One hundred and seven patients with polymyositis or dermatomyositis were followed for an average of 5 years. In 100 patients it was possible to evaluate the effect of treatment with steroids or a combination of steroids and immunosuppressive drugs. Fifty patients improved, 50 did not. Two groups of patients with polymyositis have been followed for approximately 3 years. One group was treated with prednisone alone and the other with prednisone plus azathioprine. Although the polymyositis of both groups has improved, no statistically significant difference was noted at the end of 3 months, as previously reported. Longer followup, however, has shown that the group given prednisone plus azathioprine has improved more with respect to functional disability; this group also requires less prednisone for disease control.

We attempted to identify all cases with polymyositis and/or dermatomyositis (PM-DM) admitted to hospitals in Israel during 1960-76. The diagnosis of PM-DM was retrospectively confirmed in 86 Jewish patients. The age-adjusted incidence rates increased from 1.30 in 1960-64 to 2.57 in 1970-74, with an overall incidence of 2.18 new cases diagnosed annually per 10(6) population. Strongyloides stercoralis is an intestinal nematode which infects large numbers of the population of tropical and subtropical geographic zones. Autoinfection permits the parasite to persist in asymptomatic hosts for years after they leave the tropics. Occasionally, in immunosuppressed patients, massive pulmonary or systemic invasion occurs. The conditions of three children with dermatomyositis and one child with polymyositis were treated for nine to 31 months with combined prednisone and intravenous methotrexate (1 mg/kg/wk) when prednisone alone was ineffective in controlling the disease or when there were substantial steroid-related toxic effects. A patient with toxoplasmosis had cerebellar, spinal cord, nerve root, and skeletal muscle symptoms. Toxoplasma gondii was isolated from skeletal muscle, and toxoplasma antibody titers were persistently elevated. The disease has been recurrent during a 3-year period with only temporary response to specific antimicrobial therapy.
