UNLABELLED: Mycetoma is a pathological process in which eumycotic (fungal) or actinomycotic causative agents from exogenous source produce grains. It is a localized chronic and deforming infectious disease of subcutaneous tissue, skin and bones. We report the first case of eumycetoma of the oral cavity in world literature.
CASE REPORT: A 43-year-old male patient, complaining of swelling and fistula in the hard palate. On examination, swelling of the anterior and middle hard palate, with fistula draining a dark liquid was observed. The panoramic radiograph showed extensive radiolucent area inv

INTRODUCTION: Microorganisms are able to survive and induce persistent infection in periapical tissues. The aim of this study was to investigate the composition of the microflora of persistent apical periodontitis lesions.
METHODS: Twenty apical lesion samples were obtained from 20 patients with chronic apical periodontitis by root end surgery and processed using aerobic or anaerobic culture techniques. All isolated strains were identified by 16S ribosomal DNA sequence analysis.
RESULTS: Seventy-four strains were isolated, belonging to 31 bacterial species obtained from the 20 apical lesions…

Five cases of transplantation of fully developed third molars to second or first molar sites have been reported. Three of the transplants were in the mandibular molar region and two were in the maxillary molar region. There has been long-term follow-up and evaluation of the patients in these cases up to 13 years postoperatively. None of the patients had any complaint about the transplanted tooth at the long-term follow-up visit. In all five cases, at the long-term follow-up visit, the periodontal tissues were in excellent condition and there was growth of new attachment, as demonstrated by lam

STATEMENT OF PROBLEM: Teeth deemed nonrestorable because of inadequate ferrule and extensive structural loss, often from deep caries, fractures, or resorptive defects, are routinely planned for extraction and implant placement. Long-term clinical outcome data for a standardized surgical extrusion workflow designed to regain restorability while maintaining biologic stability are limited.
PURPOSE: The purpose of this clinical study was to assess clinical outcomes of surgical extrusion using the Seif surgical extrusion protocol (SSEP) as a tooth preservation alternative to extraction in severely

OBJECTIVES: This prospective clinical study investigates long-term survival and clinical parameters of non-submerged implants with large buccal dehiscences treated with a deproteinized bovine bone mineral xenograft and a non-resorbable membrane in a one-stage approach.
MATERIAL AND METHODS: Sixteen consecutive non-submerged implants (ITI Straumann) were installed in narrow alveolar ridges in 13 patients (age range: 25-61 years). All patients were non-smokers. On the buccal site the bone dehiscence ranged between 3 and 9 mm. Primary stability was achieved in all but one implant. The exposed th

INTRODUCTION: Traumatic injury to the teeth can cause horizontal root fractures and inflammatory root resorptions (external and internal). Traditionally, traumatized teeth with horizontal root fractures resulting in pulp necrosis and inflammatory root resorptions are treated with conventional root canal therapy.
METHODS: A 15-year-old boy had a history of traumatic injury to mature tooth #8 resulting in horizontal root fracture and pulp necrosis of the coronal fragment. A 7-year-old girl suffered an avulsion injury to immature tooth #9, which developed inflammatory replacement resorption and

INTRODUCTION: Inflammatory cytokines are associated with decreased insulin signal transduction. Moreover, local oral inflammation, such as that accompanying periodontal disease, is associated with insulin resistance and type 2 diabetes mellitus. The aim of this study was to evaluate the effect of periapical lesions (PLs) on insulin signaling and insulin sensitivity in rats. We hypothesized that PLs alter systemic insulin signaling and insulin sensitivity via elevated plasmatic tumor necrosis factor α (TNF-α).
METHODS: Wistar rats were divided into control (CN) and PL groups. PLs were induced

Radiological findings and clinical data from existing treatment records have been examined using X2-test for clarification of possible relationships between pathological reaction of the periodontal ligament to continuous application of force and different patient-specific and therapeutic factors. It was shown that 70% of the total group have pathological changes following fixed appliance therapy. Apical root resorption usually could be diagnosed five times more frequently than marginal bone loss. No age-specific relationship was noted. The findings confirmed that active intrusion, extensive to

This case report describes the extraction of 3 mandibular posterior teeth (teeth #19, 20 and 21) with extensive invasive external cervical resorption (ECR) followed by guided double auto-transplantation of immature mandibular third molars (teeth #17 and 32). A 17-year-old male with a history of orthodontic treatment reported with intermittent pain in the mandibular left posterior region. Clinical and radiographic examination revealed extensive tooth tissue loss in teeth #19, 20 and 21 that was diagnosed as class 3 ECR (Patel 3Dp for teeth #20 and 21, 3Bp for tooth #19). Guided double auto-tran

Gingival fenestration is a relatively uncommon soft tissue lesion in which the root apex is exposed in the oral environment after the destruction of the overlying buccal bone plate and mucosa. At present, no clear etiology or treatment guidelines exist for gingival fenestration. This article reports three successfully treated cases of gingival fenestration associated with chronic periapical infection and can help contribute to treatment guidelines for gingival fenestration. All cases were treated with apicoectomy in conjunction with a connective tissue graft (CTG), and the first steps of each

Pulpectomy has always been a treatment choice when irreversible damage to the pulpal and periapical tissues occurs, but in a pediatric patient, expecting cooperation and adequate chair side time is a huge task as they fear the dentists and the instruments. Hence, it was required that there come an advanced treatment protocol that helps overcome the drawbacks of the existing treatment modality. A new therapeutic strategy called lesion sterilization and tissue repair (LSTR) is revolutionizing how dental lesions are treated today. Through a non-invasive antimicrobial treatment, this cutting-edge …

AIM: This report addresses the management of a large persistent discharging lesion in an 11-year-old boy. The report describes the use of aspiration-irrigation technique for the management of immature necrotic tooth with persistent discharge after a failed regenerative procedure.
BACKGROUND: Regenerative endodontics aim to provide an increase in root canal width, length, and in apical closure. Alternative procedures, such as apexification, should be attempted when regeneration fails. If the canal cannot be dried to persistent discharge, the aspiration-irrigation technique can be used. The tec

Root perforation results in the communication between root canal walls and periodontal space (external tooth surface). It is commonly caused by an operative procedural accident or pathological alteration (such as extensive dental caries, and external or internal inflammatory root resorption). Different factors may predispose to this communication, such as the presence of pulp stones, calcification, resorptions, tooth malposition (unusual inclination in the arch, tipping or rotation), an extra-coronal restoration or intracanal posts. The diagnosis of dental pulp and/or periapical tissue previou…

PURPOSE: To propose a treatment protocol for management of implant candidates suffering from active lichen planus.
MATERIALS AND METHODS: Twenty-three patients who were diagnosed with active lichen planus were referred to a dental practice seeking dental implants. Patients received dental implants using open flap technique and sequential osteotomy. After a healing time of 6 weeks, implants were put into occlusal contact using interim prostheses. Of the 55 inserted implants, 42 failed after a short clinical service time (7 to 11 weeks). Failed implants were surgically removed, and osteotomy si

OBJECTIVES: Condensing osteitis is a radiographic finding, but with no reported histologic investigations in humans. The objectives of this study were to evaluate and describe histologically condensing osteitis in human cadaver jaws. Patterns of bone formation and presence/absence and nature of inflammation were examined.
METHODS: Specimens of mandibles and maxillas were obtained from cadavers and examined radiographically. Those periapical areas with characteristics of condensing osteitis were removed en bloc, decalcified, and processed for light microscopy. For comparison, specimens that sh

Dental anesthesia is useful and important for the procedures of tooth extraction, oral tissue biopsy, periapical and periodontal surgery and more others. However, needle breakage during dental anesthesia, presents a considerable challenge in its clinical management. Retained fragments can lead to pain, trismus, dysphagia, and infections, and may migrate into deeper spaces, increasing the complexity and risk upon its retrieval. To avoid such consequences, prompt removal of the broken fragment is often advocated. However, no standard guidelines were developed. This review provides a comprehensiv
