The aim of this study was to evaluate the stress distribution in cortical and cancellous bones and within the screw when a stainless steel extra-alveolar screw (EA-S) is inserted into the retromolar region of the mandible using the finite element method (FEM). The stress values in the EA-S were highest at 60° with horizontal (shear) force and lowest at 90° with axial force. In the bones (cortical and cancellous), the stress value was greatest at 90° with horizontal force. Greater stability of the screw may be achieved by tilting it to the force when horizontal loads are applied.

This article describes a new method that enables vector control during alveolar distraction osteogenesis in the treatment of a cleft palate patient. The patient presented with unilateral complete cleft lip and palate, and the alveolar part of the defect was covered by a mobile buccal flap. The distraction was performed by sliding the surgically released tooth segment with the help of an intraoral distractor over 1.5-mm stainless steel archwires held by metal crowns. This vector-controlled method enabled new bone and attached gingiva formation in harmony with the proper alveolar shape.

A 3-and-a-half-year-old male child presented with fused cariously involved right maxillary primary central and lateral incisors as well as a previously traumatized non-vital left primary central incisor with a draining fistula. A Thomas's splint with an adjustable ring has been devised with a stainless steel hose clip, which can be adjusted up to 5 or 7.5 cm, attached on the lateral side and a hinge attached on the medial aspect of the anterior half of the ring. Thus when there is swelling in the upper part of the thigh the ring can be widened and when it goes down the ring can be tightened again so that it remains an exact fit. This prevents pressure sores developing and avoids cutting the ring in order to relieve pressure from swelling.

Transvenous placement of inferior vena cava (IVC) filters has become commonplace in selected patients with deep venous thrombosis (DVT) and pulmonary embolism (PE). IVC filters have been shown to have excellent therapeutic efficacy and low complication rates. Penetration of the IVC by filter hooks or struts has been reported and commonly noted to be inconsequential. We report a laceration of a lumbar artery by a stainless steel Greenfield (SSG) filter strut that resulted in a near fatal hemorrhage, and review the world literature on caval perforation by IVC filters.

Seventeen unstable thoracic and thoracolumbar fracture-dislocations have been treated by Harrington double-distraction rod technique supplemented by doubled 18-gauge stainless-steel wires applied to two intact laminae above and below the injury.
