CSF-Venous Fistula Transvenous Onyx Embolization: Evaluation of Onyx Migration into the CSF and Potential One-Way Physiology.

BACKGROUND AND PURPOSE: CSF-venous fistulas (CVFs) are abnormal connections between the subarachnoid space and a paraspinal vein. Transvenous Onyx embolization is a recently adopted treatment method for CVF closure, however no studies have specifically evaluated for Onyx migration into the CSF. The purpose of our study was to evaluate patients who underwent transvenous CVF embolization for Onyx migration into the CSF. MATERIALS AND METHODS: We evaluated 100 patients who underwent transvenous CVF embolization with posttreatment CT of the spine. Images were reviewed for Onyx migration into the …

A Rare Case of Cerebral Amyloid Angiopathy-Related Inflammation With Subarachnoid Diffusion Restriction Mimicking Rheumatoid Meningitis.

Some patients with cerebral amyloid angiopathy (CAA), which is characterized by amyloid β fibril deposition in cortical and leptomeningeal vessels, develop an inflammatory response, leading to CAA-related inflammation (CAA-RI). In such cases, histopathologically confirmed vasculitis is defined as amyloid β-related angiitis (ABRA). Here, we report the rare case of an 80-year-old woman who presented with anomic aphasia and mild right upper limb weakness. Magnetic resonance imaging revealed restricted diffusion in the subarachnoid space along the cerebral sulci and dura mater, mainly within the l…

Spontaneous intraventricular rupture of craniopharyngioma cyst.

BACKGROUND: Rupture of a cystic craniopharyngioma is a rare phenomenon. The rupture of the cyst causes decompression of the adjacent neural structures resulting in spontaneous improvement of the visual symptoms or level of sensorium. The leakage of its contents into the subarachnoid space gives rise to meningismus. We report an extremely rare phenomenon of an intraventricular rupture of a cystic craniopharyngioma, which resulted in acute neurological deterioration and chemical ventriculitis. CASE DESCRIPTION: A 38-year-old lady presented with a 1-year history of frontal lobe dysfunction and b…

Comparison of the hyperglycaemic effect of adrenaline and morphine introduced into the liquor space.

1. In unanaesthetized cats a comparison is made of the hyperglycaemic effects of adrenaline and morphine, when injected or infused through chronically implanted cannulae, into different regions of the cerebral ventricles or of the subarachnoid space, in order to determine their sites of action. 2. On injection into the cerebral ventricles both adrenaline and morphine have to reach the subarachnoid space beneath the ventral surface of the brain stem before they can exert their hyperglycaemic effect. The adrenaline has to reach the region rostral to the pons, i.e. the fossa interpeduncularis, an…

The glymphatic system in migraine and other headaches.

Glymphatic system is an emerging pathway of removing metabolic waste products and toxic solutes from the brain tissue. It is made of a network of perivascular spaces, filled in cerebrospinal and interstitial fluid, encompassing penetrating and pial vessels and communicating with the subarachnoid space. It is separated from vessels by the blood brain barrier and from brain tissue by the endfeet of the astrocytes rich in aquaporin 4, a membrane protein which controls the water flow along the perivascular space. Animal models and magnetic resonance (MR) studies allowed to characterize the glympha…

New evaluation method using preoperative magnetic resonance imaging for cervical spondylotic myelopathy.

A new method based on the score of preoperative magnetic resonance images (MRI) was devised to evaluate cervical spondylotic myelopathy and predict the results of cervical laminoplasty. On T1- and T2-weighted sagittal MRI, the intervertebral disc spaces at each level from the axis to the first thoracic spine were examined as to whether the anterior or posterior subarachnoid space would be maintained or not, and for the presence or absence of spinal cord deformity. The data were divided into six grades and rated, and the total score for all sites was regarded as the preoperative MRI cumulative …

Cervical intradural disc herniation: a new diagnostic clue based on MRI T2 hyperintensity and intraoperative increased water content in the intervertebral disc-a case report and mechanistic insights.

BACKGROUND: Cervical intradural disc herniation (CIDH) is a rare spinal condition characterized by herniation of nucleus pulposus through the dura mater into the subarachnoid space, resulting in spinal cord or nerve root compression. Its rarity and nonspecific imaging features make preoperative diagnosis particularly challenging. Preliminary investigations into human neurofluid transport using multiple novel non-contrast MRI methods.

We discuss two potential non-invasive MRI methods to study phenomena related to subarachnoid cerebrospinal fluid (CSF) motion and perivascular fluid transport, and their association with sleep and aging. We apply diffusion-based intravoxel incoherent motion (IVIM) imaging to evaluate pseudodiffusion coefficient, D*, or CSF movement across large spaces like the subarachnoid space (SAS). We also performed perfusion-based multi-echo, Hadamard encoded arterial spin labeling (ASL) to evaluate whole brain cortical cerebral blood flow (CBF) and trans-endothelial exchange (Tex) of water from the vascu…

Imaging of cerebrospinal fluid flow: fundamentals, techniques, and clinical applications of phase-contrast magnetic resonance imaging.

Cerebrospinal fluid (CSF) is a dynamic compartment of the brain, constantly circulating through the ventricles and subarachnoid space. In recent years knowledge about CSF has expended due to numerous applications of phase-contrast magnetic resonance imaging (PC-MRI) in CSF flow evaluation, leading to the revision of former theories and new concepts about pathophysiology of CSF disorders, which are caused either by alterations in CSF production, absorption, or its hydrodynamics. Although alternative non-invasive techniques have emerged in recent years, PC-MRI is still a fundamental sequence tha…
