Weakness in patients with subacromial pain syndrome is local and more pronounced in females.

BACKGROUND: Subacromial pain syndrome is the predominant cause of shoulder pain, accounting for approximately half of all shoulder complaints. This population presents with weakness of the involved shoulder. However, there is a gap in our understanding of how pain contributes to this weakness, and whether there are sex related differences. METHODS: Regional and global isometric strength was tested at the involved shoulder joint and remote joints (uninvolved shoulder and both knees) in patients with subacromial pain syndrome. Data were collected before and after acute pain reduction with a sub…

Glenohumeral osteoarthrosis. A late complication of the Putti-Platt repair.

Osteoarthrosis of the glenohumeral joint is a potential late complication of anterior Putti-Platt capsulorrhaphy. In ten patients (eleven shoulders), disabling pain in the shoulder began an average of 13.2 years after a Putti-Platt repair that had been done for recurrent anterior unidirectional instability. In all of the patients, the osteoarthrosis of the glenohumeral joint resulted in substantial limitation of motion. Seven shoulders were successfully treated non-operatively, and a technique of anterior release was successful in four shoulders.

The shoulder in sports.

Many sports activities that involve the upper extremity entail similar patterns of movement. Analysis of these activities, a better understanding of the throwing motion, and an awareness of shoulder diseases or injuries have led to a rational plan for investigation and management of shoulder problems. Evaluation often extends beyond the usual medical boundaries and must be based on information obtained from an analysis of sports mechanics, a review of training methods, and a physical examination directed at determination of flexibility, strength, endurance, and the presence of inflammation. Multidirectional instability of the shoulder: a systematic review with a novel classification.

PURPOSE: A variety of instabilities are grouped under multidirectional instability (MDI) of the shoulder. This makes understanding its diagnostic process, presentation and treatment difficult due to lack of evidence-based consensus. This review aims to propose a novel classification for subtypes of MDI. METHODS: A systematic search was performed on PubMed Medline and Embase. A combination of the following 'MeSH' and 'non-MesH' search terms were used: (1) Glenohumeral joint[tiab] OR Glenohumeral[tiab] OR Shoulder[tiab] OR Shoulder joint[tiab] OR Shoulder[MeSH] OR Shoulder joint[MeSH], (2) Mult…

Return to play decision after shoulder dislocation in upper limb athletes: Critical analysis between the habits of medical professionals and the literature.

BACKGROUND: The return to field is a critical moment for an athlete who has dislocated his shoulder as there is a significant risk of recurrence. The decision to return to field made by the doctor will therefore be crucial for the smooth continuation of the athlete's career. HYPOTHESIS: This objective is to compare the criteria most used by specialists in clearing an overhead athlete to return to competition after a first episode of antero-internal dislocation of the glenohumeral joint with or without surgery and those mentioned in the literature. PATIENTS AND METHODS: The target population

Arthroscopic Bankart repair versus conservative treatment for first-time traumatic anterior shoulder dislocation: a systematic review and meta-analysis.

BACKGROUND: Shoulder is vulnerable to dislocation owing to its anatomical structure and the increasing popularity of contact sports in young population. The management of first-time anterior shoulder dislocation in this group is still controversial and the prognosis are varied. This review aimed to compare the results of arthroscopic Bankart repair and conservative management for first-time traumatic anterior shoulder dislocation in young active patients. METHODS: Databases were searched till November 2021, and comparative studies between arthroscopic Bankart repair and conservative managemen

Development and Validation of a Short-Form Version of the Western Ontario Shoulder Instability Scale (Short-WOSI).

BACKGROUND: Patient-reported outcome measures (PROMs) have transitioned from primarily being used as research instruments to becoming increasingly used in the clinical setting to assess recovery and inform shared decision-making. However, there is a need to develop validated short-form PROM instruments to decrease patient burden and ease incorporation into clinical practice. PURPOSE: To assess the validity and responsiveness of a shortened version of the Western Ontario Shoulder Instability Index (Short-WOSI) when compared with the full WOSI and other shoulder-related PROM instruments. Primary Open Latarjet Procedure Results in Functional Differences but No Structural Changes in Subscapularis Muscle Quality vs the Healthy Contralateral Shoulder at Long-term Follow-up.

BACKGROUND: There are concerns that the Latarjet procedure results in loss of glenohumeral rotation and strength and in subscapularis dysfunction. The long-term effects of this procedure on subscapularis quality, glenohumeral rotation, and strength are unknown. PURPOSE/HYPOTHESIS: To analyze the long-term effect of the primary open Latarjet procedure using a muscle-splitting approach on internal and external rotation and strength, as well as subscapularis muscle quality as compared with the healthy contralateral side. We hypothesized that the primary open Latarjet procedure is associated with

Editorial Commentary: The Latarjet: Increased Shoulder Stability, and Increased Risk of Complications in Low Volume Practitioners.

The Latarjet procedure is a well-established treatment for shoulder instability. It is traditionally used for recurrent instability with significant bone loss (i.e. >20%) or after failed soft tissue procedures. It can be done open or arthroscopically with either a subscapularis takedown or a split. It has good long-term results regarding recurrence and clinical outcomes. Unfortunately, it has higher complication rates in comparison to its alternative procedures such as arthroscopic Bankart repair or open Bankart repair. These complications may be due to the learning curve of performing the pro

[Reliability of three dimensional CT measurement of glenoid track in patients with recurrent anterior dislocation of shoulder].

OBJECTIVE: To evaluate the reliability and repeatability of three-dimensional (3D)-CT measurement of glenoid track. METHODS: Glenoid and Hill-Sachs bone defects in 60 patients with recurrent anterior dislocation of the unilateral shoulder were evaluated by 3D-CT and analyzed by three independent observers(shoulder orthopedists) according to the Di Giacomo method. These injuries were classified to formulate a surgical protocol. All the patients were repeatedly measured 1 week later. Intraclass correlation coefficient (ICC) and coefficient of variation (COV) were used for reliability analysis.

Does the posterior glenoid osteotomy reduce the rate of recurrence in patients with posterior shoulder instability - A systematic review.

BACKGROUND: Posterior shoulder instability (PSI) is becoming an increasingly recognised condition. A number of different treatment modalities exist to treat PSI including arthroscopic or open surgeries when non-operative treatment has failed. The primary aim of this systematic review was to analyse the rate of recurrent instability after posterior glenoid osteotomy (PGO) for recurrent PSI, while secondary aim was to identify complication rate and the amount of retroversion correction. PATIENTS AND METHODS: A review of the online databases MEDLINE and Embase was conducted on 1 November 2019 ac

Editorial Commentary: Is Arthroscopic Bankart Repair Using Suture Anchors on the Glenoid Appropriate Treatment for Traumatic Anterior Shoulder Instability?

Fracture of the anterior glenoid rim along the sites of suture anchor insertion is not rare after arthroscopic Bankart repair for traumatic anterior shoulder instability. In addition to the influence of the number, type, and size of the suture anchors, placing multiple anchors in a linear arrangement might impose excessive stress on the surrounding bone, leading to critical loss of osseous integrity and glenoid fracture. Although highly active young male collision or contact athletes are most at risk, such fractures sometimes occur after relatively minor trauma at a long interval after surgery
