Both nonsurgical and surgical options are available for the treatment of developmental hip dysplasia and dislocation. The advantages, pitfalls, and techniques for using the Pavlik harness should be thoroughly examined before treatment. Other closed and open treatments are aimed at concentric reduction and prevention of residual subluxation and dysplasia. Early diagnosis and treatment lead to the best long-term results for these conditions.

CONTEXT: Hallux valgus is a common foot deformation in adults, but evidence for effectiveness of surgical and conservative treatments for this condition is limited.
OBJECTIVE: To compare the effectiveness of surgical and orthotic treatment with no treatment in patients with hallux valgus.
DESIGN AND SETTING: Randomized controlled trial conducted in 4 general community hospitals in Finland in 1997-1998, with a follow-up period of 12 months.
PARTICIPANTS: Two hundred nine consecutive patients (mean age, 48 years; 93% women) with a painful bunion and a hallux valgus angle 35 degrees or less.

Investigators have shown that significant segmental motion occurs in patients immobilized in halo vests. One cadaver study showed that this is decreased with a posterior pad added to the construct. The Combitube is a twin lumen device designed to establish the airway after blind insertion. Under general anaesthesia a rigid cervical collar was used to immobilise the neck in 15 ASA 1 and 2 patients. Insertion of the Combitude airway was then attempted. In 10/15 (66%) patients, blind insertion was not possible. In 5/15 (33%) successful blind insertions the Combitube entered the oesophagus on each occasion. Keeping abreast of the latest orthotic products is challenging. Commercial orthoses need to be critically evaluated for their applicability to the person with rheumatic disease. Custom-made orthoses play an invaluable role in achieving a wide range of therapeutic goals: improving function and pinch, stabilizing individual joints, preventing positional contractures, protecting joints from trauma, stretching the intrinsic muscles, correcting joint- and soft-tissue contractures, and controlling inflammation.

The advantages of the sonography of the hip joint in the diagnosis and treatment of hip dysplasia in 1992 "policlinic" children and 1100 newborns are discussed and compared with radiological methods. The treatment concept at the Orthopädische Universitätsklinik Bonn is described as well as its modification after introduction of the sonography in the hip dysplasia screening-examination. These results confirm the necessity of making an early diagnosis. The osteonecrosis of the hip as a complication of the treatment of hip dislocation is discussed.

A retrospective study was done on the effects of a change in the design of ankle-foot orthoses used in the treatment of 29 hemiplegic children. Nine of 13 children treated with plantarflexed ankle-foot orthoses required surgery, compared with only one of 16 with dorsiflexed ankle-foot orthoses. This difference is highly significant.

The results of reconstructive procedures for the treatment of burn scar contractures of the feet in 55 children undergoing 90 operations were reviewed. The patients were treated in all but one case by release of the contracture band with placement of a skin graft in the resulting defect. Between December 1982 and the end of January 1987, a total of 21,082 hip sonographies were performed at the authors' clinic to establish whether or not screening of newborns, including sonographic follow-up, was desirable. The incidence of sonographically detected neonatal dysplasia up to the eight week of life was 2.23%; the incidence of dislocation was 0.01%. Hip types IB and IIA have a tendency to deteriorate in the eighth and twelfth weeks. A prospective study of the sagittal plane motion of the cervical spine, stabilized with a halo-vest was performed in 31 consecutive patients with unstable cervical spine injuries. Motion was measured in lateral radiograms taken with the patient in different positions and while performing various exercises.
