Global solidarity in genomic surveillance improves early detection of acute respiratory virus threats.

As genomic surveillance is key to detecting novel respiratory viruses or variants, the highly unequal global distribution of respiratory virus sequencing infrastructure raises concerns about preparedness for future threats. Using mathematical models and global epidemic simulations, we demonstrate that attaining a global minimum sequencing capacity of two sequences per million people per week at fortnightly sequencing regularity could reduce the time to first detection of novel respiratory (variant) viruses by weeks to months compared to global sequencing efforts during the COVID-19 pandemic, e

Climate Change Policies and Social Inequalities in the Transport, Infrastructure and Health Sectors: A Scoping Review Protocol.

Climate action has been deemed as fundamental to counteract the impacts of rising global temperatures on health which will disproportionately affect low-income populations, racial and ethnic minorities, women, and other historically marginalized groups. Along with poverty reduction, inequality mitigation, gender equality promotion, and public health protection, climate action has been recognized as a fundamental goal for achieving Sustainable Development Goals (SDGs). However, despite growing recognition of the need to align climate action with development goals, there is a knowledge gap regar

Challenges of schistosomiasis control in Africa: The path ahead.

Schistosomiasis is a neglected tropical disease that is endemic in sub-Saharan Africa, with over 90% of global cases occurring in the region. Poverty, inadequate healthcare infrastructure, and limited access to water and sanitation contribute to the high prevalence of the disease. Despite efforts to control and prevent the transmission of schistosomiasis, a reduction in the transmission rates has not been realized owing to several challenges. This article highlights the challenges of schistosomiasis control in Africa and recommends strategies for successful interventions. These strategies incl

Special considerations in the management of adult patients with acute leukaemias and myeloid neoplasms in the COVID-19 era: recommendations from a panel of international experts.

The ongoing COVID-19 pandemic caused by severe acute respiratory syndrome coronavirus 2 is a global public health crisis. Multiple observations indicate poorer post-infection outcomes for patients with cancer than for the general population. Herein, we highlight the challenges in caring for patients with acute leukaemias and myeloid neoplasms amid the COVID-19 pandemic. We summarise key changes related to service allocation, clinical and supportive care, clinical trial participation, and ethical considerations regarding the use of lifesaving measures for these patients. We recognise that these

Resistant dermatomyositis in a rural indigenous Maya woman.

A 28-year-old indigenous Guatemalan woman presented with 7 months of progressive weakness and numerous dermatological findings. She initially sought care within the free government-run health system and was treated with oral steroids for presumed dermatomyositis. Her symptoms progressed, including severe dysphagia, hypophonia and weakness preventing sitting. She was lost to follow-up in the public system due to financial and cultural barriers. A non-governmental organisation tailored to the needs of Maya patients provided home intravenous pulse dose methylprednisolone in the absence of first-l

Evaluation of user satisfaction and service responsiveness in municipalities enrolled in the Mais Médicos (More Doctors) Program.

The Mais Médicos (More Doctors) Program (PMM) addresses shortages of doctors, one of whose objectives is to reduce regional inequalities in access to health care. This descriptive cross-sectional study evaluated service responsiveness and user satisfaction with PMM doctors among 263 users of the public health system in 32 municipalities with 20% or more of extreme poverty. Questionnaires of open and closed questions were used. Users were satisfied with patient-doctor relationships, information about diseases and treatment and the clarity and comprehensibility of medical indications. The doctor

Sexually transmitted diseases and managed care: an inquiry and review of issues affecting service delivery.

To understand the potential role of managed care organizations (MCOs) in prevention and control of sexually transmitted diseases (STDs), we conducted a systematic review of articles on STDs and managed care and sought qualitative information from MCOs on STD-related activities. The review focused on prevention, risk assessment, patient education, counseling, screening, and costs of care, but revealed relatively few published articles. Barriers to STD service delivery included competing priorities, lack of time or supporting organizational structures, and differing mandates of health department

Rabies post-exposure prophylaxis in India: a SWOT analysis.

Rabies remains a pressing public health issue, particularly in countries like India, where it inflicts a significant toll on marginalised communities. With approximately 20,000 deaths annually due to rabies, the disease underscores the urgency for effective prevention strategies. Despite the existence of post-exposure prophylaxis (PEP), rabies continues to claim lives, highlighting the importance of understanding and addressing the challenges in its implementation. This study aims to conduct a comprehensive analysis of strengths, weaknesses, opportunities and threats (SWOT) of India's rabies P

Acceptability and perceived feasibility of strategies to increase public transport use for physical activity gain - A mixed methods study.

ISSUE ADDRESSED: Public transport (PT) users typically accumulate more physical activity (PA) than motor vehicle users. This mixed methods study aimed to determine acceptability and perceived effectiveness of strategies to increase bus use for PA gain in a regional Australian setting. METHODS: In a 2017 online survey, Tasmanian adults (n = 1091) rated the likelihood of increasing their bus use according to ten hypothetical strategies (fare-, incentives-, information- or infrastructure-based). Three focus groups and five interviews (n = 31) included infrequent bus users from the survey to dete

Navigating intersecting public health crises: a qualitative study of people with opioid use disorders' experiences during the COVID-19 pandemic.

BACKGROUND: The decades-long opioid epidemic and the more recent COVID-19 pandemic are two interacting events with significant public health impacts for people with opioid use disorder (OUD). Most published studies regarding the intersection of these two public health crises have focused on community, state, or national trends using pre-existing data. There is a need for complementary qualitative research aimed at identifying how people with opioid use disorder (OUD) are understanding, experiencing, and navigating this unprecedented time. The current study examines understandings and experienc

From screening to enrollment drop-offs: perspectives of minoritized participants in Indiana.

Vaccine uptake among Black and Latine populations remains persistently lower than among white populations, yet drop-offs between screening and enrollment in clinical trials is underexamined. The current sociopolitical moment, marked by "DEI" rollbacks, disruptions of federally funded health research, and sensitivity to health equity language, heightens the urgency of this work and threatens infrastructure supporting equitable recruitment. This qualitative study investigated non-enrollment among individuals positively screened for a vaccine trial at Indiana University School of Medicine and the

Mpox in conflict zones: Lessons from displaced populations in the Democratic Republic of Congo (DRC).

The Mpox outbreak in the Democratic Republic of Congo (DRC) highlights the critical intersection of conflict, displacement and public health. With over 15,000 reported cases and 700 deaths, the crisis has disproportionately affected internally displaced persons (IDPs), particularly women and girls, who face heightened vulnerabilities due to caregiving roles and unsanitary living conditions in overcrowded camps. These conditions, compounded by limited healthcare access, poor sanitation and widespread stigma, have amplified disease transmission. The inadequacies of vaccination campaigns, diagnos

Challenges associated with the implementation of institutional quarantine and isolation strategies during major multicountry viral outbreaks in Africa (2000-2023): a scoping review.

BACKGROUND: Quarantine and isolation (Q&I) are interrelated but not mutually exclusive public health practices for disease control, which may face public resistance in the context of health emergencies due to associated challenges. Hence, it is often tough for most countries to implement Q&I even in the context of health emergencies. Therefore, this scoping review examines the challenges associated with the implementation of institutional Q&I strategies during major multicountry viral outbreaks (Ebola, Lassa and COVID-19) in Africa between 2000 and 2023. METHODS: This scoping review was desig

Antimicrobial Resistance Under a Changing Climate.

The landscape of antimicrobial resistance (AMR) is changing across the animal, human and environmental ecosphere, with both climate change and AMR recognised as leading threats to human health. In this viewpoint, we argue that climate change is likely to reshape the conditions under which AMR emerges, persists and spreads, with important implications for how risks are understood and managed in Aotearoa New Zealand (ANZ). These risks arise because climate-related exposures interact with existing vulnerabilities and adaptive capacity across human, animal and environmental systems. Addressing AMR
