We report a case of thyrotoxic hypokalemic periodic paralysis occurring in a black man. This previously healthy 41-year-old man had a 3-year history of recurrent paroxysmal episodes of paresis in the upper and lower extremities proximally, often occurring after heavy exercise. The patient had no history of hyperthyroidism, nor family history of periodic paralysis. This case reveals a particularly rare characteristic of this disorder in which periodic paralysis is the presenting symptom of hyperthyroidism. Continuous electrocardiographic monitoring was performed in 20 term parturients during labor, vaginal delivery, and recovery. Mean duration of monitoring was 13.37 h. The need for a routine preoperative intravenous cholangiogram (IVC) has been controversially discussed. We decided to assess if preoperative criteria such as history, clinical examination or laboratory findings could be used for selective indication for preoperative IVC. SETTING: Academic Medical Centre (Amsterdam) and University Hospital Dijkzigt (Rotterdam), the Netherlands.
METHOD: The medical records of all new leprosy patients in the period 1970-1991 were analysed.

Lyme disease, a tick-transmitted spirochetal illness caused by Borrelia burgdorferi, usually begins with a characteristic erythema chronicum migrans accompanied by flu-like symptoms. This phase may later be followed by meningitis, neuritis, carditis or arthritis. Congenital abnormalities due to maternal infection during pregnancy have been described. We report on a case of a 36-year old V gravida III para. After a normal pregnancy and a Cesarean section the patient developed postpartal an acute Lyme arthritis.

The frequency of thyrotoxicosis in peace and war time periods was compared in the same region with the same population. In one year peace and war time periods 54 and 87 cases of thyrotoxicosis were found, respectively, which indicates the increase for 62%. The results point to the conclusion that psychic trauma increases the number of patients affected by thyrotoxicosis.

Colonoscopy was performed preoperatively and 1, 2, 3 and 7 days following a left hemicolectomy with a primary anastomosis in 11 dogs. The remaining colon and anastomosis were adequately visualized. Maximal intraluminal pressures achieved were from 15 to 35 mmHg. No leakage of air or intra-abdominal abscesses were present. The authors observed 6 patients, who developed acute cholecystitis at day 2--5 after the operation. In 3 patients, an operation on the abdominal organs was performed, in 3--the urologic intervention. The leading symptoms of postoperative cholecystitis are the following: epigastric pain, stable intestinal paresis, high body temperature. The authors recommend a wide use of the ultrasound investigation of the abdominal cavity, and in its negative results, or in absence of the apparatus--laparoscopy. All the patients were reoperated. One patient died.
