Of balloon axilloscopy and avoidance of iatrogenic injury to the long thoracic nerve.

BACKGROUND: Laparoscopic removal of axillary lymph nodes is possible and affords an excellent view of all structures, allowing preservation of vessels and nerves. The technique uses pediatric trocars and a lifting device to maintain the newly created axillary space. OBJECTIVE: To prove that a newly developed technique of balloon axilloscopy can be performed using only one 10-mm and two 5-mm standard trocars and constant carbon dioxide flow to preserve the axillary space and that preservation of all nerves and vessels is possible with this approach. DESIGN: Prospective study on 4 fresh-frozen

Prevalence of the sternalis muscle in a sample of routinely dissected human cadavers.

PURPOSE: Sternalis muscle is an uncommon muscle head attaching between the sternoclavicular joint superiorly and the costal cartilage near the xiphoid process. METHODS: In this study, 36 cadavers (18 male, 18 female) were obtained through the Anatomical Board of the State of Florida and complete, routine musculoskeletal dissection was performed while adhering to all relevant laws and practices. RESULTS: Two examples of the sternalis muscle belly (5%) were identified, one in a black male and one a white female. The sternalis muscles were bilateral in both cases and each was formed of a single

Osteosynthetic thoracic stabilization after complete resection of the sternum.

We present a case of a 66-year-old diabetic patient with chronic non-healing sternum after CABG operation. After four attempts of sternum refixation within 2 years, the need of bone debridement resulted in a nearly complete loss of sternum. Finally, a mesh graft and a pectoralis muscle flap were used to bridge the sternal space. Despite good wound healing, the thoracic instability led to intolerable chest pain persisting over the next years. In this case report, we describe the successful thoracic stabilization by using transverse plate fixation, which resulted in perfect thoracic stabilizatio

[Dextrocardia and Poland syndrome in a 59-year-old patient].

Poland syndrome consists of unilateral absence of the sternal head of the pectoralis major muscle, ipsilateral symbrachydaktylia, and occasionally associated other malformations of the anterior chest wall, mammilla, and mamma. To our knowledge a simultaneous occurrence with dextrocardia was reported in seven patients. We report on an additional patient with this unusual coincidence. A 59-year old man showed mild symbrachydaktylia of the left hand and deformity of the left part of the anterior chest wall, both present since birth. We found an additional dextrocardia with situs solitus, d-loop,

Longitudinal trajectory of thoracic CTA-derived sarcopenia stratifies mortality risk in transcatheter aortic valve replacement.

BACKGROUND: Although sarcopenia is a known predictor of outcomes in cardiovascular diseases, its disease trajectory, including improvement, stability, or worsening, following transcatheter aortic valve replacement (TAVR) remains insufficiently explored. This study pioneered using pectoralis muscle index (PMI) from routine thoracic computed tomography angiography (CTA) to track sarcopenia changes post-TAVR and determine prognostic value. METHODS: This single-center cohort included consecutive TAVR patients with serial thoracic CTA at baseline and 1 year. PMI was calculated as pectoralis muscle

Significance of the Lateral Thoracic Artery in Pectoralis Major Musculocutaneous Flap Reconstruction: Quantitative Assessment of Blood Circulation Using Indocyanine Green Angiography.

Free tissue transfer is the preferred reconstruction option in most major head and neck reconstructions. The pectoralis major muscle musculocutaneous (PMMC) flap is commonly used in salvage of necrotic free flaps and is the first choice for patients who are not candidates for free flaps. The lateral thoracic artery (LTA), which is thought to contribute to blood perfusion of the inferior and lateral mammary area, is not preserved in a conventionally harvested PMMC flap. With regard to blood supply, it has been suggested that the LTA should be preserved, in addition to the pectoral branch of the

The anatomy of the pectoral nerves and their significance in brachial plexus reconstruction.

Twenty-nine brachial plexuses from 13 embalmed and 5 fresh cadavers were examined under x3.5 loupe magnification to collect systematic and topographic anatomical data regarding the lateral and medial pectoral nerves. Additionally, nerve biopsy specimens were harvested in 5 fresh cadavers to obtain histomorphometric data. In all dissections the pectoral nerves exited at the trunk level as 3 distinct nerves. The superior pectoral nerve (from the anterior division of the superior trunk) commences just distal to the suprascapular nerve and courses laterally to innervate the lateral clavicular port

Biomechanical analysis of wheelchair propulsion for various seating positions.

The pattern of propulsion was investigated for five male paraplegics in six seating positions. The positions consisted of a combination of three horizontal rear-wheel positions at two seating heights on a single-purpose-built racing wheelchair. To simulate wheelchair propulsion in the laboratory, the wheelchair was mounted on high rotational inertia rollers. For three trials at each seating position, the subjects propelled the designed wheelchair at 60 percent of their maximal speed, which was determined at the beginning of the test session. At each trial, the propulsion technique of the subje

Comparison of Shoulder Protraction Strength and Electromyography Activity of Serratus Anterior and Pectoralis Major in Subjects With or Without a Winged Scapula.

CONTEXT: A winged scapula (WS) is associated with faulty posture caused by weakness of the serratus anterior (SA), which mainly acts as a scapular stabilizer muscle. It is important to accurately assess and train the SA muscle with a focus on scapula stabilizers during musculoskeletal rehabilitation of individuals with a WS. OBJECTIVE: The authors examined muscle activity in the SA and pectoralis major (PM), upper trapezius (UT), and anterior deltoid (AD) as well as shoulder protraction strength during isometric shoulder protraction in individuals with and without a WS. DESIGN: Cross-section

DDD/DDT pacemakers in the treatment of ventricular tachycardia.

The possibility of adapting multimode dual-chamber pacemakers for programmed and burst stimulation was explored in a group of patients with ventricular tachycardia. The potential usefulness and pitfalls of these pacemakers were exemplified in one patient, presented in detail. The implanted DDD pacemaker was programmed to an all-synchronous mode (DDT), permitting programmed ventricular stimulation through synchronization with chest wall stimulation provided by a standard external programmable stimulator. With the ventricular sensing refractory period shortened to 200 ms, both programmed electri

Isolated Pectoralis Major and Tricep Atrophy Secondary to Neuralgic Amyotrophy.

Neuralgic amyotrophy (NA), or Parsonage-Turner syndrome, describes a spectrum of relatively rare peripheral neuropathies characterized by acute pain followed by patchy, multifocal sensory disturbances, weakness, and atrophy, predominantly in the upper extremity. While typically affecting the upper brachial plexus, NA can also present with selective involvement of distal nerve branches. This case report details a unique presentation of NA in a 39-year-old male bodybuilder who developed isolated weakness and atrophy of the left pectoralis major and a single head of the left triceps following a p

The Pectoral (PECS) Regional Block: A Scoping Review.

Among the various surgical procedures, breast surgeries rank as a frequently conducted procedure. Interfacial blocks such as the Pectoral (PECS) block became possible with the currently available knowledge on innervations and ultrasound. Interfacial blocks target the deep fascial planes, which are potential spaces for injecting local anesthetics. The Pectoral I (PECS I) consists of the injection of local anesthetics in the plane between the pectoralis major and minor muscles. The PECS II block, a modified version of the block, is achieved by adding another, deeper injection in the plane betwee

Acute muscle excitation response across various bench press ranges of motion.

This study examined muscle excitation using surface electromyography (sEMG) during different ranges of motion (ROM) in the bench press. Given the limited research on upper-body muscle excitation across ROMs, this study aimed to establish reference values for muscle excitation in distinct bench press ROMs. Nineteen male participants performed a 10-repetition maximum (10RM) bench press across three ROMs: full ROM, upper-half ROM, and lower-half ROM. sEMG measurements were taken on the pectoralis major (PM), triceps brachii (TB), and anterior deltoid (AD). Mean and peak EMG amplitudes were analyz

McCleery Syndrome Caused by Pectoralis Minor Hypertrophy Treated with Multimodal Physical Therapy-A Case Report.

We present a case of a healthy young male professional water polo player who presented with swelling and pain in the upper arm and elbow after vigorous exercise. Diagnostic workup included an MRI and dynamic duplex ultrasound, which revealed compression of the axillary vein by a hypertrophic pectoralis minor muscle without thrombosis, constituting McCleery syndrome. This is a rare entity within the multiple thoracic outlet syndrome aetiologies. Taking a detailed history and physical examination complemented with diagnostic imaging are vital to the diagnosis. Afterward, the patient was treated
