BACKGROUND: A transient endothelial hyperpermeability is a hallmark of severe dengue infections. Sphingosine-1-phosphate (S1P) maintains vascular integrity and protects against plasma leakage. We related plasma S1P levels to dengue-induced plasma leakage and studied mechanisms that may underlie the decrease in S1P levels in dengue.

OBJECTIVE: Our objective was to examine the effects of itraconazole on the pharmacokinetics and cortisol-suppressant activity of budesonide administered by inhalation.
METHODS: In a randomized, double-blind, 2-phase crossover study, 10 healthy subjects took 200 mg itraconazole or placebo orally once a day for 5 days. Factors which influence the plasma drug concentrations in whole blood have been investigated in-vitro using human blood containing radio-labelled phenytoin or chlorpromazine. Blood glucose, blood lactate, plasma free fatty acids, plasma alpha-amino nitrogen, plasma free amino acids and blood urea nitrogen were measured in 15 hypothermic preterm infants (mean rectal temperature: 34.5 +/- 0.28 degrees C) during a 2-hour 'warming-up' period following admission to our neonatal intensive care unit. Hypothermia was associated with lactacidemia and hyperalaninemia. The increase in body temperature of the infants resulted in the decrease of lactacidemia and hyperalaninemia. Normal levels of other metabolites remained unchanged.

Athough in general, measurement of renal vein renin appears to give a good prediction as to the subsequent response to surgery, its main value lies in its ability to reflect changes in renal plasma flow; true changes in renin secretion rate being much more difficult to detect. Although it is a little early to say how much information can be derived from saralasin infusions, caution must be exercised in necessarily assuming that the test accurately reflects subsequent surgical response.

Clinical significance and diagnostic value of plasma estradiol were assessed in hypothalamic-pituitary, adrenal and gonadal disorders.
