Distinction between benign and malignant causes of cervical, axillary, and inguinal lymphadenopathy: value of Doppler spectral waveform analysis.

OBJECTIVE: Vessels in inflamed lymph nodes are dilated compared with vessels in lymph nodes involved with metastases, which may be compressed by tumor cells. Accordingly, we hypothesized that the Doppler spectral waveform might be different in lymph nodes involved by benign disease and those involved by metastases. We compared spectral waveforms of benign and malignant superficial lymphadenopathy to determine the value of color Doppler sonography in distinguishing between the two. SUBJECTS AND METHODS: Palpable superficial lymph nodes (41 cervical, one axillary, one inguinal) of 43 untreated

Impact of calcific aortic valve disease on valve mechanics.

The aortic valve is a highly dynamic structure characterized by a transvalvular flow that is unsteady, pulsatile, and characterized by episodes of forward and reverse flow patterns. Calcific aortic valve disease (CAVD) resulting in compromised valve function and increased pressure overload on the ventricle potentially leading to heart failure if untreated, is the most predominant valve disease. CAVD is a multi-factorial disease involving molecular, tissue and mechanical interactions. In this review, we aim at recapitulating the biomechanical loads on the aortic valve, summarizing the current a

Gastrointestinal Bleeding in Patients Who Underwent HeartMate 3 Left Ventricular Assist Device Implantation.

BACKGROUND: Continuous-flow left ventricular assist devices (CF-LVADs) have yielded improved outcomes compared with pulsatile-flow devices; … 3-LVAD support but does not significantly impact survival. As the implantation of CF-LVADs with non-pulsatile flow gains popularity for both bridge-to-transplant and destination therapy, a better understanding of the pathophysiology of GIB in these patients will reduce the prevalence of this complication.

Dynamic Augmentation of Left Ventricle and Mitral Valve Function With an Implantable Soft Robotic Device.

Left ventricular failure is strongly associated with secondary mitral valve regurgitation. Implantable soft robotic devices are an emerging technology that enables augmentation of a native function of a target tissue. We demonstrate the ability of a novel soft robotic ventricular assist device to dynamically augment left ventricular contraction, provide native pulsatile flow, simultaneously reshape the mitral valve apparatus, and eliminate the associated regurgitation in an Short-term large animal model of acute left ventricular systolic dysfunction.

Radial artery pseudoaneurysm complicating a Brescia-Cimino dialysis fistula.

The authors describe an unusual case of radial artery pseudoaneurysm in a patient with a Brescia-Cimino fistula. Evaluation of a pulsatile mass on the patient's forearm which occurred after hemodialysis led to a nondiagnostic fistulogram. Color flow ultrasound was helpful in identifying the nature of the abnormality, which was subsequently confirmed arteriographically. This case illustrates that fistulography is not 100% sensitive and should be supplemented with other imaging modalities when the clinical and fistulographic findings are discordant.

Hemodynamic changes with paradoxical blood flow in expectant management of abruptio placentae.

BACKGROUND: Because the early diagnosis of abruption often is missed or misinterpreted, emergency situations frequently do not permit adequate management. CASE: A woman of 26-weeks' gestation was transferred with symptoms and laboratory findings typical for abruption. Ultrasound, including Doppler, revealed a subchorionic hematoma, pathologic blood flow in the uterine arteries, low pulsatility index values in the fetal cerebral and umbilical arteries (paradoxical blood flow), and high maximal velocities in the fetal aorta. Repeated Doppler and laboratory examinations revealed a gradual restor

Influence of arteries, vein and placenta interactions on the blood flow in the human umbilical cord.

The mechanisms governing umbilical venous return in early pregnancy remain incompletely understood, particularly the roles and interactions of arterial pulsatility, umbilical cord mechanics, and placental compliance. In this study, we combine numerical modelling and in vivo measurements to investigate hemodynamic and mechanical interactions within the feto-placental circulation. A fully coupled three-dimensional fluid-structure interaction model of the umbilical cord-comprising two pulsatile arteries, a compliant vein, and Wharton's jelly-was developed and connected to a zero-dimensional compl

Doppler study of cerebral arteries in hypercholesterolemia.

BACKGROUND: Hypercholesterolemia is one of the major modifiable risk factors for atherosclerosis of the coronary and carotid arteries. Although transcranial Doppler (TCD) studies of the cerebral arteries are indicative of decreased cerebral blood flow velocities in patients with hypercholesterolemia, the number of these studies has been limited. The aim of this study was to assess the hemodynamic status of the cerebral arteries in patients with hypercholesterolemia using TCD. METHODS: In a case-control study, 60 individuals, including 30 hypercholesterolemic cases (low-density lipoprotein [LD

Cerebral blood flow velocity in infants with polycythemia and hyperviscosity: effects of partial exchange transfusion with Plasmanate.

To determine if there are significant changes in cerebral blood flow velocity and vascular resistance in neonates with polycythemia and hyperviscosity, 11 such infants were studied before and after partial exchange transfusion with Plasmanate. Seven matched control infants were also studied. Blood flow velocity of the anterior cerebral artery was measured with a bidirectional continuous waveform Doppler velocimeter. Blood flow and vascular resistance were calculated from the velocity tracings, using the area under the velocity curve and the Pulsatility Index. Reduction in hematocrit and viscos

Arterial pulsations and transmantle pressure synergetically drive glymphatic flow.

Clearance of waste material from the brain by the glymphatic system results from net flow of cerebrospinal fluid (CSF) through perivascular spaces surrounding veins and arteries. In periarterial spaces, this bulk flow is directed from the cranial subarachnoid space towards the brain's interior. The precise pumping mechanism explaining this net inflow remains unclear. While in vivo experiments have shown that the pulsatile motion in periarterial spaces is synchronized with arterial pulsations, peristalsis alone has been deemed insufficient to explain bulk flow. In this study we examine an alter

Increasing airflow ventilation in a nasal maxillary ostium using optimised shape and pulsating flows.

Ventilation of the maxillary sinus is essential for regulating pressure, preventing infection and providing mucous to the nasal anatomy. During infection, the pathway between the sinus and the nasal airway (ostia) can become inflamed and restrict ventilation. Surgery is often required to restore airflow. The current surgical standard involves the widening of the ostium. Although this restores fluid flow, it has been linked to post-surgical sequelae. This study examined the effects of pulsating flow and geometric modifications on airflow distribution in a T-junction model analogous to a nasal m

Common carotid pulsatility is deteriorated by autoimmune thyroiditis in children with type 1 diabetes mellitus - A pilot study.

Autoimmune thyroiditis (AIT) frequently coexists with type 1 diabetes (DM1) and additionally increases the extent of microcirculatory complications due to DM1. We hypothesized that in pediatric patients with DM1, impairment of macrocirculation could be further augmented by a coexisting autoimmune process. Therefore, we investigated the influence of AIT on large arteries in DM1 pediatric patients. Our group consisted of 19 DM1, 19 DM1 + AIT patients and 29 control subjects. The groups were comparable regarding age and gender. The DM1 and DM1 + AIT patients were matched for age at onset of DM1 a

Twelve Weeks of Reversed Umbilical Flow in a Fetal Growth Restriction Case Associated with Severe Periconceptional Maternal Anemia.

We present a case of a patient diagnosed with severe intrauterine growth restriction and periconceptional severe anemia, where the fetal well-being features, including reversal of umbilical artery end-diastolic flow and abnormal cerebroplacental ratio, remained stationary for 12 weeks. We are not aware of reports with similar findings, and the professionals should take into consideration the possibility of a "frozen" fetal well-being for a long time. Possibly, in our case, the evolution may be due to the prompt correction of the chronic severe maternal anemia. This approach may offer the oppor

Predictors of the pulsatility index in the middle cerebral artery of acute stroke patients.

Cervical and transcranial Doppler (TCD) are widely used as non-invasive methods in the evaluation of acute ischemic stroke (AIS) patients. High-grade carotid artery stenosis induces haemodynamic changes such as collateral flow and a so-called post-stenotic flow pattern of the middle cerebral artery (MCA), which appears flattened, with a reduction of the velocity difference between systole and diastole. We studied the influence of carotid artery stenosis and other variables on the flow pattern in the MCA using the pulsatility index (PI), a quantitative TCD parameter reflecting the flow spectrum
