Shigella spp. are a major cause of diarrhea and dysentery in children under 5 years old in the developing world. The development of an effective vaccine remains a public health priority, necessitating improved understanding of immune responses to Shigella and identification of protective antigens. The presence of cryptic fliC alleles in the genomes of 120 strains representative of the four Shigella species was investigated. One fragment was obtained by PCR amplification of fliC, with a size varying from 1.2 to 3.2 kbp, depending on the species or serotype. After digestion with endonuclease HhaI, the number of fragments in patterns varied from three to nine, with sizes of between 115 and 1,020 bp. Two patients with hemolytic uremic syndrome admitted to Udornthani Hospital, Thailand, were reported. The preceding illness was shigellosis. The stool culture was positive for Shigella dysenteriae in one patient and Shigella flexneri in another. The management was successful with peritoneal dialysis in one and symptomatic treatment in another. Both patients had complete recovery.

Shigella commonly causes gastroenteritis but rarely spreads to the blood. During 2002-2012, we identified 11,262 Shigella infections through population-based active surveillance in Georgia; 72 (0.64%) were isolated from blood. Bacteremia was associated with age >18 years, black race, and S. flexneri. More than half of patients with bacteremia were HIV-infected.
