Approach to the treatment of primary anterior shoulder dislocation: A review.

Glenohumeral joint dislocation is common among younger, active patients. Anterior dislocation is the most common direction of instability following a traumatic event. Due to a high rate of recurrence following primary traumatic anterior shoulder dislocation, an evidence-based approach is necessary to determine the best treatment regime for a patient presenting with this problem. A history, physical examination, and radiographic imaging can help guide treatment recommendations by determining the extent of soft tissue damage following dislocation. Controversies in the treatment of the first-time

Arthroscopic Management of the Contact Athlete with Anterior Instability.

Anterior glenohumeral instability is one of the most common injuries suffered from sport. Despite padding and conditioning, the shoulder joint remains particularly vulnerable to injury, especially in the setting of contact. The overall rate of anterior instability is reported to be 0.12 injuries per 1000 athlete exposures, although this is increased up to 0.40 to 0.51 in the contact athlete. Successful treatment requires consideration of restoring stability while minimizing loss of glenohumeral motion. Common treatment strategies involve addressing the pathology that results from anterior shou

MRI of trauma and sports-related injuries of tendons and ligaments. Part I: Spine and upper extremities.

MRI is valuable in evaluation of tendons and ligaments in sports-related injuries. Tendons and ligaments show dark signal intensity on all MR sequences. Alteration in size, shape, continuity, and signal intensity of these structures can be well visualized with MRI. Impingement syndrome is the leading cause of atrophy and degeneration of the rotator cuff. Shape of the acromion, presence of subacromial spur, and involvement in certain sport activities that lead to shoulder instability may cause impingement of the rotator cuff. This article systematically reviews the injuries of tendons and ligam

Effect of Arthroscopic Rotator Cuff Repair Combined With Different Long Head of the Biceps Tendon Fixation Techniques on Postoperative Clinical Outcomes and Shoulder Function.

AIM: Rotator cuff tears and long head of the biceps tendon (LHBT) injuries are primary causes of shoulder pain. During rotator cuff repair, concurrent LHBT injuries are frequently identified. Surgical management of LHBT injuries can effectively relieve pain and improve shoulder function. Two commonly used surgical approaches for LHBT repair are interference screw compression fixation and suture anchor ligation fixation. However, no definitive conclusion has been reached regarding which technique yields superior clinical outcomes. This study compared the efficacy of arthroscopic rotator cuff re

Biomechanical Differences Between Japanese and American Professional Baseball Pitchers.

BACKGROUND: Although baseball injuries are common in both Japan and the United States, the majority of pitching injuries in Japanese players occur at the shoulder, whereas most pitching injuries in American players occur at the elbow. A biomechanical comparison between Japanese and American pitchers may help to identify the different injury mechanisms. HYPOTHESIS: Japanese pitchers produce greater shoulder kinetics whereas American pitchers generate greater elbow kinetics. Also, kinematic differences will be found between the 2 groups, including longer stride and greater lead knee flexion for

Understanding Fibrous Tissue in the Effective Healing of Rotator Cuff Injury.

The rotator cuff is a crucial group of muscles and tendons in the shoulder complex that plays a significant role in the stabilization of the glenohumeral joint and enabling a wide range of motion. Rotator cuff tendon tears can occur due to sudden injuries or degenerative processes that develop gradually over time, whether they are partial or full thickness. These injuries are common causes of shoulder pain and functional impairment, and their complex nature highlights the essential role of the rotator cuff in shoulder function. Scar formation is a crucial aspect of the healing process initiate

Accessory Nerve Disruption Following Core-Needle Biopsy: A Review of Iatrogenic Accessory Nerve Injuries and an Illustrative Case.

Iatrogenic spinal accessory neve (SAN) injury is a rare complication. It most commonly occurs after open lymph node biopsies. Needle biopsy is thought to present lower risk to the SAN. However, we describe a case of a 66-year-old man who underwent core needle biopsy for lymphadenopathy and subsequently experienced ipsilateral shoulder weakness and pain. Physical examination revealed shoulder depression, lateral scapular winging, and decreased range of motion. Subsequent studies and magnetic resonance imaging demonstrated complete paralysis and denervation of SAN muscles. The nonfunctional SAN

Wrist activity monitor counts are correlated with dynamic but not static assessments of arm elevation exposure made with a triaxial accelerometer.

UNLABELLED: There is evidence in the literature of a link between workplace arm elevation exposure and atraumatic shoulder injuries. However, there are several methods that can be used to assess this exposure. The goal of the present study was to compare the outcomes of an activity monitor attached to the wrist and a triaxial accelerometer mounted on the humerus. Twenty-one workers wore both sensors over the course of a full workday. While the activity monitor data was not significantly correlated with any static humeral parameters, it was strongly correlated with all dynamic parameters. The u…

Subscapularis tendon injuries in adolescents: a report of 2 cases.

BACKGROUND: Although shoulder injuries in adolescents are relatively common, injuries to the rotator cuff are relatively rare and not well characterized in the literature. We review 2 cases of adolescent subscapularis tendon avulsions treated surgically. METHODS: The 2 patients were high-level athletes, 1 is a baseball pitcher and the other is a boxer. Both were injured during participation in their respective sports. In each case, the diagnosis was made by magnetic resonance imaging. Patients underwent an open repair by a deltopectoral approach, using suture anchors for fixation. RESULTS: A…

Direct injury to the axillary nerve in athletes playing contact sports.

We performed long-term followup (31 to 276 months) of 11 contact athletes who had sustained isolated injuries to their axillary nerves during athletic competition. There were no known shoulder dislocations. Electromyographs were taken of 10 patients, and all patients had confirmation of clinically defined injuries that were confined to their axillary nerves. Nine injuries were sustained while tackling opposing players in football; two were sustained in hockey collisions. In seven athletes, the mechanism of injury was a direct blow to the anterior lateral deltoid muscle. In four athletes, there

Long-term results of surgical management high-energy scapula fractures at a major trauma center.

BACKGROUND: Scapular fractures are rare injuries typically resulting from high-energy trauma and often occur in polytrauma patients. Historically managed nonoperatively, recent trends in surgical intervention have evolved, particularly in major trauma centers (MTCs) where expertise and case volume have increased. However, there remains limited literature on the surgical management and long-term outcomes of these injuries. METHODS: This retrospective cohort study reviewed all patients who underwent surgical fixation for scapular fractures over a 10-year period at a single UK MTC. Patient demog

[Ultrasonography of the tendons: from image to pathology].

Radiology allows to define etiologies of tendon injuries and authorize a most suitable treatment. For that reason, the use of the sonography has been developed from several years and has four main purposes to define the type of injury and to orient the treatment: to confirm the real existence of the tendon tear, to need the exact location the same one, to determine the gravity of the lesion, and finally, to evaluate its acute or chronic character. In this paper, we will try to define the normal pattern of the tendon and describe the more frequent lesions of the shoulder or the ankle: complete

Case report: pulsed radiofrequency surgery combined with platelet-rich plasma injection in the treatment of supraspinatus injury.

RATIONALE: The shoulder joint is the most movable joint of the human body, and the incidence of aseptic inflammation of the muscles and tendons around the shoulder joint and acute and chronic muscle injuries is relatively high. Pulsed radiofrequency neuromodulation technology is gradually being used in shoulder joint diseases. Platelet-rich plasma (PRP) is a high-power platelet plasma solution obtained by centrifugation of autologous blood. Platelet cells contain many growth factors that promote tissue repair. PATIENT CONCERNS: Shoulder soreness, limited movement of the shoulder joint, abduct

Anatomical relationship of the suprascapular and axillary nerves to the glenohumeral joint and its surgical significance: a systematic review.

PURPOSE: The suprascapular (SSN) and axillary (AXN) nerves are vital for shoulder mobility and are at notable risk for injury during glenohumeral surgeries due to their proximity to key anatomical landmarks. This systematic review evaluates their anatomical positioning relative to the glenohumeral joint and highlights the clinical significance of these relationships in shoulder procedures. METHODS: A thorough literature search across PubMed, MEDLINE, and Google Scholar (2018-2025) yielded 424 studies. After applying inclusion criteria, 21 articles-including clinical, cadaveric, and systematic

[Paediatric brachial plexus injury--an interdisciplinary challenge to obstetricians, neonatologists and plexus surgeons].

Brachial plexus injuries are an interdisciplinary challenge to obstetricians, neonatologists and plexus surgeons. The incidence of brachial plexus injuries is 1-4/1,000 live births, and the incidence of permanent lesions has been estimated to be 1/10,000 live births. Shoulder dystocia is associated with a 75-100-fold increase in plexus injuries. The antenatal (intrauterine) development of brachial plexus injuries is still a matter of controversial debate. The early recognition of antenatal risk factors of shoulder dystocia and its proper management by experienced obstetricians are mandatory; 9
