Radial to Axillary Nerve Transfers: A Combined Case Series.

PURPOSE: Loss of active shoulder abduction after brachial plexus or isolated axillary nerve injury is associated with a severe functional deficit. The purpose of this 2-center study was to retrospectively evaluate restoration of shoulder abduction after transfer of a radial nerve branch to the axillary nerve for patients after brachial plexus or axillary nerve injury. METHODS: Patients who underwent transfer of a radial nerve branch to the anterior branch of the axillary nerve between 2004 and 2014 were reviewed. A total of 27 patients with an average follow-up of 22 months were included. Out

Cross-neurotization of the caudal brachial plexus with the contralateral C-8 ventral nerve branch in the cat. Potential surgical applications, effects of graft collection on the healthy donor limb and results.

Contralateral transfer of the right, eighth ventral nerve branch (C8) (C8 cross-transfer - C8CT) was performed in 6 adult cats, in which the caudal part of the left brachial plexus (C8 and T1) had been severed, in order to mimic nerve root avulsion. Clinical and electrophysiological parameters, muscle contraction force measurements and histology were used to evaluate the effects of the surgery in a 14- to 36-month follow-up. The right forelimb (donor side) was clinically normal (no lameness) in all the cats at the end of the study. Electromyography performed 14 days after surgery revealed dene

Norms of Interocular Circumpapillary Retinal Nerve Fiber Layer Thickness Differences at 768 Retinal Locations.

PURPOSE: The onset and progression of optic neuropathies like glaucoma often occurs asymmetrically between the two eyes of a patient. Interocular circumpapillary retinal nerve fiber layer thickness (cpRNFLT) differences could detect disease earlier. To apply such differences diagnostically, detailed location specific norms are necessary. METHODS: Spectral-domain optical coherence tomography cpRNFLT circle scans from the population-based Leipzig Research Centre for Civilization Diseases-Adult study were selected. At each of the 768 radial scanning locations, normative interocular cpRNFLT diffe

Assessment of Median Nerve Mobility by Ultrasound Dynamic Imaging for Diagnosing Carpal Tunnel Syndrome.

Carpal tunnel syndrome (CTS) is the most common peripheral neuropathy and is characterized by median nerve entrapment at the wrist and the resulting median nerve dysfunction. CTS is diagnosed clinically as the gold standard and confirmed with nerve conduction studies (NCS). Complementing NCS, ultrasound imaging could provide additional anatomical information on pathological and motion changes of the median nerve. The purpose of this study was to estimate the transverse sliding patterns of the median nerve during finger movements by analyzing ultrasound dynamic images to distinguish between nor

Dual AAV amelioration of Lama2-null muscular dystrophy and neuropathy.

UNLABELLED: The dy 3K /dy 3K Lama2 -/- mouse is a model for the severe form of LAMA2-related dystrophy and peripheral neuropathy (LAMA2-RD). In the dystrophic mice, a compensating laminin subunit, Lmα4, that lacks polymerization and α-dystroglycan-binding activity, replaces the missing Lmα2 subunit. It was previously found that an α4-laminin can be modified with two small laminin-binding linker proteins, i.e. αLNNdΔG2' and miniagrin to facilitate polymerization and α-dystroglycan binding respectively, to enable the key missing functions. Adeno-associated virus serotype 9 (AAV 9 ) was used to d

Pectoralis Minor Tenotomy with Occasional Secondary Neurolysis Significantly Reduces Self-Reported Pain and Headaches Across Heterogenous Chronic Pain Disorders of the Upper Limb.

Background and Objectives: Many patients suffer from chronic pain of the shoulder, neck, upper back, and/or arm. They may be diagnosed with fibromyalgia, complex regional pain syndrome, myofascial pain, thoracic outlet, subacromial pain, cervical radiculopathy, cervicogenic headaches, post-mastectomy pain, and/or occupational shoulder disorder. The pectoralis minor (PM) is the only muscle of the scapula controlled by the lower trunk of the brachial plexus. In the Human Disharmony Loop (HDL), this neurologic asymmetry produces persistent protraction of the scapula. Protraction deforms the scapu

Symptom Improvement in a Patient With Chronic Pronator Teres Syndrome Treated With Ultrasound-Guided Hydrodissection and Manual Therapy: A Case Report.

Pronator teres syndrome (PTS) is an uncommon neuropathy caused by compression of the median nerve at the proximal forearm, resulting in paresthesia and pain in the radial digits. Although conservative management is typically the preferred initial approach, chronic and refractory cases often require multimodal treatment strategies. A 55-year-old female truck driver presented with a five-year history of persistent bilateral numbness and pain radiating from the palms to the distal phalanges. Physical examination revealed positive pronator compression and bilateral upper limb neurodynamic tests, w

Inflammatory vasculopathy in multifocal diabetic neuropathy.

Besides the common distal symmetrical sensory-motor polyneuropathy (DSP) that is often associated with autonomic dysfunction, diabetic patients may develop multifocal sensory-motor deficits (MDN) secondary to roots, plexus and nerve trunk involvement. Nerve ischaemia has been suggested as a common mechanism for the different patterns of diabetic neuropathies, yet the important clinical differences that exist between DSP and MDN suggest concurrent factors. In order to learn more on the subject, we prospectively studied 22 consecutive diabetic patients with MDN, for which other causes of neuropa

High-resolution ultrasonography in the diagnosis and intraoperative management of peripheral nerve lesions.

OBJECT: The diagnosis of peripheral nerve lesions relies on clinical history, physical examination, electrodiagnostic studies, and radiography. Magnetic resonance neurography offers high-resolution visualization of structural peripheral nerve lesions. The availability of MR neurography may be limited, and the costs can be significant. By comparison, ultrasonography is a portable, dynamic, and economic technology. The authors explored the clinical applicability of high-resolution ultrasonography in the preoperative and intraoperative management of peripheral nerve lesions. METHODS: The authors

What factors can reduce the need for repeated revision for humeral loosening in revision total elbow arthroplasty?

BACKGROUND: This study aimed to determine the re-revision rate in a cohort of patients who underwent revision total elbow arthroplasty (rTEA) for humeral loosening (HL) and identify factors contributing to re-revision. We hypothesized that proportional increases in the stem and flange lengths would stabilize the bone-implant interface significantly more than a disproportional increase in stem or flange length alone. Additionally, we hypothesized that the indication for the index arthroplasty would impact the need for repeated revision for HL. The secondary objective was to describe the functio

Treatment of Proximal Trans-ulnar Fracture-Dislocations Based on a Step-By-Step Coronoid-Centric Surgical Technique: Proposal of a Surgical Algorithm Based on Coronoid Management and Windows Approach.

INTRODUCTION: Complex proximal ulna fractures that are associated with radial head dislocation or fracture-dislocation, often referred to as "Monteggia-like" or trans-ulnar fracture-dislocations, constitute a heterogeneous group that poses challenges for classification and treatment. Management often relies on surgeon expertise rather than systematic approaches, leading to poor outcomes, high complication rates, and frequent surgical revisions. We hypothesize that a systematic, coronoid-centric, step-by-step surgical technique could improve outcomes in trans-ulnar fracture-dislocations. METHO

T1-nerve root neuroma presenting with apical mass and Horner's syndrome.

BACKGROUND: The appearance of dumbbell neuroma of the first thoracic root is extremely rare. The extradural component of a T1-dumbbell neuroma may present as an apical mass. The diagnosis of hand weakness is complex and may be delayed in T1-neuroma because of absence of the palpable cervical mass. One-stage removal of a T1-root neuroma and its intrathoracic extension demanded an extended posterior midline approach in the sitting position. CASE PRESENTATION: A 51-year old man had suffered a traumatic partial tendon rupture of his wrist flexor muscles 6 years ago. Since the incident he occasion

Intraneural perineurioma. A clonal neoplasm associated with abnormalities of chromosome 22.

The nature of perineurioma, variably termed "localized hypertrophic neuropathy," "intraneural neurofibroma," and "hypertrophic interstitial neuritis" has long been an issue of contention. Most authors consider it a neoplasm, but some a reactive process. Eight clinically and morphologically typical perineuriomas were studied by histologic, immunohistochemical and ultrastructural methods. One perineurioma was subject to tissue culture and cytogenetic study and another to fluorescence in situ hybridization (FISH) analysis. The patients, 3 males and 5 females, ranged in age from 11 to 38 years. Al

Case Report: Application of extracorporeal shockwave therapy in medial epicondylitis with concomitant ulnar nerve instability: a case series with long-term follow-up.

BACKGROUND: Medial epicondylitis is an overuse syndrome characterized by degeneration of the flexor-pronator tendons in the elbow, resulting from repetitive forced wrist flexion and forearm pronation. Due to its anatomical location, medial epicondylitis patients may also feature ulnar nerve instability, which can exacerbate symptoms and negatively impact treatment outcomes. Although conservative treatments remain the cornerstone of care for managing medial epicondylitis, the optimal treatment method remains an open question. OBJECTIVE: To evaluate the effects of a combined extracorporeal shoc
