From February 1981 to January 1984, 20 patients with a tumor of the upper clivus received proton irradiation at the Harvard Cyclotron Laboratory. For 15 patients with known neurovisual status (including visual acuity, color vision, visual field, and fundus examinations) we obtained a cumulative dose-volume histogram (DVH) of the optic nerves (ON) and the optic chiasm. The prescribed tumor doses ranged from 66.6 to 74.4 Cobalt Gray Equivalent (CGE) with a daily fraction size of 1.8 to 2.1 CGE. Radiation therapy and chemotherapy have decreased the mortality rates of cancer patients, but the morbidity associated with oral complications is high in many cases. A pretreatment oral evaluation and institution of a preventive care program reduce oral symptoms such as glossodynia considerably. When oral symptoms are minimized, the dentist can improve the patient's quality of life.

Total surgical excision is the main goal of therapy for intracranial meningiomas. The controversy today involves the efficacy of postoperative radiation therapy. To evaluate this question, 132 patients with benign intracranial meningiomas, treated between October 1964 and April 1985, were evaluated. All patients had a minimum 2-year follow-up. Twelve patients with recurrent supratentorial gliomas were treated with implanted 125I seeds. The interval from initial surgery to interstitial radiation ranged from 3 months to 9 years. The techniques for brachytherapy included volume implantation by craniotomy in one, stereotactic implantation with low-activity seeds in 7 and high-activity seeds in 4 patients. A study was made of the effectiveness of a new class of radioprotective agents, polypeptides, obtained from zootoxins of scorpion, tarantula, Lathrodectes tredecimguttatus, and bee under conditions of a short-term and long-term irradiation. The peptide fraction of the scorpion venom, butoxin, was most radioprotective: it provided 65% survival after LD98/30. Butoxin exerted a stimulatory effect on the hypophysis-adrenal system and haemopoiesis of intact and irradiated animals.

Twelve patients with incompletely resected meningiomas were treated with postoperative radiation therapy. Nine of these patients had previously undergone incomplete surgical resection, and three had suffered one or more postoperative recurrences. A retrospective study of 93 patients who underwent pterygium excision revealed a low prevalence of myopia in comparison with normal population. These results support the theory that solar ultraviolet radiation is a contributory factor in the formation in pterygium and some protection against this radiation is afforded by continuously worn myopic correction.

In 1975 Samaan et al described the effects of radiation damage of the hypothalamus in 15 patients with head and neck cancer. Shalet et al in 1977 described endocrine morbidity in adults who as children had been irradiated for brain tumors. This report describes instances of hyperprolactinemia and associated hypothalamic, pituitary, and thyroid dysfunction following irradiation of a young adult female for brain neoplasia.

The role of radiation therapy in the management of adrenal cortical carcinoma has had little documentation in the literature. Fourteen patients with this malignancy were given 18 courses of palliative, preoperative or postoperative irradiation. The clinical results are presented. Significant palliation was obtained in all patients along with occasional long term local control of unresectable lesions.
