[Surgical treatment and prognostic analyses of bronchopulmonary carcinoid tumors].

OBJECTIVE: To explore the surgical treatment and prognostic factors of bronchopulmonary carcinoid tumors (BPC). METHODS: The clinical data of 65 patients undergoing surgery for BPC from May 1999 to December 2007 were reviewed retrospectively. The predictors of univariate and multivariate analyses included gender, age, smoking history, pathological type and tumor stage. RESULTS: The procedures included segmentectomy (n = 1), lobectomy (n = 36), bilobectomy (n = 6), sleeve resection (n = 4), pneumonectomy (n = 11), carinal resection (n = 2), bronchoplastic resection (n = 4) and exploratory tho…

Clinical manifestation of mediastinal fibrosis and histoplasmosis.

We treated 20 patients thought to have mediastinal fibrosis secondary to Histoplasma capsulatum. All but 1 were symptomatic. The most common symptoms were dyspnea (8), hemoptysis (6), postobstructive pneumonia (5), and superior vena caval obstruction (2). Nine patients had severe stenosis of the trachea, carina, or main bronchus. Special stains identified Histoplasma capsulatum in surgical specimens in 9 patients. Surgical procedures were done for 18 of 20 patients (resection of subcarinal mass, 6; right middle and lower lobectomy, 5; carinal pneumonectomy, 4; esophagoplasty, 4; sleeve resecti…

[Severe pulmonary mycobacteriosis caused by Mycobacterium simiae].

We report a case of pulmonary mycobacterial infection due to Mycobacterium simiae followed between 1968 and 1985 and leading to unilateral pulmonary destruction. Formal bacteriological proof was obtained by culturing from the pneumonectomy specimen. None of the usual features of immunosuppression were found. This is the first case suffering from this germ described in this part of the world.

Management of postpneumonectomy syndrome with a bronchoscopically placed endobronchial stent.

Postpneumonectomy syndrome is a rare complication of right pneumonectomy characterized by marked shift and counterclockwise rotation of the heart and mediastinum into the evacuated hemithorax. Typically the left primary bronchus is draped over the aorta or spine and is markedly compressed. We describe one patient with postpneumonectomy syndrome successfully managed nonsurgically with a bronchoscopically placed Silastic endobronchial stent.

[Massive traumatic hemoptysis].

A case is reported of a patient who suffered a rupture of one lung as result of thoracic trauma. This gave rise to respiratory distress with massive haemoptysis which was initially treated with a double-lumen endotracheal tube, with separate lung ventilation, a chest drain and massive transfusion. A haemostatic pneumonectomy had to be performed because of the persisting and profuse bleeding.

Gastric Volvulus After Left Lower Lobectomy.

Gastric volvulus is a rare complication after pulmonary resection. To date, only eight cases of postpulmonary resection gastric volvulus have been reported in the English literature, and several of these patients underwent left pneumonectomy or had hiatal hernia. This report describes a case of postlobectomy gastric volvulus in a 73-year-old woman without hiatal hernia.

Unusual late presentation of metastatic extrathoracic thymoma to gastrohepatic lymph node treated by surgical resection.

In advanced stages, thymic tumors tend to spread locally. Distant metastatic disease is rare. We present the first report of single metastatic abdominal lymph node in a 37-year-old female patient and 5 years after an extrapleural pneumonectomy for stage IV thymoma followed by radiotherapy with no other evidence of abdominal disease successfully treated by robotic surgical resection.

Prognostic coronary surgery in a case of malignant mesothelioma previously managed with trimodality treatment.

We report a case of malignant pleural mesothelioma treated with trimodality treatment. At three years after the extrapleural pneumonectomy, coronary artery revascularisation surgery for NSTEMI was performed in view of favourable long term prognostic and survival outcome. Five years following pleuropneumonectomy there is no clinical or radiological evidence of mesothelioma and the patient remains free of cardiac symptoms.

Closure of a broncho-pleural fistula using an atrial septal defect occluder.

Broncho-pleural fistulae (BPF) are recognised as a rare complication following pneumonectomy. We describe a patient, who after failing conservative treatment, underwent closure of a persistent fistula with an atrial septal defect (ASD) occluder. Additionally we review the literature regarding management of BPF and the emerging role of cardiac defect closure devices as a possible treatment option.

[Origination of mediastinal hernia in late follow up period after performance of pulmonectomy].

In 25-40 years after pneumonectomy conduction the mediastinal hernia (MH) occurred in 62.4% of 210 patients. Principal clinico-radiological signs of MH were vicarious hypertrophy and true pulmonary emphysema. Vicarious hypertrophy of single lung is forming, as a rule, in patients, operated on in childhood, is characterized by favourable clinical course and constitutes the MH compensated stage.

Pulmonary actinomycosis--a master of disguise.

Three cases of pulmonary actinomycosis are described. In all three cases, an initial diagnosis of pulmonary malignant neoplasia was made. The correct diagnosis was made histologically after the three patients underwent either lobectomy or pneumonectomy. All patients recovered after appropriate therapy. Pulmonary actinomycosis should always be considered in the differential diagnosis of pulmonary neoplasia, especially in young adults.

Postpneumonectomy syndrome after left lung resection.

A female infant with a right-sided aortic arch and ligamentum arteriosum underwent a left pneumonectomy for perinatal interstitial emphysema. Radiographic, bronchoscopic, and surgical evidence of compression of the right bronchial tree by the aorta, ligamentum arteriosum, right pulmonary artery, and thoracic spine subsequently developed. This report documents a rare example of postpneumonectomy syndrome following left lung resection.

Recurrent laryngeal nerve paresis postpneumonectomy contralateral to site of surgery.

Recurrent laryngeal nerve (RLN) injury is a serious complication of thoracic surgery that increases morbidity and mortality. It is important to consider the long-term effects of mediastinal shift and the possibility of postpneumonectomy syndrome on RLN function. This report documents a unilateral vocal fold paresis contralateral to the side of pneumonectomy that presents for investigation 6 six years after surgery.

A patient with insidious onset of exertional dyspnoea.

The case history is presented of a 42 year old woman with pulmonary artery occlusion due to tuberculous vasculitis that masqueraded as chronic pulmonary artery embolism and led to severe life threatening haemoptysis necessitating emergency pneumonectomy. It is concluded that obliterative tuberculous endarteritis of the pulmonary arteries should be considered in the differential diagnosis of any acquired obstruction of pulmonary arteries.

Amiodarone and the development of ARDS after lung surgery.

An evaluation of amiodarone as prophylactic treatment for supraventricular tachyarrhythmias after pulmonary surgery was stopped because of a high incidence of the adult respiratory distress syndrome (ARDS) after a pneumonectomy. Retrospective analysis of all cases of resection for pulmonary neoplasm in our hospital between 1987 and 1991 indicates that amiodarone may be implicated in the development of ARDS after lung surgery.

High frequency ventilation combined with spontaneous breathing during bronchopleural fistula repair: a case report.

PURPOSE: We report the addition of high frequency oscillatory ventilation (HFOV), combined with spontaneous breathing under general anesthesia, during an uncommon technique to occlude a late post-pneumonectomy bronchopleural fistula. CLINICAL FEATURES: A 41-year-old woman underwent an extended right pneumonectomy with chest wall resection and prosthetic reconstruction for a large adenocarcinoma of the upper lobe (T3N0M0). Her postoperative recovery was satisfactory, and she subsequently received adjuvant chemotherapy. Four months later, however, she was readmitted for investigation of confusi…

Limited and radical resection for tracheal and bronchopulmonary carcinoid tumour. Report on 227 cases.

Bronchopulmonary carcinoid tumours occur at all levels from the trachea to the lung periphery. Over a 20-year period. 227 patients with carcinoid tumour underwent thoracotomy. The age at operation ranged from 14 to 79 years. Haemoptysis, chronic cough, recurrent infection and wheeze were the most common symptoms; 24% of patients were asymptomatic. The primary tumour was within the trachea or the main, lobar or segmental bronchi in 190 patients (83.7%). A variety of surgical procedures were employed: pneumonectomy in 32 patients; lobectomy and bilobectomy including bronchial sleeve resection in…

A late complication of a diagnostic mediastinoscopy.

A 69-year-old man with a moderately well differentiated squamous-cell carcinoma of the lung, underwent a mediastinoscopy. All histological examinations of the mediastinal nodes were negative. The patient was treated with a curative left pneumonectomy. Three months after this negative mediastinoscopy the patient developed a metastasis in the mediastinoscopy scar. A report of this case and a review of the literature are presented.
