Restraint asphyxiation in excited delirium.

Eleven cases of sudden death of men restrained in a prone position by police officers are reported. Nine of the men were hogtied, one was tied to a hospital gurney, and one was manually held prone. All subjects were in an excited delirious state when restrained. Three were psychotic, whereas the others were acutely delirious from drugs (six from cocaine, one from methamphetamine, and one from LSD). Two were shocked with stun guns shortly before death. The literature is reviewed and mechanisms of death are discussed.

Bright light treatment of behavioral and sleep disturbances in patients with Alzheimer's disease.

OBJECTIVE: The authors tested the hypothesis that evening bright light pulses would improve sleep-wake patterns and reduce agitation in patients with Alzheimer's disease who have severe sundowning (a syndrome of recurring confusion and increased agitation in the late afternoon or early evening) and sleep disorders. METHOD: Ten inpatients with Alzheimer's disease on a research ward of a veterans' hospital were studied in an open clinical trial. All patients had sundowning behavior and sleep disturbances. After a week of baseline measurements, patients received 2 hours/day of exposure to bright

The brain renin-angiotensin system and prolactin secretion in the male rat.

The present studies investigated the role of the brain renin-angiotensin system in the regulation of PRL secretion in the male rat. Blood samples were taken from conscious rats before, during, and after administration of test substances into the third cerebral ventricle. In the first series of experiments, we determined the sensitivity of the PRL response to intracerebroventricular (icv) administration of angiotensin II (Ang II) and found that PRL levels were significantly suppressed in a dose-related manner (10-500 ng). A dose of 1 ng did not significantly affect PRL values, compared to those

The dangerous patient.

The dangerous patient requires proper management to ensure appropriate disposition and preclude injury. A safety-conscious public has propelled the issue into the courtroom, and a slowly evolving standard of care is emerging. In some states a legal duty to protect victims of violence exists. Understanding human aggression, the potentiating effects of the environment, and prior methods of coping, as well as assessing current behavioral controls and certain statistical correlates all aid management decisions. Stabilization of the dangerous patient begins with the interview and progresses to medi

Differential effects of naloxone on basal and stress-induced release of ACTH and prolactin in the male rat.

The effect of i.v. injection of various doses of naloxone (NAL) on plasma adrenocorticotropin (ACTH) and prolactin (Prl) in conscious animals bearing an indwelling intrajugular catheter was assessed. The effects were evaluated in animals which were left undisturbed and in others subjected to either restraint or ether stress. The results revealed that the dose of 3 mg/kg of NAL significantly reduced basal Prl levels, whereas a dose of 6 mg/kg of NAL was required to block completely either ether or restraint stress-induced release of Prl. The behavior of ACTH contrasted with that of Prl. There w

Protection by the calcium antagonist Wy-47,037 against stress ulceration in the rat.

Wy-47,037, a novel compound with both intracellular and extracellular calcium-blocking properties, was evaluated for its effects on cold/restraint stress induced ulceration in the rat. Wy-47,037 dose-dependently inhibited ulcerogenesis; with an ED50 of 8 mg/kg, PO, it was approximately twice as potent as nitrendipine (ED50 = 15 mg/kg). Wy-47,037 also reduced basal gastric acid secretion (ED50 = 7 mg/kg) and gastrointestinal motility (ED50 = 16 mg/kg). It is thus possible that Wy-47,037's alteration of basal gastric acid secretion and/or of gastrointestinal motility may contribute to its therap

Thermoregulatory effects of D-met2-pro5-enkephalinamide.

Changes elicited in the body temperature by subcutaneous injection of the synthetic D-met2-pro5-enkephalinamide (D-met2-pro5-EA) were studied in CFY rats. D-met2-pro5-EA in doses of 8, 20 and 40 mg/kg caused hyperthermia. If the animals were restrained, this hyperthermic effect was attenuated. Ten mg/kg naloxone completely prevented the elevation of body temperature. Previous administration of 300 mg/kg capsaicin in fractionated daily doses resulted in a facilitation of the hyperthermic reaction. In contrast to other opioids, hypothermia was never observed. Our observations support the concept

Age differences and stress ulcer in the rat.

Rats 2-, 7- and 12-mo. old were exposed to supine body restraint plus cold (5 degrees C) for 3 hr. The mean cumulative length of lesions for the 2-, 7-, and 12 mo. old rats was 22.0, 43.0 and 16.0 mm. respectively. The same experimental design was used in a second study, but the pylorus was ligated prior to restraint. Total acid output/hr. was 134.6, 178.2, and 64.7 muEq/60 min. respectively for the three age groups. Older rats were not more susceptible to stress-ulcer and gastric acid was not significantly related to degree of ulceration.

Physical rehabilitation of cats: 1. Common conditions benefiting from physiotherapy.

PRACTICAL RELEVANCE: Physical rehabilitation (physiotherapy) involves restoring a patient's movement and function following an injury, illness or trauma. It can be used for acute and chronic conditions and, in some cases, the techniques are used as a preventive treatment. Some therapies can also provide pain relief, which can be especially helpful when analgesic drugs are contraindicated in a patient. In recent years, there has been increasing interest in the application of physiotherapy to small animals, and while physical rehabilitation has traditionally been used more commonly for canine th

Treatment of anorexia nervosa in Ireland: striking an ethical balance.

Anorexia nervosa (AN) is a complex psychiatric illness with severe and life-threatening medical sequelae, including death. Existing evidence-based treatments are linked to good prognosis and full recovery in many. For a small minority of critically ill patients, treatment decisions extend beyond voluntary engagement. Severe cases may involve involuntary hospitalisation, nasogastric feeding, physical restraint, and other coercive measures. While these interventions are sometimes necessary to prevent death, they raise profound ethical concerns. This article explores the ethical tensions in treat

Practice-based evidence from a clinical cohort that received pediatric constraint- induced movement therapy.

PURPOSE: Constraint-Induced Movement Therapy (CIMT) is now designated a highly efficacious treatment for children with cerebral palsy, based on rigorous clinical trials. Yet virtually no evidence confirms that these moderate to large size effects can be replicated in clinical practice for a more heterogeneous clinical population. Thus there is a need to collect and report treatment outcome data based on actual clinical practice as a critical next step for implementation. METHODS: This study presents results from a prospective study conducted on a clinical cohort of 88 children, 18 months to 1

Effect of small group treatment of the modified constraint induced movement therapy for clients with chronic stroke in a community setting.

This study investigated the effects of the group treatment component of the modified constraint induced movement therapy (mCIMT) protocol for clients with chronic stroke in a community setting. A within-subjects longitudinal study was conducted to which eight participants with chronic stroke being treated in a community setting in Hong Kong were recruited. Ten 3-h group sessions were conducted on two occasions within a four-week period, with four participants per group. Participants' less-affected hands were restrained in a mitt, with a target of wearing it for 4h per weekday. Laboratory based

Long durations of immobilization in the rat result in enhanced mechanical properties of the healing supraspinatus tendon insertion site.

Rotator cuff tears frequently occur and can lead to pain and decreased shoulder function. Repair of the torn tendon back to bone is often successful in relieving pain, but failure of the repair commonly occurs. Post-operative activity level is an important treatment component that has received minimal attention for the shoulder, but may have the potential to enhance tendon to bone healing. The objective of this study was to investigate the effect of short and long durations of various activity levels on the healing supraspinatus tendon to bone insertion site. Rotator cuff tears were surgically

Perceived intrusiveness of coercive measures: a Danish survey of mental health service users.

INTRODUCTION: Coercive measures (CM) are commonly used in mental health care. From a mental health service user perspective, CM are often associated with physical and psychological harm and a serious infringement on personal autonomy. Danish health legislation, therefore, adheres to principles requiring the use of the least intrusive CM. However, limited knowledge exists about service users' perceptions of what constitutes the least intrusive CM. PURPOSE: To survey mental health service users' perceptions of intrusiveness related to various CM and to identify which interventions they consider

Factors associated with boarding and length of stay for pediatric mental health emergency visits.

OBJECTIVE: To determine demographic and clinical risk factors associated with boarding (length of stay ≥24 h) for pediatric mental health emergency department (ED) visits. METHODS: This is a retrospective cross-sectional analysis of mental health visits identified by diagnosis codes for children 5-18 years old presenting to a tertiary pediatric ED in 2016. We performed multivariate logistic regression to identify demographic and clinical factors associated with boarding. RESULTS: There were 1746 mental health visits and 386 (22%) visits had length of stay ≥24 h. In the multivariate logistic

Hypertensive Encephalopathy: A Case of a Male Who Bit Off His Fingers.

Although altered consciousness and other neurologic manifestations are frequently seen in hypertensive encephalopathy, behavioral and psychotic symptoms are rarely seen. We describe a patient with no previous psychiatric history who was admitted for hypertensive crisis. A few days after admission, his blood pressure remained uncontrolled and he started to exhibit episodes of confusion, agitation, and psychosis. During one particular episode, he overcame multiple staff members and physical restraints to bite off two of his fingers without any signs of pain. Brain computed tomography (CT) was no

[The procedures for sampling specimens in pediatrics].

Methods to obtain specimens from children are invasive, and the procedures require skill. Because children are smaller and weaker than adults, the procedures often become difficult. The equipment for the procedures must be chosen based on the physical constitution of the patient. In addition, pediatric patients often do not cooperate with procedures, and so restraint of the patient is extremely important. It is particularly important when obtaining specimens from small neonates and infants, after establishing the minimum quantity of specimens, to choose an appropriate procedure. Because the st

Exploring staff experiences: A case for redesigning the response to aggression and violence in the emergency department.

INTRODUCTION: A Code Black is the coordinated response by healthcare staff and security to an act or threat of verbal and physical aggression or violence perpetrated by a patient, family member or visitor towards healthcare staff within a hospital setting. Behavioural disturbance is often verbally de-escalated by staff. However, as a last resort physical and/or chemical restraint may be necessary. Reports show that there is an increasing number of violent incidents in Emergency Departments (ED), emphasising the importance of staff training for Code Black events. This research examines the resp
