Meningeal sporotrichosis in an immunocompetent host: A case report.

A 78-year-old woman presented to hospital with altered mental status. Hyponatremia was diagnosed and treated. She maintained mental confusion despite normal sodium. No headache, fever or focal signs were present. CSF analysis showed chronic meningitis; MRI demonstrated basilar enhancement. She was empirically treated for tuberculosis, with no improvement. PCR for Sporothrix in the CSF was positive. After treatment with Amphotericin followed by oral itraconazole, she completely recovered her cognitive abilities. Follow-up CSF was normal. This report illustrates the need to consider sporotrichos…

Sporotrichosis in the metropolitan area of Cusco, Peru, and in its region.

Eight cases of sporotrichosis originating from the metropolitan area of Cusco, Peru, and its region are described, including the circumstances of infection and the isolation of Sporothrix schenckii from the lesions. This finding classifies this particular area with high altitude, low temperature and dry weather as an endemic zone of interest in the epidemiological and ecological study of Andean sporotrichosis. Among the eight cases observed, five were of the fixed cutaneous type and three were lymphocutaneous. Six patients were male and two female. Two paediatric cases were also observed. Ther…

Sporotrichal Tenosynovitis Diagnosed Helpfully by Musculoskeletal Ultrasonography.

A 72-year-old man presented with persistent oligoarthritis and positive results for rheumatoid factor and was suspected of having rheumatoid arthritis (RA). However, the musculoskeletal ultrasonography (MSUS) findings were not consistent with those of typical RA. He had undergone surgery for carpal tunnel syndrome, which allowed both histopathological and microbiological examinations to be performed. A synovial tissue culture was positive for Sporothrix schenckii, and he was diagnosed with sporotrichal tenosynovitis. He received anti-fungal therapy, and the sporotrichal tenosynovitis resolved.…

Sporotrichosis: clinical and histopathological manifestations.

Sporotrichosis is a chronic fungal infection with a global distribution; both sporadically occurring infections and outbreaks of cases are not uncommon. Sporothrix schenckii, as the causative agent, can be isolated from soil, wood, land, and even marine animals. We present the clinical and histopathological presentations of sporotrichosis collected in our department for about 40 years. A series of interesting pictures illustrating asteroid bodies and organism materials, which are rarely seen, are also showed. Asteroid body is characterized by the presence of radiating star-like asteroid or clu…

Ocular Sporotrichosis.

PURPOSE: To present 10 cases of Parinaud oculoglandular syndrome caused by sporotrichosis. OBSERVATIONS: We report 10 cases of Parinaud oculoglandular syndrome after contact with domestic cats diagnosed with sporotrichosis. They all showed ocular hyperemia associated with unilateral tarsal conjunctival granulomas. After histopathological study and culture of the scrapings and conjunctival secretions, six patients were positive for Sporothrix schenkii and four had a presumed diagnosis of Parinaud oculoglandular syndrome due to sporotrichosis. Treatment with 200 mg/day of oral itraconazole was …

Successful treatment of terbinafine in a case of sporotrichosis.

Sporotrichosis is a chronic subacute infection caused by fungi belonging to the Sporothrix Complex. In the present clinical case, nasal sporotrichosis was treated with potassium iodide. This was unsuccessful, and the treatment was restarted with a combination of potassium iodide and itraconazole. This however resulted in a further recurrence of the infection. The mycological cultures were tested in vitro for antifungal activity to assist in treatment. Terbinafine, an antifungal drug, produced the best results and was therefore used for the rest of the treatment course, with no recurrence after…

Cutaneous sporotrichosis of face: polymorphism and reactivation after intralesional triamcinolone.

Cutaneous sporotrichosis, a subcutaneous mycotic infection is caused by the saprophytic, dimorphic fungus Sporothrix schenckii. It commonly presents as lymphocutaneous or fixed cutaneous lesions involving the upper extremities with facial lesions being seen more often in children. The lesions are polymorphic. The therapeutic response to saturated solution of potassium iodide is almost diagnostic. We describe a culture-proven case of cutaneous sporotrichosis of the face mimicking lupus vulgaris initially and basal cell carcinoma later, who did not tolerate potassium iodide and failed to respond…

Current situation of sporotrichosis in China.

Sporotrichosis, a mycosis resulting from cutaneous or subcutaneous infection with the dimorphic fungus Sporothrix, has been reported in China, particularly in the northeast region. In this review, we conducted a thorough examination of the recent advancements in sporotrichosis in China, encompassing aspects such as etiology, epidemiology, pathogenesis, clinical manifestations, diagnosis and treatment strategies. Within the Chinese context, fixed cutaneous sporotrichosis represents the prevailing clinical manifestation. Fungal culture stands as the gold standard for diagnosing sporotrichosis, w…

Zoonotic sporotrichosis: Systematic review and clinical aspects of feline and canine cases.

INTRODUCTION: Sporotrichosis is a worldwide endemic mycosis caused by thermodimorphic fungi of the genus Sporothrix. Of the around 70 Sporothrix species, four are classified within the clinical or pathogenic clade (Sporothrix schenckii, S. brasiliensis, S. globosa, and S. luriei), which are usually isolated from animal and human infections. The disease shows various clinical presentations (fixed and disseminated cutaneous, lymphocutaneous, systemic, or extracutaneous forms), with itraconazole being the antifungal of choice in most cases. The cat is the key player in the zoonotic scenario of sp…

Sporotrichosis: an overview of the neglected disease reported worldwide in the last five years.

Sporotrichosis is a fungal infection caused by species belonging to the genus Sporothrix. It primarily affects domestic cats, which are considered definitive hosts, and can be transmitted to humans and other animals in various regions worldwide. We compiled information on the clinical aspects, diagnoses, clinical manifestations, prevention, and therapeutic management of cases reported in the literature between 2018 and 2022. The databases used were BVS-Bireme, PubMed, Scopus, and Web of Science. In total, 121 studies were included. S. brasiliensis emerged as the most prevalent species, althoug…

First three cases of cat-associated zoonotic cutaneous sporotrichosis in Colombia.

BACKGROUND: Sporotrichosis is a subcutaneous mycosis caused by dimorphic fungi of the genus Sporothrix. The route of entry of the causative agent is direct inoculation by skin trauma, generally with decaying wood and other vegetation. However, cat-transmitted sporotrichosis has become more relevant in recent decades especially in South America. Until now, there are no case reports of cat-transmitted sporotrichosis in Colombia. CASE REPORT: We present three cases of cutaneous sporotrichosis after the scratches of an ill cat, in the feline's owners and its veterinarian, confirmed by culture and…

Systemic sporotrichosis in an alcoholic patient.

A 44-year-old male patient presented with nodules that evolved with inflammation, following drainage of seropurulent secretion and ulceration. The patient had a 6 year-history of alcohol addiction and reported contact with cats. At the physical examination, the patient had skin-colored and erythematous nodules, and ulcers covered with thick, blackened crusts on the face, trunk and limbs. A culture of a nodule fluid revealed growth of Sporotrix sp. He also had pulmonary involvement and therefore the disease was classified as systemic sporotrichosis, a rare form that usually affect patients infe

Sensitivity of cytopathological examination in the diagnosis of feline sporotrichosis.

Sporotrichosis is caused by Sporothrix schenckii. The cat is the animal species most affected by this mycosis and plays an important role in the zoonotic transmission of this disease. The definitive diagnosis is made by isolation of the fungus in culture; however, cytopathological examination is frequently used in cats. Medical records from cats treated at Instituto de Pesquisa Clínica Evandro Chagas/Fiocruz, Rio de Janeiro, Brazil, between 2004 and 2006 were reviewed. Criteria for inclusion were a diagnosis by isolation of S schenckii in culture and cytopathological examination of the same le…

Sporotrichosis in India: first authentic case report from the north-western region and a critical literature review.

The first case of sporotrichosis from north-western region is reported along with a critical review of the work done on this disease in India. The patient, a 55-year-old housewife with multiple lymphocutaneous lesions on her left arm, had never travelled to north-eastern India where sporotrichosis occurs endemically. She was a resident of Ranikhet, District Nainital, Uttar Pradesh (U.P.), situated about 235 km north-east of Delhi. The diagnosis was established by demonstration of Sporothrix schenckii through direct microscopy and culture of pus collected from multiple suppurative lesions, veri…
