Adrenergic modulation of a spinal sympathetic reflex in the rat.

Patients with spinal cord injury involving transection of the lower cervical or upper thoracic spinal cord can experience an autonomic hyperreflexia characterized by exaggerated blood pressure increases in response to visceral or somatic stimuli such as skin stimulation and urinary bladder or rectal distention. These cardiovascular responses are mediated by activation of spinal sympathetic reflex (SSR) circuits in the segments below the transection that are no longer controlled by their supraspinal inputs. We have examined the SSR in decerebrate, unanesthetized, paralyzed, artificially ventila…

Pudendal reflexes in women with pelvic floor disorders.

The objective of this work was to study two pudendal anal reflexes: Deep Pudendal Reflex (DPR) and classical Bulbocavernosus Reflex (BCR) in women with primary and recurrent genital prolapse to obtain support to the hypothesis of pelvic nerve damage in patients with pelvic floor disorders. 124 women were studied: 68 were normal; 38 with genital prolapse (GP); and 18 with recurrent GP. Clinical and electrophysiological studies were carried out. Delayed reflex responses were found in 44/56 of patients (79%). [27/38 in genital prolapse group (71%); 17/18 in recurrent GP group (94%)]. The evaluati…

[Syndrome of delayed maturation of the sucking-swallowing reflex].

BACKGROUND: Neonates with neurological diseases often have difficulty in sucking or swallowing. This report describes such difficulties in a group of infants with normal development, suggesting late maturation of sucking and swallowing. POPULATION AND METHODS: Seven infants (four girls, three boys) had suffered from aspiration since their first day of life (four cases) with severe asphyxia (two cases), late repeated episodes of airway obstruction by accumulated secretions (two cases), and weak isolated sucking and swallowing (one case). The lack of sucking and swallowing or difficulty with th

Reflex mechanisms of death in missile injuries of the neck.

Three cases of missile injury to the neck are reported, two homicides and one suicide. In two fatalities, the autopsy revealed contusion of the spinal cord due to direct missile injury of the cervical spine but without laceration of the spinal dura. It was concluded that the temporary cavity following penetration of the bullet caused contusion of the spinal cord with subsequent reflex cardiac arrest. In the suicide case, the entrance wound was in the mouth, the bullet track traversed the pharynx and the cervical spine with complete transection of the cord, and the bullet lodged in the right sc…

Effect of age on the sensitivity of upper airway reflexes.

We have recorded the threshold concentration of inhaled ammonia vapour required to elicit reflex glottic closure (NH3TR) in 102 healthy, nonsmoking volunteers (39 female) aged 17-96 yr in order to assess the effect of age upon upper airway reflex sensitivity. A single measurement of sensitivity was made in each subject using a system delivering small concentrations of ammonia vapour for single intermittent breaths to the upper airway and recording glottic closure using an inspiratory pneumotachograph. We found a strong positive correlation between age and NH3TR, indicating a decrease in upper …

The blink reflex in neonates with a subsequent poor outcome.

We examined maturational changes in the electrical blink reflex in 11 handicapped children, i.e., 4 cases of developmental delay, 4 of cerebral palsy, 2 of congenital hydrocephalus, and 1 of congenital cytomegalovirus infection. The developmental delay and cerebral palsy cases were all born at 25-36 weeks' gestation. In all cases, prolonged latency, low amplitude, or the absence of the late component of the blink reflex was observed in the follow-up study. These results indicate that prematurity and some congenital impairment mainly influence the maturation of the long-loop reflex arc in the b…

The pterygoid reflex in man and its clinical application.

A technique for eliciting and recording the stretch reflex (R) of the medial pterygoid muscle (Pter) is described. The latency was 6.9 +/- 0.43 ms in 23 healthy volunteers (mean age 23.7 years) showing a side-to-side difference of 0.29 +/- 0.21 ms. The PterR latencies were little shorter and side-to-side differences little greater than of the masseter reflex. Observations in 5 selected patients with small brainstem lesions suggest that the neurons of the PterR afferents form a cluster within the caudal portion of the trigeminal mesencephalic nucleus. Testing the masseter and pterygoid reflexes…

Stapedius reflex in patients with an inner ear prosthesis.

The stapedius reflex elicited by electrical stimulation was investigated in 21 deaf individuals supplied with an inner ear prosthesis. All patients were implanted with the Vienna cochlear implant using intra or extracochlear electrodes. Analog stimulation with sinus bursts of various durations (25, 50, 100, 300, 500 ms) and frequencies (125, 500, 1000, 2000 Hz) was used. The time course of the contralateral acoustic reflex was monitored using a fast-response impedance meter. The stapedius reflex was observed in about half of the cases. Reflex threshold was observed at a stimulation level close…

Cardiovascular reflexes evoked by histamine stimulation of the stomach.

This study examined the potential for histamine to cause cardiovascular reflexes when applied to the serosal or mucosal surface of the stomach. Thus, in chloralose-anesthetized cats, histamine was applied to the serosal surface of the stomach in concentrations ranging from 0.5 to 1,000 micrograms/ml. This resulted in graded increases in mean arterial pressure (MAP), maximal left ventricular pressure over time (dP/dt), and heart rate ranging from 9 +/- 4 to 30 +/- 3 mmHg, 450 +/- 103 to 1,710 +/- 610 mmHg/s, and 2 +/- 1 to 13 +/- 4 beats/min, respectively. Histamine stimulation of the gastric s…

Physiology and clinical applications of hand muscle reflexes.

Hand muscle reflexes to stretch or electric stimulation of mixed nerves consist of two main components, the short latency reflex (SLR, M1) or Hoffmann reflex (HR) and the long latency reflex (LLR, M2). The SLR is most likely a spinal, monosynaptic reflex and all the evidence presently available supports a transcortical pathway of the LLR. Investigations in normal subjects demonstrate that the LLR is a reflex mediated by fast conducting muscle and cutaneous afferents. Group II muscle afferents do not significantly contribute to this reflex and it cannot be explained by repetitive excitation of …

Effects of NH4+ on reflexes in cat spinal cord.

1. In deeply barbiturate-anesthetized animals. NH4+ decreases spinal excitatory synaptic transmission by neuronal depolarization and subsequent block of conduction of action potentials into presynaptic terminals of low-threshold (presumably Ia-) afferents. Because barbiturates by themselves depress excitatory synaptic transmission and may have modified the effects of NH4+, this study examines the effect of NH4+ on excitatory synaptic transmission in the unanesthetized animal. 2. The effects of NH4+ on monosynaptic and polysynaptic excitatory reflexes as well as di- and polysynaptic inhibition …

Cardiovascular reflexes from cardiac sensory receptors.

The mammalian heart, especially its left ventricle, is densely innervated by sensory nerves. One set of these travels to the brainstem in the vagus nerves; the other to the spinal cord in sympathetic nerves. Excitation of vagal cardiac afferents, especially unmyelinated afferents from the left ventricle, cause a reflex bradycardia and fall in blood pressure and, under some conditions, a massive release of AVP. The sympathetic afferents convey the sensation of cardiac pain, but innocuous stimuli may cause a reflex tachycardia and rise in blood pressure. Both sympathetic and vagal cardiac affere…

The blink reflex in patients with idiopathic torsion dystonia.

The blink reflex and its recovery cycle were examined in 57 patients with idiopathic dystonia affecting different parts of the body. The group comprised 9 patients with generalized and 15 with segmental forms, 19 with torticollis, and 14 with focal arm dystonia. None had blepharospasm. The duration and amplitude of the R2 component of the blink reflex showed only minor changes. However, its recovery cycle to paired supraorbital nerve stimuli was abnormal in all groups of patients, except those with focal arm dystonia. These findings may be interpreted as showing abnormal control of the interne…

Attenuation of the baroreceptor reflex by propofol anesthesia in the rat.

The effect of the intravenous (i.v.) short-acting anesthetic propofol on baroreflex control of heart rate (HR) was examined in normotensive rats. Propofol was infused at a fixed rate of 9 mg/kg.h immediately after a bolus dose of 10 mg/kg. Blood pressure (BP) was maintained with a plasma substitute. Conscious rats were used for comparisons. Reflex HR response were recorded after i.v. doses of the pressor agent phenylephrine and the depressor agent sodium nitroprusside. Baroreceptor reflex parameters were determined by sigmoidal computerized curve-fitting. Propofol produced tachycardia at rest …

It also produced systemic hypertension and enhanced crisis of autonomic dysreflexia. The overall effect of clonidine on urodynamics in acute decerebrate cats was to bring about an active, small (normotonic) bladder with urinary frequencies. Nonlinearity and asymmetry in the human vestibulo-ocular reflex.

The horizontal vestibulo-ocular reflex was studied in normal and unilaterally vestibulopathic human subjects; the latter before and after surgical vestibular ablation. Subjects were oscillated at 0.025-4 Hz, 50 o/s peak head velocity, and for 0.025 and 0.25 Hz, at 50-300 o/s peak head velocity. Gain (peak eye/head velocities), phase (delay in degrees between peak eye and head velocities), and asymmetry (including d.c. bias; the average eye velocity over the cycle) were assessed for each rotation. In patients preoperatively, gain was subnormal while phase lead was greater than normal only at lo…

Studies of the horizontal vestibulo-ocular reflex in spaceflight.

Changes in the vestibulo-ocular reflex (VOR) during space flight have been suspected of contributing to space motion sickness. The horizontal VOR was studied in nine subjects on two space shuttle missions. Active unpaced head oscillation at 0.3 Hz was used as the stimulus to examine the gain and phase of the VOR with and without visual input, as well as the visual suppression of the reflex. No statistically significant changes were noted inflight in the gains or phase shifts of the VOR during any test condition, or between space motion sickness susceptible and nonsusceptible populations. Altho…
