The use of the role construct repertory grid in studying changes during psychotherapy.

A case study including a detailed principal component analysis of repertory grids is reported in illustration of the use of grid method in the evaluation of psychotherapy. Grid method covers areas not accounted for by more symptom-bound criteria. Some of the problems of comparability and interpretation are discussed.

Beyond the treatment manual: Examining factors influencing Black client engagement in psychotherapy.

Despite the benefits of psychotherapy to improve mental health outcomes, therapy remains significantly underutilized among Black Americans. Yet, once therapy is initiated, Black clients experience mental health outcomes comparable to those in the general population, underscoring potential barriers to engagement. To address this gap, our qualitative study explored factors that contribute to client engagement throughout the therapeutic process, including core therapeutic skills related to therapist qualities, provider-client alliances, and nontherapeutic factors that shape the client experience.…

[Ketamine and esketamine in treatment-resistant depression - Pharmacological effects and augmented psychotherapy].

Esketamine is an NMDA receptor antagonist and was approved as a nasal spray in March 2019, initially in the USA and then in December 2019 in … lacebo group due to the characteristic side effects of esketamine. Proponents of ketamine-augmented psychotherapy (KAP) postulate that the rapid improvement in synaptic plasticity induced by ketamine, particularly in pathways to the prefrontal cortex, opens a 'therapeutic window' in which psychotherapeutic intervention is particularly effective. Due to the func …

Microprocesses predicting early emotional bond in videoconferencing and face-to-face psychotherapy.

Research on microprocesses underlying alliance formation remains limited compared to outcome-based studies, particularly in videoconferencing psychotherapy (VCP). This study examined therapist and client interaction patterns predicting early emotional bond in VCP and face-to-face therapy. A total of 56 early therapy sessions from 28 therapist-client dyads were coded with the Therapeutic Relationship Coding System, yielding 35,715 behaviors. Alliance strength was determined through an extreme-groups design based on the Bond subscale of the Working Alliance Inventory-Patient version. Bayesian lo…

Embodied movement markers of alliance ruptures in psychotherapy: A machine learning study.

OBJECTIVE: Alliance ruptures are central to psychotherapy process and outcome, yet therapists often fail to detect them in real time, particularly withdrawal ruptures, which tend to manifest subtly. This study examined whether machine learning applied to moment-to-moment movement dynamics during psychotherapy sessions can identify markers of alliance ruptures. METHOD: We analyzed 474 videotaped psychotherapy sessions from 86 dyads drawn from a randomized controlled trial for major depressive disorder. Alliance ruptures were coded using the Rupture Resolution Rating System (3RS). Patient and t…

Use of repair strategies and therapeutic alliance in child psychotherapy: therapists' reports.

OBJECTIVE: This study examined rupture-repair processes in child psychotherapy, focusing on therapist-reported repair strategies and their relation to child therapeutic alliance. The role of parental alliance and child's diagnostic profile in shaping rupture type and severity was also examined. METHODS: Ninety-four child psychotherapists attended workshops on rupture and repair strategies adapted for children and reported on a single therapy session with children aged 3-13. Therapists completed measures of child and parent alliance, rupture type and intensity, and reported interventions based…

Moments of Change in Group Psychotherapy: Exploring Therapeutic Factors and Relational Processes.

Transformative experiences are often described as pivotal moments of change in psychotherapy, yet relatively little is known about how such experiences are perceived within group settings. This study explored how group members and therapists describe moments of change in group psychotherapy and examined the therapeutic factors and relational processes associated with these experiences. Eight semi-structured interviews were conducted with four former group members and four licensed group therapists. Data were analyzed using reflexive thematic analysis, while descriptive counts were used to illu…

Dream processes and affect regulation in psychotherapy: a longitudinal clinical case study.

Anxiety disorders frequently emerge in contexts of chronic interpersonal stress and are often accompanied by disturbances in affect regulation and heightened sensitivity to bodily signals. In patients with chronic somatic conditions, these processes are commonly conceptualised within psychosomatic frameworks emphasising interactions between emotional regulation and stress-related bodily systems. Although psychotherapy may facilitate psychological adaptation, less is known about how dream processes reflect ongoing changes in affect regulation during treatment. The present study explores longitu…

Power animal journeying in Western psychotherapy: Applications, therapeutic safety, and future promises.

CONTEXT: A decolonial turn in psychology has inspired the practice of Western psychotherapy and clinical psychology. Recently, an increasing number of therapists have begun using the method of power animal journeying. This is a guided imagery-based technique originated from shamanic healing traditions. However, its use opens many questions regarding clinical applicability within the Western psychotherapeutic context. OBJECTIVE: The study aimed to explore the therapeutic use, mechanism, suitability, risks, and future promises of power animal journeying for Western psychotherapy. DESIGN: Fourt…

Intensive Short-Term Dynamic Psychotherapy for Complex, Chronic, and Treatment-Resistant Conditions.

Over the past 50 years, intensive short-term dynamic psychotherapy (ISTDP) has been developed, implemented, and evaluated with respect to the treatment of a broad spectrum of complex, chronic, and treatment-resistant conditions. This therapy was developed specifically to treat a range of patients, including those who are highly defensive, those who experience the repression of emotions, and those who have cognitive-perceptual disruptions along with primitive defenses. These three groups of patients are characterized by patterns of attachment trauma and deficits related to neglect. Among the ma…

Attitudes toward psychedelics and psychedelic-assisted psychotherapy among Australian mental healthcare providers.

BACKGROUND: Recent regulatory changes in Australia have approved 3,4-methylenedioxymethamphetamine for treating post-traumatic stress disorder and psilocybin for treatment-resistant depression. However, limited data exists on Australian mental healthcare providers' attitudes, knowledge and readiness to implement with psychedelic-assisted psychotherapy. METHODS: A cross-sectional survey was conducted between December 2023 and March 2024, targeting Australian general practitioners, psychiatrists and psychologists. Participants completed online questionnaires developed based on the Knowledge, At…

Prior Expectations of Volatility Following Psychotherapy for Delusions: A Randomized Clinical Trial.

IMPORTANCE: Persecutory delusions are common, distressing, and difficult to treat. Testing computational neuroscience models of delusions can identify new therapeutic targets. OBJECTIVE: To determine whether change in delusion severity is associated with a corresponding change in volatility priors and brain activation estimated during a belief updating task. DESIGN, SETTING, AND PARTICIPANTS: This randomized clinical trial was conducted from April 9, 2021, to December 5, 2023, within the Vanderbilt University Medical Center Psychiatric Hospital and at a community mental health center in Nash
