapse of comminuted distal femur fractures after open reduction and internal fixation with a lateral condylar buttress plate.

Twenty-six comminuted distal femur fractures treated with a lateral condylar buttress plate were followed up until fracture union or implant revision (mean follow-up, 26 months). Mean postoperative angle (medial distal femoral angle immediately after surgery) was 96 degrees, and mean final angle (angle at fracture healing or implant revision) was 91 degrees (P = .06). Distal femoral angle decreased 5 degrees or less in 15 patients, 6 degrees to 10 degrees in 8 patients, and more than 10 degrees in 3 patients. Final distal femoral angle was 90 degrees or less in 13 of the 26 fractures: 38% of t

Open reduction and internal fixation of un-united fractures of the distal radius: does the size of the distal fragment affect the result?

Two cohort populations consisting of 13 patients with an un-united fracture of the distal radius in whom the distal fragment had more than 5 mm of subchondral bone supporting the articular surface distal to the site of the nonunion and ten patients with an un-united fracture of the distal radius with a smaller distal fragment were compared. There were no preoperative differences with respect to age, gender, interval between injury and index procedure, preoperative amount of radius tilt, ulnar inclination and ulnar variance, or the preoperative function. Independent of the size of the distal fr

Double-blind randomized prospective study of the efficacy of antibiotic prophylaxis for open reduction and internal fixation of closed ankle fractures.

Antibioprophylaxis has been proven to be efficient for some orthopaedic procedures. However, its efficacy for clean limited procedures with metallic implants is not clear. One hundred twenty-two closed ankle fracture patients undergoing open reduction and internal fixation were randomized, double-blindly, to receive either cephalothin (1 g i.v. every 6 h x four doses) or a placebo starting before tourniquet application. Mean trauma-surgery delay was 30.2 h, and average tourniquet duration was 65.4 min. Four patients (three of 62 on placebo and one of 60 on cephalothin) developed a superficial

Predictors of intermaxillary fixation (IMF) after open reduction and internal fixation (ORIF) in maxillofacial fractures: A retrospective cohort study.

INTRODUCTION: Open reduction and internal fixation (ORIF) has several advantages over closed treatment modalities of maxillofacial fracture by offering an early return to function and better patient acceptance. The goal is to achieve good occlusion without placing the patient in maxillomandibular fixation (MMF). Nevertheless, there are certain situations when MMF becomes imperative in the postoperative period, even after ORIF. This study aims to identify the factors responsible for MMF following open reduction and internal fixation of maxillofacial fractures. MATERIALS AND METHOD: Maxillofaci

Comparison of open and closed reduction and percutaneous pinning for pediatric lateral humeral condyle fractures: A systematic review and meta-analysis.

BACKGROUND: Open reduction and percutaneous pinning (ORPP) is commonly regarded as the primary treatment option for serious displaced lateral condyle fractures of the humerus (LCFs) in children. However, some authors have suggested that closed reduction and percutaneous pinning (CRPP) may be an appropriate method for treating LCFs. This meta-analysis aims to compare the outcomes of these 2 fixation techniques. METHODS: Our study conducted a search of the Pubmed, Embase, and Cochrane Library databases for published research up to October 1, 2022. Our analysis comprehensively compared the opera
