Urine Drug Testing among Opioid-Naïve and Long-Term Opioid Nevada Medicaid Beneficiaries.

Current guidelines recommend that, when prescribing opioids, providers use urine drug testing (UDT) for harm reduction. Objective: To identify whether Medicaid beneficiaries in Nevada at increased risk for opioid misuse received UDT. Methods: We used Nevada Medicaid claims data (2017-2018) to describe UDT among three samples: opioid naïve patients (N = 11,326), opioid naïve patients with a second follow-up prescription (N = 8,910), and long-term opioid patients (N = 19,173). Predictors of opioid misuse include past diagnoses of mental health and substance use, demographic characteristics and p

Mental health of extremely low birth weight survivors in their 30s.

OBJECTIVE: To determine the risk for psychiatric disorders among extremely low birth weight (ELBW) survivors in their early to mid-30s and to determine whether those born small for gestational age or those exposed to a full course of antenatal corticosteroids (ACS) were at particularly high risk. METHODS: A prospective, longitudinal, population-based cohort of 84 ELBW survivors and 90 normal birth weight (NBW) control participants born in Ontario, Canada from 1977 to 1982 were assessed by interviewers naive to birth weight status using the Mini-International Neuropsychiatric Interview. Comorbidity between substance use disorders and psychiatric conditions.

AIM: To review information relevant to the question of whether substance-induced mental disorders exist and their implications. DESIGN AND METHOD: This paper utilized a systematic review of manuscripts published in the English language since approximately 1970 dealing with comorbid psychiatric and substance use disorders. FINDINGS: The results of any specific study depended on the definitions of comorbidity, the methods of operationalizing diagnostic criteria, the interview and protocol invoked several additional methodological issues. The results generally support the conclusion that substa

Pharmacogenetics of drug dependence: role of gene variations in susceptibility and treatment.

Drug dependency is a highly prevalent mental health disorder that imposes a significant burden on those directly affected, health care systems, and society in general. There is substantial heritability in the susceptibility to drug addiction, which indicates that there are genetic risk factors. Variation in the human genome is abundant and can directly affect drug dependency phenotypes, for example, by altering the function of a gene product or by altering gene expression. Pharmacogenetic studies can assess the effects of genetic variation on the risk for a particular phenotype (e.g., being an

Weather-Related Trends in Drug Use Incidents Aboard Transit Vehicles.

With changing trends in drug misuse in the United States, including the increased prominence of fentanyl misuse, transit operators have indicated concern that people who use drugs (PWUD) seek transit vehicles as shelter to escape from inclement weather. Research has yet to investigate the relationship between drug misuse on transit vehicles and weather conditions. We hypothesized that there would be a higher frequency of drug use incidents on transit vehicles during periods of reduced daylight hours, low temperatures, high precipitation, or high wind speeds. Using a dataset of drug use inciden

Navigating Consent and Confidentiality in Adolescent Substance Use Treatment.

Consent and confidentiality are central to adolescent substance use care, functioning not only as legal and ethical frameworks but as early preventive interventions during a critical developmental window. This urgency is highlighted by rising rates of substance use disorders and overdose deaths driven by illicit fentanyl and polysubstance exposure. Expanded harm reduction efforts have intensified tensions between adolescent autonomy, parental involvement, legal requirements, and the clinician's duty to protect safety. This article provides a clinically grounded framework for navigating consent

From molecular mechanisms to digital surveillance: the 2010-2025 evolution of benzodiazepine and Z-drug misuse research.

The misuse of benzodiazepines (BZDs) and Z-drugs poses significant global public health challenges. This study maps the scientific evolution and paradigm shifts in this field from 2010 to 2025. A bibliometric analysis of 6,311 publications from the Web of Science Core Collection was performed using VOSviewer and CiteSpace to identify collaboration networks, research hotspots, and emerging frontiers. The USA and Harvard Medical School dominated global contributions. Research hotspots evolved through three distinct phases: (1) 2010-2014 focused on GABAA receptor subtype-specific pharmacology, sp

[Being biased].

Thyroid storm is an acute, life-threatening exacerbation of hyperthyroidism. It is crucial that patients receive immediate intensive medical care. In this case, we were initially misled by the patient's drug history, the paramedics' report and our own experience. It is important not to let yourself be biased by a history of drug abuse. Had the diagnosis in this patient been missed for longer, the outcome could have been fatal.

The repeatable battery for the assessment of neuropsychological status (RBANS) and substance use disorders: a systematic review.

BACKGROUND: Cognitive deficits are prevalent among substance use disorder (SUD) patients and affect treatment retention and outcome. The Repeatable Battery for the Assessment of Neuropsychological Status (RBANS) is a well-researched instrument in diverse patient groups and has the potential to serve as an effective and accurate method for identifying cognitive impairment in SUD patients. This systematic review examines the RBANS' ability to detect cognitive impairment in SUD patients. Limitations of knowledge and the need for further research are discussed. METHODS: We conducted a systematic

Trends in distribution of harm-reduction equipment for persons who use drugs in Norway, 2016-2022. Archival analysis based on nationwide data collections.

Harm-reduction strategies are interventions designed to mitigate the adverse effects of substance use, without requiring abstinence. Evidence supports the effectiveness and efficacy of harm reduction as a broad framework for addressing illicit drug use. To ensure the implementation of these measures, Center for Alcohol and Drug Research (KORFOR) was mandated by the Norwegian Directorate of Health in 2016 to annually assess municipalities' adherence to harm-reduction guidelines. This study aims to present national trends in the distribution of harm-reduction equipment for the prevention of infe

Harm reduction practises for users of psychedelic drugs: a scoping review.

Psychedelic use in naturalistic settings in Australia is increasing. Although the risks and harms of psychedelics from a physical perspective are low, psychedelic drugs carry a unique psychological risk profile which is increased in uncontrolled settings. Harm reduction support services align with the Australian Government's Federal Drug strategy, which includes harm reduction as the third pillar in the overall harm minimisation approach to drug use for the period of 2017-2026. This study examined the harm reduction behaviours which users of psychedelics in naturalistic settings currently use,

Tuberculosis and people who use drugs: why focus on this overlooked population is important and why adapted interventions are necessary.

People who use drugs show a higher incidence and prevalence of tuberculosis than people who do not use drugs in areas where Mycobacterium tuberculosis is endemic. However, this population is largely neglected in national tuberculosis programmes. Strategies for active case finding, screening, and linkage to care designed for the general population are not adapted to the needs of people who use drugs, who are stigmatised and difficult to reach. Moreover, access to care, linkage to care, and treatment adherence are challenging for such a marginalised population. Learning from the HIV field about

Evaluating generative AI responses to real-world drug-related questions.

Generative Artificial Intelligence (AI) systems such as OpenAI's ChatGPT, capable of an unprecedented ability to generate human-like text and converse in real time, hold potential for large-scale deployment in clinical settings such as substance use treatment. Treatment for substance use disorders (SUDs) is particularly high stakes, requiring evidence-based clinical treatment, mental health expertise, and peer support. Thus, promises of AI systems addressing deficient healthcare resources and structural bias are relevant within this domain, especially in an anonymous setting. This study explor

Set and setting predict psychopathology, wellbeing and meaningfulness of psychedelic experiences: a correlational study.

BACKGROUND: In psychedelic therapy, the importance of set and setting is a fundamental but under-researched assumption. The aim of this study is to correlate variables of set (psychedelic use motivation) and setting (psychedelic use location and type of companion) with psychopathology, wellbeing and personality variables. RESEARCH DESIGN AND METHODS: A sample of 1022 participants of the Spanish-speaking population was collected through an online survey. A novel instrument, the Psychedelic Use Scale (PUS), was developed to measure substance use variables of LSD, mescaline, psilocybin, DMT, 5-M
