Effects of foot orthoses and valgus bracing on the knee adduction moment and medial joint load during gait.

BACKGROUND: Lateral shoe wedges and valgus knee braces are designed to decrease the force acting in the medial knee compartment by reducing the external adduction moment applied at the knee. The biomechanical changes introduced by these orthoses can be relatively small. Computer modeling and simulation offers an alternative approach for assessing the biomechanical performance of these devices. METHODS: A three-dimensional model of the lower-limb was used to calculate muscle, ligament, and joint loading at the knee during gait. A lateral shoe wedge was simulated by moving the center of pressur

Kinematics and kinetics throughout the entire acceleration phase of 60-m sprints in different footwear conditions.

Footwear influences force production, impact absorption, and sprint performance. While research has examined footwear effects on sprint times and kinematics, effects on ground reaction forces during acceleration remains unclear. This study investigated how barefoot, running shoes, and spiked shoes affect sprint biomechanics throughout acceleration. Seventeen male sprinters performed 60-meter sprints in barefoot, shod, and spiked conditions. Fifty force plates recorded ground reaction forces and spatiotemporal variables over the first 50 meters. Repeated-measures ANOVA tested differences betwee…

Nurse practitioners and barriers to diabetic foot treatment.

BACKGROUND: Even with full practice authority, nurse practitioners (NPs) face limitations in care and treatment options. An example of this is current federal law prohibiting NPs from ordering diabetic shoes. PURPOSE: This study reviewed the background of this problem, potential impacts on patient health and NP practice, and viable solutions for consideration. METHODS: This was a descriptive study of NPs in a northwestern state with full practice authority. An anonymous online survey was used. There were 54 total NP respondents. RESULTS: The potential negative patient outcomes associated wi…

Footwear and footstrike change loading patterns in running.

Loading rates have been linked to running injuries, revealing persistent impact features that change direction among three-dimensional axes … s in the anterior-posterior and medial-lateral directions relative to forefoot running in cushioned shoes. Loading patterns depend on footwear and footstrike interactions, which require shoes that match the runner's footstrike pattern.

Walking on high heels changes muscle activity and the dynamics of human walking significantly.

The aim of the study was to investigate the distribution of net joint moments in the lower extremities during walking on high-heeled shoes compared with barefooted walking at identical speed. Fourteen female subjects walked at 4 km/h across three force platforms while they were filmed by five digital video cameras operating at 50 frames/second. Both barefooted walking and walking on high-heeled shoes (heel height: 9 cm) were recorded. Net joint moments were calculated by 3D inverse dynamics. EMG was recorded from eight leg muscles. The knee extensor moment peak in the first half of the stance …

Management of the diabetic foot.

The increasing prevalence of diabatic foot ulcers (DFUs) is not only costly, but carries a large mortality burden. In this article, we discuss important traditional concepts in the management of DFUs and elaborate on how new technologies have expanded our ability to treat DFUs effectively. New supplies and wound care products have been developed to target the following traditional areas of focus: tissue, infection/inflammation, moisture, and edge. Offloading strategies have grown from standard orthotics or insoles to total contact casting and three-dimensional-printed orthotics to produce the

Effectiveness of offloading methods in preventing primary diabetic foot ulcers in adults with diabetes: a systematic review.

BACKGROUND: The incidence of foot ulceration related to diabetes is increasing. Many foot care professionals recommend offloading measures as part of management strategies for modulating excess pressure to prevent development of diabetic foot ulcers (DFUs). These measures may include padding, insoles/orthotic devices and footwear. There is a lack of evidence-based guidance on the effectiveness of the different offloading options for preventing primary ulceration in those with diabetes. OBJECTIVES: To identify, critically appraise and synthesize the best available evidence on methods of offloa

[Shoemaker's vibration disease].

Exposure to vibration at work with associated health hazards is rather common. Severe symptoms can significantly impair working and functional capacity, whereby occupational health services should invest in the primary and secondary prevention of vibration-induced handicaps. Patients with work-related numbness of the fingers and Raynaud's syndrome should be referred to occupational health service. We describe a shoe worker diagnosed with occupational disease caused by vibration. His profession and way of exposure are unusual, but the symptoms are typical for vibration disease.
