Experience with a new pure gold piston in stapedotomy for cases of otosclerosis.

This study reports the evaluation of the hearing results after 62 implantations of a pure gold (99.9%) piston in cases of otosclerosis. A retrospective analysis of the pre- and postsurgery audiological results of these patients was carried out by microcomputer. This study shows that the use of a pure gold piston ((K-piston) with a diameter of 0.4 mm gives good results in cases of stapedotomy for otosclerosis. The hearing results show a closure of the pure-tone average (PTA) air-bone gap to within 10 dB was 70.9% (44/62) and to within 20 dB in 95.1% (59/62). A residual airbone-gap of > 20 dB wa

Two cases of selective developmental voice-recognition impairments.

Recognizing other individuals is an essential skill in humans and in other species. Over the last decade, it has become increasingly clear that person-identity recognition abilities are highly variable. Roughly 2% of the population has developmental prosopagnosia, a congenital deficit in recognizing others by their faces. It is currently unclear whether developmental phonagnosia, a deficit in recognizing others by their voices, is equally prevalent, or even whether it actually exists. Here, we aimed to identify cases of developmental phonagnosia. We collected more than 1,000 data sets from sel

The otologic approach in the management of posterior petrous surface meningiomas.

PURPOSE: Report our experience in the management of posterior petrous surface meningiomas (PPSMs), and identify features that affect hearing, facial nerve (FN) function, and control of the disease. METHODS: Retrospective case series of 131 patients surgically managed for PPSMs. FN status, hearing and tumour radicality were assessed and compared between patients with tumours of different locations (Desgeorges classification) and internal auditory canal involvement (IAC). RESULTS: At the time of surgery 74.8% of patients had a hearing loss. Hearing was mostly unserviceable in tumors attached t

Sudden deafness and anterior inferior cerebellar artery infarction.

BACKGROUND AND PURPOSE: Acute ischemic stroke in the distribution of the anterior inferior cerebellar artery (AICA) is known to be associated with vertigo, nystagmus, facial weakness, and gait ataxia. Few reports have carefully examined the deafness associated with the AICA infarction. Furthermore, previous neurological reports have not emphasized the inner ear as a localization of sudden deafness. The aim of this study was to investigate the incidence of deafness associated with the AICA infarction and the sites predominantly involved in deafness. METHODS: Over 2 years, we prospectively iden

Patterns of Aided Loudness Growth in Experienced Adult Listeners with Early-Onset Severe-Profound Hearing Loss.

BACKGROUND: Individuals with early-onset severe-profound bilateral hearing loss (S/PHL) manifest diverse levels of benefit and satisfaction with hearing aids (HAs), even with prescriptive HA fitting. Such fittings incorporate normal loudness values, but little is known about aided loudness outcomes in this population and how those outcomes affect benefit or satisfaction. PURPOSE: To describe aided loudness growth and satisfaction with aided listening in experienced adult HA users with S/PHL. RESEARCH DESIGN: The Contour Test of loudness perception was administered to listeners with S/PHL in

Multimodal, multi-device wearable phenotyping for early childhood mental health: balancing predictive performance and implementation burden.

Childhood mental health conditions such as ADHD, anxiety, and depression affect 13- 20% of children, yet 25-62% go undetected and untreated. Pediatric digital phenotyping could add objective signal, but prior work has largely tested single modalities, leaving open which signals matter most and whether combining them helps. We analyzed electrodermal, cardiovascular, temperature, movement, and speech (acoustic and linguistic) data from 103 children aged 4-8 during a ∼7-minute structured behavioral assessment. Machine-learning models trained against gold-standard clinical-interview diagnoses disc

Recovery of bilateral vestibular loss in Cogan's syndrome--a case report.

OBJECTIVE: To show that both hearing and vestibular loss can be reversible and quantified in Cogan's syndrome. PATIENT AND METHODS: Immunosuppressive medication was continued for more than 6 years in a young woman with Cogan's syndrome. Standard pure tone audiometry (PTA) and speech discrimination score (SDS) for hearing, motorized head impulse test (MHIT) for horizontal, angular vestibulo-ocular reflex, and erythrocyte sedimentation rate were followed serially. RESULTS: The PTA before medication was 59/74 dB, and it stabilized to 13/46 dB in 2 years. The initial SDS of 92/72% improved later

Tinnitus selectively disrupts phoneme level speech perception.

Chronic subjective tinnitus has been linked to impaired speech-in-noise perception, yet its impact on phoneme level processing remains unclear. We assessed phoneme discrimination in 49 normal hearing participants (26 with chronic subjective tinnitus and 23 matched controls), with matched audiometric thresholds and supra-threshold processing, and found no statistically significant differences between groups on the assessed cognitive measures, using a two-alternative forced-choice identification task with fricatives (/s, ʃ/) and plosives (/ph, t/) presented in quiet and individualized noise cond

Combined Electric acoustic stimulation in a patient with otitis media with antineutrophil cytoplasmic antibody-associated vasculitis.

There are few reports of the treatment for severe hearing loss due to otitis media with antineutrophil cytoplasmic antibody-associated vasculitis (OMAAV) achieved by cochlear implantation (CI). Here, we have reported the case of a patient with severe bilateral sensorineural hearing loss with low-frequency residual hearing by OMAAV. CI was performed in her right ear based on the results of contrast-enhanced magnetic resonance imaging (CE-MRI) and promontory stimulation test (PST). The residual hearing in her right ear was preserved after CI and utilized for combined electric acoustic stimulatio

Incidence of genetic factors in the causation of deafness in childhood.

Routine investigations of 1568 children with severe-profound hearing loss, during a 15 years' period, revealed the following classification: Hereditary Deafness 25%, Acquired Deafness 43% (prenatally 11%, perinatally 16%, postnatally 16%), Unknown Deafness 32%. This group of unknown causes is usually large in any surgery of deafness and in many of these children deafness is believed to have a genetic basis. In the light of this premise, we performed more careful genetic and clinical investigation in 268 children coming under observation in the 3 last years. Moreover, we examined a group of 44

Cross-Domain Transfer of Depression Voice Biomarkers Depends on the Outcome Instrument: Leakage-Controlled Cross-Sectional Evaluation Study.

Whether voice biomarkers of depression generalize across clinical settings is largely untested. Generalization is usually framed as a question about populations. It is also a question about the outcome instrument a model is scored against, a dimension confounded in existing studies with all else that differs between them. In a US-nationwide online study, 446 sessions from 390 pregnant participants at 22 weeks' gestation (analytical N=316) each gave four voice recordings, the PHQ-8 and a modified 9-item EPDS (mEPDS-9). Discrimination was assessed under leakage-controlled cross-validation, with

Subclinical dysfunction of cochlea and cochlear efferents in migraine: an otoacoustic emission study.

Otoacoustic emission (OAE) testing enables us to identify the cochlear component of a hearing disorder and to monitor objectively minute changes in cochlear status undetectable by other audiological methods. Contralateral sound-induced suppression is mediated by medial superior olivary complex efferents which induce hyperpolarization counteracting the amplifying effects of outer hair cell (OHC) activity. The aim of this study was to assess functions of cochlea and its efferents in migraine using OAE testing and contralateral suppression of transiently evoked OAEs (TEOAE). Fifty-three migraineu

Cross-cultural studies using a modified mini mental test for healthy subjects and patients with various forms of vascular dementia.

Existing neuropsychological tests are often complex and time-consuming. We designed a modified Mini Mental Test (MMT) battery for clinical assessment of the global and regional higher cortical functions of the brain. We tested its applicability in healthy subjects with different ethnic, cultural and educational backgrounds. The usefulness of our MMT as a tool for the clinical evaluation of patients with various forms of vascular dementia was determined. The MMT comprises five subtests, including clinical evaluations of: (A) orientation (6 points); (B) attention, right-left discrimination, spee

Vestibular schwannoma in the only hearing ear: cochlear implant or auditory brainstem implant?

OBJECTIVE: To explore the dilemma faced by neurotologists confronted with the patient who develops a vestibular schwannoma in the only hearing ear, the other having been deaf from birth, and to consider the choice between auditory rehabilitation using a cochlear implant (CI) on the congenitally deaf side and an auditory brainstem implant (ABI) on the tumor side. STUDY DESIGN: A record review of two patients born deaf in one ear and who developed a vestibular schwannoma in the contra lateral ear, who then received a CI in the congenitally deaf ear. SETTING: Tertiary referral center with speci

Neurologic Characterization of Craniosynostosis: Can Direct Brain Recordings Predict Language Development?

PURPOSE: Nonsyndromic craniosynostosis (NSC) is associated with language deficits. Conventional tests, such as the Bayley Scales of Infant Development (BSID), may not reflect accurate long-term cognition. Alternatively, mismatch negativity (MMN) waves recorded via electroencephalogram (EEG) measure neural responses to speech and may objectively predict language development. This study aimed to (1) correlate infant MMN to future language achievement and (2) compare MMN among subtypes of NSC. METHODS: Pre and postoperatively (mean operative age 9.5 months), NSC participants received the BSID an

Auditory detection and perception in normal man and in patients with adrenal cortical insufficiency: effect of adrenal cortical steroids.

Auditory detection thresholds for sinusoidal tones and various tests of auditory perception were determined in 12 patients with adrenal cortical insufficiency (seven with Addison's disease and five with panhypopituitarism) and compared to those in normal volunteers. In adrenal cortical insufficiency auditory detection sensitivity was significantly more acute than normal, and judgments of loudness and of the contralateral threshold shift were made at levels more than 20 db below those of normal subjects. Thus both the lower and the upper limits of the dynamic auditory range are significantly de
