INTRODUCTION: Retroperitoneal fibrosis is a rare disorder consisted of a periaortic fibrous mass that compresses retroperitoneal organs.
CASE REPORT: We report a case of retroperitoneal fibrosis diagnosed after a spinal cord compression. Magnetic resonance imaging scan showed a prevertebral mass which affected a vertebral body. It extended to the epidural space and compressed the spinal cord. The final diagnosis was established through the mass biopsy by means of mediastinoscopy. After prednisone treatment, both clinical remission and mass reduction in the follow-up MRI were observed.

PURPOSE: To determine if the gadolinium-enhancement characteristics of magnetic resonance images (MRI) differ for acute and chronic benign retroperitoneal fibrosis (RPF).
METHOD: Seven male subjects, 3 with newly diagnosed nontreated RPF (acute group) and 4 with long-standing stable RPF (chronic group) who had been treated with steroids, ureteric stents or both, underwent MRI examinations with gadolinium enhancement. Patients in the acute group were examined again after treatment. Mean dynamic gadolinium-enhancement ratios (both dynamic and delayed) were calculated for each group.
RESULTS: T…

Primary biliary cirrhosis has rarely been reported to be associated with idiopathic retroperitoneal fibrosis. We describe the case of a 57-year-old man who was confirmed to have both conditions, reflecting the likely autoimmune aetiopathogenesis common to both disorders.

Corticosteroids were successfully used to treat a 66-year-old man with retroperitoneal fibrosis (RPF) and previously diagnosed membranous nephropathy. Proteinuria was noted at the age of 51 years, and membranous nephropathy was diagnosed by renal biopsy. Ten years later, he presented with right hydronephrosis and renal dysfunction, and was diagnosed as having RPF based on the typical diagnostic imaging findings. Steroid therapy was successful, resulting in improvement of the hydronephrosis and renal function. The hydronephrosis recurred three years later, but corticosteroids were again effecti…

Idiopathic retroperitoneal fibrosis (ERF) is a rare disease characterized by non-neoplastic fibroblastic and inflammatory proliferation. We describe a case of a 58-year-old man presenting with abdominal pain, weight loss and abdominal mass on clinical examination. The patient was evaluated by abdominal CT scan and magnetic resonance imaging which disclosed a periaortic soft-tissue mass with marked longitudinal extension below renal arteries origin. The diagnosis of ERF was histologically confirmed after surgical exploration and biopsies followed by extensive ablation of fibrous tissue. After 6…

The inflammatory aneurysm of the abdominal aorta is actually a well known and distinct entity. However its etiology is unknown and its clinical expression often remains a trap. The authors, through a six case experience insist on the surprising characters of the symptoms, and emphasize the contribution of the paraclinical triad (RSV, IUV, and CT Scan). They confirm the benefit of the surgical treatment on the aneurysm on the fibrosis decrease.

A patient with Riedel's struma (invasive fibrous thyroiditis) with hypothyroidism, hypoparathyroidism, and retroperitoneal fibrosis is reported. This disorder appeared to emanate from Hashimoto's thyroiditis in this case. The possible relationship of the two entities is discussed.

Retroperitoneal fibrosis is characterized by symptoms associated with gradual compression of retroperitoneal structures. This case report demonstrates the use of Ga-67 imaging and CT in the early diagnostic work-up of this disorder.

15 patients suffering from retroperitoneal fibrosis with ureteral obstruction have been treated. 6 patients were operated at the first onset, 3 after an attempt of corticotherapy and 6 patients had exclusive corticotherapy. Results of surgery referred to kidney units are 9 good results out of 12 and these of corticotherapy 11 good results out of 16. Besides, in the literature, the complications of surgery are frequent and serious. Medical treatment, with renal drainage, when needed is so to consider first, surgical treatment being kept for corticotherapy failure.

We encountered a case of primary biliary cirrhosis in a nonalcoholic man who had been operated on for idiopathic retroperitoneal fibrosis 20 years previously. Chronic pancreatitis was also detected on endoscopic retrograde examination. After several episodes of digestive bleeding due to ruptured esophageal varixes, the patient died of massive hemorrhage. Postmortem examination showed stage 3 primary biliary cirrhosis and a thick retroperitoneal fibrous plaque, consisting of densely fibrotic areas of collagen with rare vessels and mononuclear cells. We suggest that idiopathic retroperitoneal fi…

Idiopathic retroperitoneal fibrosis and mediastinal fibrosis are localized expressions of a systemic sclerosing disease of unknown cause which, on rare occasions, may coexist in the same patient. Although pericardial involvement may occur, recurrent constrictive pericarditis that is unrelieved by pericardiectomy has not previously been reported in association with either idiopathic retroperitoneal or mediastinal fibrosis. Reported herein is a case of recurrent constrictive pericarditis that was unrelieved by two pericardiectomies , and autopsy revealed unsuspected combined idiopathic retroperi…

A 46-yr-old man, in whom retroperitoneal fibrosis had been found 4 yr previously, presented with abdominal pain, fever, diarrhea, and marked dilation of the transverse colon with superficial ulceration. The megacolon was unresponsive to nasogastric suction, corticosteroids, antibiotics, and total parenteral nutrition. Arteriograms revealed total occlusion of the celiac axis and superior and inferior mesenteric arteries. Laparotomy showed encasement of the retroperitoneal vessels by dense fibrous tissue. A vascular bypass graft was performed, connecting the distal superior mesenteric artery to …

A 9 year old boy presented an "idiopathic" retroperitoneal fibrosis. Fibrosis extended to the liver, compressing the major biliary tree, the portal vein, the urinary tract and the inferior vena cava. The process led two successive bilio-digestive shunts and was controlled after one year corticosteroid treatment. A malignant lymphoma was responsible for the death which occurred 4 years after the initial symptoms of retroperitoneal fibrosis.
