"Orbiting around" the orbital myositis: clinical features, differential diagnosis and therapy.

Orbital myositis (OM) is a rare disease whose clinical heterogeneity and different treatment options represent a diagnostic and therapeutic challenge. We aim to review the state of knowledge on OM, also describing a cohort of patients diagnosed in our centre, to highlight some remarkable clinical features. A literature review was conducted in PubMed and Medline databases. The herein described cohort is composed of seven OM patients, diagnosed according to clinical, laboratory and neuroradiological features, whose clinical data were retrospectively analysed. OM is a non-infectious, inflammatory…

Partial orbit irradiation achieves excellent outcomes for primary orbital lymphoma.

PURPOSE: Primary radiation therapy (RT) achieves excellent local control and overall survival when treating localized orbital lymphoma. However, evidence supporting irradiation of partial orbit volumes to spare nearby critical structures is lacking. We sought to investigate outcomes for patients with localized orbital lymphoma treated with partial orbit irradiation. METHODS AND MATERIALS: We retrospectively reviewed patients with orbital lymphoma treated with RT at our institution who met our inclusion criteria: biopsy-confirmed, low-grade lymphoma, localized disease, partial orbit treatment …

A case of odontogenic orbital cellulitis causing blindness by severe tension orbit.

We report a very rare case of odontogenic orbital cellulitis causing blindness by severe tension orbit. A 41-yr old male patient had visited the hospital due to severe periorbital swelling and nasal stuffiness while he was treated for a periodontal abscess. He was diagnosed with odontogenic sinusitis and orbital cellulitis, and treated with antibiotics. The symptoms were aggravated and emergency sinus drainage was performed. On the next day, a sudden decrease in vision occurred with findings of ischemic optic neuropathy and central retinal artery occlusion. Deformation of the eyeball posterior…

MRI of orbital cellulitis and orbital abscess: the role of diffusion-weighted imaging.

OBJECTIVE: Our aims were to describe the role of diffusion-weighted imaging (DWI) in detecting abscess as a complication of orbital cellulitis and to assess whether abscess can be diagnosed with a combination of conventional unenhanced sequences and whole-brain DWI with parallel acquisition. Nine cases of orbital cellulitis imaged with MRI were retrospectively reviewed, including six cases with pyogenic abscess. CONCLUSION: In this preliminary study, DWI improved diagnostic confidence in nearly all cases of orbital abscess when used in conjunction with contrast-enhanced imaging. DWI also conf…

Acute traumatic orbital cerebrospinal fluid cystocele mimicking orbital abscess.

Posttraumatic cranio-orbital cerebrospinal fluid (CSF) fistula is rare, occurring when there is a communication between orbit and subarachnoid space. This communication can be caused by orbital tumors with intracranial extension or can be a result of orbital surgery. Low suspicion for traumatic orbital CSF fistula can lead to delay in diagnosis, which increases the risk of meningitis. We report a cranio-orbital CSF cystocele and fistula that mimicked orbital cellulitis in a 3-year-old boy following an orbitocranial penetrating injury.

[Case of invasive sino-orbital aspergillosis developing orbital apex syndrome].

Sino-orbital invasive aspergillosis has been regarded as a lethal disease. The authors report a case with a successful treatment result. A 65 year-old woman, with mild diabetes mellitus, presented progressive right visual disturbance, diplopia, ptosis, and severe periorbital pain over a period of 2 weeks. MR images with gadolinium contrast showed a heterogeneously enhanced mass extending from the right orbital apex to the cavernous sinus. Despite steroid pulse therapy, her symptoms progressed. An open biopsy revealed invasive sino-orbital aspergillosis. Intravenous and oral antifungal agents w…

Orbital perforating branch of the infraorbital artery: an important landmark in orbital surgery.

The orbital branch of the infraorbital artery is an important surgical landmark but is frequently omitted from texts that discuss orbital anatomy and surgical technique. This report reviews the anatomy of this artery to familiarize the reader with its existence and location to reduce the risk of intraoperative hemorrhage from this vessel.

Classification and surgical management of orbital fractures: experience with 111 orbital reconstructions.

Orbital skeletal injuries are frequently associated with other significant injuries and require a substantial surgical effort to correct. The use of a unified classification of orbital injury may better predict the surgical effort required to correct such injuries and help with future comparisons of results. In an attempt to summarize the principles of reconstruction of the orbital skeleton following trauma and introduce a unified classification system for orbital injuries, a retrospective review of all consecutive orbital reconstructions in a tertiary care teaching hospital was conducted. The…

Persistent infra-orbital nerve hyperaesthesia after blunt orbital trauma.

BACKGROUND: Hyperaesthesia involving the infra-orbital nerve is rarely associated with orbital floor fracture. METHODS: An 11-year-old boy and a 34-year-old man had persistent and severe hyperaesthesia in the distribution of the infra-orbital nerve 12 and 26 months, respectively, after blunt orbital trauma. RESULTS: Surgical decompression of the infra-orbital nerve resulted in rapid and complete resolution of the hyperaesthesia in both cases. CONCLUSION: Persistent hyperaesthesia of the infra-orbital nerve is another indication for exploration of the orbital floor following blunt orbital tr…

Stimulation of orbital growth by the use of expandable implants in the anophthalmic cat orbit.

We evaluated the efficacy of expandable orbital implants to stimulate bone growth in the anophthalmic cat orbit. Eighteen cats unilaterally enucleated at 2 weeks of age received either expandable orbital implants (groups A1 and A2), solid silicone sphere implants of 12 mm or 8 mm (groups B1 and B2), or no implant (group C). Those cats with expandable implants (group A) had the implant size increased by 0.5 ml injections of saline at 2-week intervals starting at 8 weeks of age until a final volume of 4 cc was reached. Four of the expandable implants were found to be only partially inflated at 2…

Deformities of the midface resulting from malunited orbital and naso-orbital fractures.

In midfacial fractures the crucial area is the upper portion of the midfacial skeleton which lodges the orbits, the interorbital space with its anatomic relationship with the anterior cranial fossa, the lacrimal apparatus, the levator muscle, and the skeletal structures of the nose. Because of the severity of the injuries in a multisystem injured patient, these fractures may be neglected or receive inadequate primary treatment. In some cases the damage is so extensive that primary treatment can only accomplish part of the task. Late reconstruction is necessary, therefore, and is the subject of…

Direct wavepacket dynamics with spin-orbit coupling: simulation of thioformaldehyde.

The non-radiative photophysical phenomenon known as intersystem crossing is simulated, for the first time, in fully coupled direct nuclear wavepacket dynamics based on Gaussian basis functions (DD-vMCG method). As a case-study, the ultrafast photoinduced dynamics of thioformaldehyde is studied using this method. Working in the spin-diabatic basis, the simulations presented here incorporate the effect of spin-orbit coupling, thus allowing the transfer of population density between states of different spin multiplicities (in this case, singlets and triplets). Even though no significant intersyst…

Observation of Spin-Orbit Coupled Polariton Vortices at Room Temperature.

Spin-orbit coupling gives rise to intriguing physical phenomena in bosonic condensates such as formation of stripe phases and domains with vortex arrays. However, how the non-Hermiticity affects the spatial distribution of spin-orbit coupled topological defects such as the vortex pair is still challenging to study. In the present work, we realize a nonequilibrium room-temperature exciton polariton condensate within a microdisk potential in a liquid crystal (LC) microcavity. We use the interplay of TE-TM mode splitting and Rashba-Dresselhaus spin-orbit coupling (RDSOC) to realize electrically t…
