Hypogammaglobulinemia is a well-defined risk factor for infection. B-cell-directed immunotherapies given in addition to conventional chemotherapy are now core elements of effective therapy for children with B lymphoid malignancies. These therapies are associated with depletion of normal B cells and consequent hypogammaglobulinemia. This review summarizes the current state of knowledge regarding the mechanism, incidence, and clinical outcomes related to hypogammaglobulinemia in children with mature B-cell non-Hodgkin lymphoma and B-cell acute lymphoblastic leukemia, as well as provides practica

The main challenges in the treatment of T-cell lymphoblastic lymphoma (T-LBL) in children and adolescents are twofold: to increase survival rates in concert with reduction of acute and long-term toxicities including the rate of secondary malignancies. The need for molecular and prognostic markers in T-LBL is crucial to allow for systematic treatment optimization and may serve as targets for new treatment approaches.

Pediatric patients with hematologic malignancies require several procedural sedations by means of propofol infusion. We retrospectively analyzed the medical records of leukemic pediatric patients who had undergone procedural sedations at an Italian tertiary referral center (San Gerardo Hospital, Monza) from January 2011 to November 2013. We retrieved the following: demographics; diagnosis; chemotherapy phase; use of corticosteroids; induction dosage of propofol, fentanyl and/or ketamine; and the type of procedure. We used a multivariate linear mixed model to evaluate the factors affecting indu

While outcomes for children and adolescents with T-cell acute lymphoblastic leukemia (T-ALL) have improved significantly with contemporary therapy, outcomes for newly diagnosed adults and all patients with relapsed or refractory (r/r) disease remain poor. Improved understanding of T-ALL genomics and the integration of novel agents into treatment have the potential to improve outcomes further by enhancing risk stratification and improving salvage rates for those with r/r disease. In this review, we will discuss landmark genomic studies that have identified therapeutic targets in T-ALL, shed fur

L-Asparaginase (E.C.3.5.1.1; L-ASNase) hydrolyzes L-asparagine (an essential amino acid for the growth of leukemic cells) to aspartic acid and ammonia. It is obtained from various sources, including: bacteria, yeast, fungi, plants, and animals. It is used as a chemotherapeutic agent to treat acute lymphoblastic leukemia (ALL) and reduce acrylamide formation in baked and fried foods. Globally, various recombinant and pegylated L-ASNases formulations, including Escherichia coli and Erwinia-derived variants, are in Phase II or active clinical trials for ALL and related conditions across the USA,

Acute lymphoblastic leukemia (ALL) is the most common pediatric malignancy, with standard treatment consisting of intensive chemotherapy and corticosteroids. While curative in most cases, this regimen leads to significant toxicity and long-term sequelae. Recent advancements in cancer immunotherapy, including chimeric antigen receptor T cells and bispecific T cell engagers, have improved outcomes, yet are limited by toxicity and immune escape by target downregulation. Thus, novel less toxic treatment modalities are highly warranted. The tumor mutational burden in ALL is low, which results in a

Acute lymphoblastic leukemia (ALL) is a common pediatric cancer marked by uncontrolled proliferation of lymphoblasts, primarily affecting children aged two to five years. While treatment success rates have reached 90% in children, challenges such as therapy toxicity and relapse persist. The gut microbiome, known to impact immune function, is often disrupted in ALL patients, potentially worsening with treatments like chemotherapy and antibiotics. Research suggests probiotics may help restore microbial balance and mitigate side effects, with certain strains showing promise in reducing gastrointe

There is an unmet medical need for the treatment of elderly patients with acute lymphoblastic leukemia (ALL), as only a minority of them achieves long-term survival with current anti-ALL chemotherapy. Here, the recent management of these patients is discussed, including current approaches and future directions, as novel agents developed for refractory/relapsed ALL are now being incorporated into front-line therapies with very promising results. The manuscript will sequentially consider Philadelphia chromosome (Ph) negative ALL, then Ph+ and finally T-ALL. Accordingly, novel strategies using ch

BACKGROUND: Therapy-related acute lymphoblastic leukemia (t-ALL) is an aggressive subtype of ALL that arises after cytotoxic therapy. It is associated with adverse cytogenetics and older age, but survival outcomes compared with de novo ALL remain uncertain. We aimed to systematically evaluate survival differences in adults with t-ALL versus d-ALL. METHODS: We performed a systematic review and meta-analysis according to PRISMA guidelines. PubMed, EMBASE, Scopus, Web of Science, and Cochrane Library were searched to Dec 31, 2024. Eligible studies included adults (≥ 18 years) with t-ALL or de nov

The fifth edition of the WHO classification of haematolymphoid tumours (WHO-HEM5) introduces significant updates to the classification of acute lymphoblastic leukaemia, ALAL (including mixed phenotype acute leukaemia (MPAL)), myeloid/lymphoid neoplasms with eosinophilia and tyrosine kinase gene fusions (MLN-TK), and histiocytic and dendritic cell neoplasms, reflecting the advances in the understanding of the genetic basis of these diseases. This review provides an overview of these changes, highlighting a shift to a more refined molecular-genetic approach. The incorporation of newly recognised

Developing nanoscale platforms with high integration, assembly efficiency, and structural stability for performing complex computations in specific cells remains a significant challenge. To address this, the Three-dimensional Hierarchical Octahedral Robotic (THOR) DNA nanoplatform is introduced, which integrates targeting, logic computation, and sensing modules within a single framework. This nanoplatform specifically binds to cancer cell surface proteins, releasing aptamer-linked fuel chains to initiate subsequent computational processes. Three logic gates efficiently compute any arbitrary bi

Unlike other cases of acute leukemia, the diagnosis of T-cell acute lymphoblastic leukemia/lymphoma (T-ALL/LBL) is uniquely based on morphology and flow cytometry. Although the genomic background has been broadly uncovered, the large spectrum of genes involved and the variability of the molecular mechanisms underlying gene deregulation have delayed the introduction of molecular cytogenetics into diagnostic flowcharts. To overcome these limitations and implement a genetic diagnosis of T-ALL/LBLs, a whole transcriptome expression assay (WTEa) was repurposed as a "priority test" to classify T-ALL

Acute lymphoblastic leukemia (ALL) is the most common cancer in children, and prevention of this disease would significantly reduce childhood mortality and morbidity. While the role of germline genetic variation in the etiology of ALL is well established, the contribution of modifiable risk factors is less well known. Epidemiologic studies have identified several environmental and infectious or immune-related factors associated with childhood ALL, although studies often relied on questionnaire data, which can be prone to recall bias, and/or provided limited information on the potential leukemo

Acute lymphocytic leukaemia (ALL) is a haematological malignancy of the lymphoid progenitor cells. Enhanced genetic analyses have led to the identification of over 23 subtypes of B-cell and 17 subtypes of T-cell ALL. In parallel, the development of highly sensitive measurable residual disease assays have refined disease monitoring and risk stratification. Breakthroughs in molecular therapeutics and immunotherapies have improved treatment efficacy while reducing toxicity, challenging the traditional notion of 2·5-3 years of intensive chemotherapy. Notable progress includes the use of more poten

Acute Lymphoblastic Leukaemia (ALL) is a deadly form of blood cancer which occurs predominantly in children. It represents one of the most prevalent paediatric cancers. Both the diagnosis and monitoring of ALL presents clinicians with significant challenges. Diagnosis and monitoring of such paediatric conditions can be challenging due to the limitations in the patient's ability to communicate symptoms clearly. Early diagnosis is vital for improving the chances of positive outcomes for patients. Similarly rapid, real-time prognostics allows clinicians to adjust treatment in a timely manner. Thi

Superior vena cava syndrome (SVCS) is a clinical picture resulting from the obstruction of superior vena cava. SVCS usually causes symptoms and signs related to the head, the neck, the upper extremities and the upper thorax. We report a case of unusual clinical presentation of SVCS in a 25-year-old male patient who suffered from lymphoblastic lymphoma in childhood. Clinicians should be aware of abdominal wall presentations of SVCS especially in patients having undergone central venous catheterization, even years before. SVCS could be an emergency situation, requiring specific treatment or cont

Leukemia, a hematological disease affecting the bone marrow and white blood cells (WBCs), ranks among the top ten causes of mortality worldwide. Delays in decision-making often hinder the timely application of suitable medical treatments. Acute lymphoblastic leukemia (ALL) is one of the primary forms, constituting approximately 25% of childhood cancer cases. However, automated ALL diagnosis is challenging. Recently, machine learning (ML) has emerged as an important tool for building detection models. In this study, we present a hybrid detection model that improves the accuracy of the detection
