Paroxysmal atrial fibrillation (PAF) is one of the most frequent cardiac arrhythmias. Our purpose in the current study was the presentation of the main PAF aetiology and the most effective methods of therapy. The study revealed that both coronary artery disease and arterial hypertension are the most frequent reasons of PAF whereas acute physical effort seems to be the main triggering factor. The blockade of K+ channels and enhancement of neuromuscular transmission by dendrotoxin (DTX), a convulsant peptide from mamba snake venom, were examined in normal and mutant larval preparations of Drosophila. Forty-five consecutive patients with polymorphous ventricular tachycardia (PVT) were studied. The arrhythmia proved to be of a drug-related cause in 27 and due to an electrolyte disorder in four patients. Coexistent cardiac diseases without metabolic or drug-related abnormalities included ischemic heart disease in three, cardiomyopathy in three, and mitral valve prolapse in two. Ventricular ectopic activity in a young person with an otherwise normal heart usually requires no treatment. In older patients, 10 or more ectopic beats per minute, multiform beats and ectopy occurring close to the preceding beat are indications for drug control. Exercise-induced ectopic beats may respond to physical training. Three or more consecutive ventricular ectopic beats constitute ventricular tachycardia, which requires prevention. Supraventricular tachyarrhythmias, when paroxysmal, may be aborted by maneuvers to enhance vagal tone. When they are frequent, treatment is required.
