We describe a patient who entered a stuporous state after receiving benzodiazepine treatment for generalized tonic-clonic status epilepticus. A diagnosis of generalized NCSE with tonic seizures was made on the basis of the clinical picture and response to barbiturate anaesthetic, although the EEG pattern was not typical of the changes previously described in tonic seizures-tonic status epilepticus. We report on two patients with transient abnormalities on MRI during status epilepticus(SE). Diffusion-weighted MR imaging(DWI) and T2-weighted and fluid-attenuated inversion recovery (FLAIR) MR images showed cortical and thalamic hyperintensity with swelling of the cortex. Midazolam clearance was examined in two patients with medically refractory convulsive status epilepticus. One patient received a constant infusion of midazolam for 68 hours and the other patient received a constant infusion of midazolam for 148 hours. In both patients the decline in level was, overall, much slower than expected. A retrospective case note review was conducted of 47 patients of 15 years and older who had sustained simple partial status epilepticus (SPSE) in The Netherlands between 1980 and 1987. In 46 patients the type of SPSE was somatomotor (in four adversive), and in one aphasic with visual and auditory hallucinations. SPSE was more common over the age of 50. Six of 27 patients with previous epilepsy had an acute symptomatic cause. In 20 patients without previous epilepsy stroke was the most frequent cause (75%). A 4-year-old girl with status epilepticus and severe encephalopathy associated with cat-scratch disease is described. She had prolonged seizures lasting more than 2 weeks and required treatment with pentobarbital coma. She developed unusual abnormalities on magnetic resonance imaging scans that involved the thalami and the cortex of the temporal, parietal, and occipital lobes. She has developed a persistent encephalopathy manifesting as visual disturbance and frequent partial seizures.
