Pathophysiology of portal hypertension.

In last years significant progress in recognizing mechanisms of portal hypertension pathophysiology was done. However, some unclear topics in this disease still exist. Portal hypertension is primarily caused by the increase in resistance to portal outflow and secondly by an increase in splanchnic blood flow. Portal hypertension is associated with changes in the intrahepatic, systemic, and portosystemic collateral circulation. Alterations in vasoreactivity (vasodilation and vasoconstriction) play a central role in the pathophysiology of portal hypertension by contributing to increased intrahepa

Idiopathic myelofibrosis complicated by portal hypertension treated with a transjugular intrahepatic portosystemic shunt (TIPS).

Idiopathic myelofibrosis may be accompanied by portal hypertension. The authors report a 56-year-old man with idiopathic myelofibrosis and splenomegaly complicated by hepatopathy, severe portal hypertension and recurrent variceal bleeding. A transjugular intrahepatic porto-systemic shunt (TIPS) was inserted. Variceal bleeding never recurred. A short episode of encephalopathy, which is a known complication of porto-systemic shunting, ceased promptly after conservative treatment. The patient eventually died six months later due to metabolic deterioration and hepatic failure related to his underl

Portal Venous Gas on Point-of-Care Ultrasound in a Case of Cecal Ischemia.

BACKGROUND: Portal venous gas has mainly been studied in pediatrics and seen in cases of necrotizing enterocolitis. It is a rare finding in adults and is typically associated with underlying intestinal ischemia or other malignant intra-abdominal pathology. Portal venous gas is seen more readily on ultrasound compared to radiographs in both pediatric and adult patients. Findings include lucencies extending to the periphery of the liver, echogenic bubbles flowing centrifugally throughout the portal venous system, and bidirectional spikes interrupting the monophasic portal venous waveform on spec

Capillary connections between sensory circumventricular organs and adjacent parenchyma enable local volume transmission.

Among contributors to diffusible signaling are portal systems which join two capillary beds through connecting veins. Portal systems allow diffusible signals to be transported in high concentrations directly from one capillary bed to the other without dilution in the systemic circulation. Two portal systems have been identified in the brain. The first was discovered almost a century ago and connects the median eminence to the anterior pituitary gland. The second was discovered a few years ago and links the suprachiasmatic nucleus to the organum vasculosum of the lamina terminalis, a sensory ci…

Acute portal vein thrombosis secondary to donor/recipient portal vein diameter mismatch after orthotopic liver transplantation: a case report.

The incidence of portal vein thrombosis in end-stage liver disease is estimated as varying between 5% and 21%, whereas in candidates undergoing liver transplantation, this is 3-13%. Portal vein thrombosis occurring after liver transplantation can be managed surgically by thrombectomy, retransplantation, splenorenal shunt, or Wall-stent placement, or nonsurgically by angioplasty, local high-dose infusion of thrombolytic agents, combination of portal thrombolysis, or embolization of a pre-existing spontaneous splenorenal shunt. We report a case of portal vein thrombosis after liver transplantati

Acquired Portosystemic Shunts in Cirrhosis and Portal Vein Thrombosis: A Case Report.

Acquired portosystemic shunts (PSS) are abnormal blood vessels that develop between the portal vein and systemic circulation as a result of portal hypertension. Recurrent hyperammonemic encephalopathy in our 62-year-old patient with cirrhosis and chronic portal vein thrombosis led to the discovery of an extremely rare and functioning portosystemic shunt (PSS). The PSS connected the inferior mesenteric and left renal veins. Such shunts are considered pathological structures and may require surgical intervention. The large PSS reported herein likely provided decompression of the portal hypertens

Data Access, User Engagement, and Interoperability Standards in Public Health Data Portals: Environmental Scan Study.

BACKGROUND: Rapid digitalization has led to the generation of vast volumes of health data across diverse sources, including public health surveillance, electronic health records from hospitals, primary care facilities, and telemedicine. However, it often remains fragmented across multiple domains and systems due to siloed data collection practices and a lack of interoperability standards, limiting its potential to generate actionable public health insights. OBJECTIVE: In this study, we examined publicly available health data portals and assessed their scope, user engagement features, and impl

Changes in the blood flow to the digestive organs of sheep induced by feeding.

The blood flow to the digestive organs of nine sheep was determined by the use of isotopically labelled microspheres before, during and at 2 h and 4 h after feeding. Within 3 min of the start of feeding, the blood flow to the salivary glands and to the smooth muscle of the rumen and reticulum increased three-fold. The blood flow to the epithelium of the rumen and reticulum also increased before any appreciable effect on ruminal fermentation could have occurred. This increase in flow was greater in absolute but smaller in relative terms than that to the muscle. At 2 h after feeding blood flow t

Analysis of Coagulation Characteristics of Portal Venous Blood in Patients with Decompensated Cirrhotic Portal Hypertension.

OBJECTIVE: Coagulopathy is a common complication in patients with decompensated cirrhotic portal hypertension (DCPH), presenting a significant clinical challenge. Nonetheless, there is limited understanding of the coagulation profile of portal venous blood in DCPH patients. The objective of this study was to evaluate the coagulation characteristics of portal venous blood in DCPH patients by collecting blood samples through a transjugular intrahepatic portosystemic shunt (TIPS). METHODS: A total of 48 DCPH patients were enrolled to measure the activities of pro- and anticoagulant factors in bo

Varicose vein embolization and portal vein stenting in a patient with sinistral portal hypertension-induced gastrointestinal hemorrhage: a case report.

INTRODUCTION AND IMPORTANCE: Massive gastrointestinal hemorrhage is a severe hemorrhage that occurs in the gastrointestinal tract and is a life-threatening condition. Sinistral portal hypertension is a common etiology for massive gastrointestinal hemorrhage, whose occurrence might be derived from pathological changes induced by obstruction and/or increased blood flow to the portal vein system. However, there is a rare study reporting pancreatic disease-induced sinistral portal hypertension. CASE PRESENTATION: An 80-year-old female pancreatic cancer patient was admitted to our hospital on 22 J

Vascular, hemodynamic and renal effects of low-dose losartan in rats with secondary biliary cirrhosis.

BACKGROUND: In cirrhosis, splanchnic and systemic vasodilatation induce a hyperdynamic circulatory dysfunction, portal hypertension and renal sodium retention. This vasodilatation is in part because of an impaired vascular response to alpha1-adrenoceptor agonists. Recently, the angiotensin II type 1-receptor antagonist losartan has been shown to attenuate portal hypertension. We hypothesized that losartan decreases portal pressure by counteracting the impaired vascular responsive to alpha1-adrenoceptor agonists. METHODS: We studied, in rats with secondary biliary cirrhosis and sham-operated r

A rare Abernethy Ib malformation was initially misdiagnosed as chronic portal vein thrombosis in a 27-year-old female.

Abernethy malformation or congenital portosystemic shunt is a rare congenital vascular malformation and anomaly of the splanchnic venous system defined by diverting portal blood away from the liver. It is commonly associated with multiple congenital anomalies. Imaging modalities such as computed tomography or magnetic resonance have a crucial role in prompting diagnosis and determining the prognosis based on the type of malformation and associated anomalies. Misdiagnosis could be harmful and may lead to inappropriate treatment. We present a case of Abernethy malformation with a complete end-to

[Systemic mastocytosis disclosed by rupture of esophageal varices].

A 77-year-old woman had variceal bleeding related to systemic mastocytosis. Physical examination revealed minimal ascites and mild hepatomegaly noted 11 years before. Liver function tests were nearly normal. Because of early recurrent bleeding, a mesocaval shunt was performed. Wedged liver biopsy showed a moderate fibrosis of portal tracts and massive mast cell infiltration within portal tracts and sinusoids. Perisinusoidal collagen deposition was demonstrated ultrastructurally. We suggest that systemic mastocytosis be added to the list of diseases related portal hypertension with perisinusoid
