A 50-year-old man, with history of chronic alcohol intake was brought in a stuporous state to the emergency services having been found in that condition in his home the same day. Examination revealed the patient in an akinteic mute state with apparently normal cranial nerves, hypotonia and quadriplegia with bilateral extensor plantar reflex. CT scan and MRI of the brain revealed bilateral infarct parasagittally with normal Magnetic resonance venogram suggestive of bilateral anterior cerebral artery infarct. Follow-up magnetic resonance angiogram revealed an azygous anterior cerebral artery thu…

There are no reports of xylene/amitraz ingestion in children. Studies have characterized the adverse reactions caused by dermal applications in dogs, but the mechanisms involved were undetermined Accidental ingestion of the product by a child prompted a retrospective search of the medical records of the Georgia Animal Poison Information Center to profile the signalment, clinical signs, route of exposure and target sites of the suspected toxicosis in dogs. Clinical signs of depression, ataxia, stupor, and coma were most attributable to the xylene and propylene oxide components. Hyperglycemia, h…

A case of a 51 year old female with herpes encephalitis is reported. She underwent surgery for chronic pancreatitis with pseudocyst formation. On the third postoperative day, she developed a severe vesicular nasolabial eruption associated with a deep stupor. Anti-Herpes simplex viral (HSV) antibodies were found in both blood and cerebrospinal fluid. Klapper's anti-HSV antibody ratio was calculated and agreed with the hypothesis of herpes encephalitis. The patient was given nucleoside analogues. She rapidly improved and was discharged from the intensive care unit without any sequelae. The frequ…

A 19 years' old white male presented with a stuporous state, initially suspected to be of a toxic origin. It appeared soon that the patient had a left oedematous cerebellar infarction with obstructive hydrocephalus. The angiographic study has shown an occlusion of the left vertebral artery at the level of C2. After reviewing the literature about the different etiologies of vertebral occlusions in the young people, it appeared likewise that, in this case, it was the result of a traumatic compression due to an alcoholic coma. Despite an external ventricular drainage, the neurological state worse…

Computed tomography was performed on 1,378 patients admitted within 3 days of subarachnoid hemorrhage (SAH) at 71 centers in a cooperative study. CT on the day of SAH (day 0) was abnormal in 95.8%. Intracranial blood was detected in 95.3% of patients on day 0, 90.5% on the first day after SAH (day 1), and 73.8% on day 3. CT was normal in only 1 of 284 patients in stupor or coma but was normal in 14.6% of 638 alert patients. In the first few days after SAH, CT is highly accurate, often obviating the need of CSF examinations. The interval from SAH until CT and the seriousness of the patient's co…

The symptoms of 2269 psychiatric patients on admission, documented by the AMP (PAS) system, were factor-analysed to build syndromes of psychopathology. The nine syndromes could be cross-validated. We believe a useful description of the findings on admission can be made by the following syndromes: paranoid-hallucinatory, manic, psycho-organic, depressive, apathetic, hostile, stuporous, somatic, compulsive. The correlation with other syndromes based on AMP by other scientists is substantial. For each item we computed the percentage of occurrence and the item-scale correlation, and for the syndro…

Four patients with pathologically documented polycystic cavitation of the brain had an acute illness characterised by stupor, seizures, CSF erythrocytic and monocytic pleocytosis, increased CSF protein, and diminished CSF glucose. The acute phase was followed by chronic decerebation, disappearance of the CSF abnormalities, and radiological evidence of polycystic cavitation of the brain. In one patient Herpes simplex was isolated from a cutaneous vesicle. The CSF abnormalities in the disorder have received scant attention, and have not previously been correlated with the acute and chronic stage…

Delirium is a frequently encountered clinical syndrome which can pose serious problems for the physician and patient. Numerous etiological possibilities exist, and each case is usually associated with multiple causal factors. Although the pathophysiology is poorly understood, the clinical presentation is marked either by stupor and hypoarousal or agitation and hyperarousal. Both types of delirium must be treated by searching for and correcting reversible causative factors. In addition, medication may be quite efficacious in managing the clinical aspects of agitated delirium. Most cases of agit…

Six cases of dissecting aneurysm of the aorta, four with cerebral manifestations, one with peripheral neuropathy and one with ischaemic necrosis of the spinal cord are reported. Cerebral manifestations include confusion and stupor, syncope, grand mal epilepsy, ischaemia of the spinal medulla, carotid artery occlusion and cerebral hemisphere infarction. Ischaemic neuropathy is characterized by severe limb pain not of peripheral nerve distribution, but associated with areflexia and a peripheral pattern of sensory loss. Ischaemic necrosis of the cord is characterized by flaccid paraplegia and sph…
