Severe palmoplantar hyperhidrosis both affects activities of daily living and diminishes quality of life. This study evaluated overall safety and efficacy of endoscopic lumbar sympathectomy (ELS) using a clamping method in a large series of consecutive patients. However, 10% to 15% of patients do not benefit from this intervention. No additional procedure has been developed for this disorder. INTRODUCTION: First bite syndrome (FBS) is the development of pain in the parotid region after the first bite of each meal and can be seen after surgery of the parapharyngeal space. The cause is not clear but has been proposed to involve a loss of sympathetic nerve function to the parotid, causing a denervation supersensitivity of salivary gland myoepithelial cells. The purpose of this study was to review the records of 12 patients with FBS to determine any common features of the operations performed that would support this theory of parotid "sympathectomy" as an etiologic factor of FBS.

Harlequin syndrome is a rare autonomic disorder characterized by unilateral facial flushing and sweating with contralateral anhidrosis induced by exercise, heat, and emotion. It is usually idiopathic but could be the first manifestation of several serious underlying medical conditions. Medical or surgical treatments are not required for idiopathic Harlequin syndrome, but social and psychological factors may indicate sympathectomy or botulinum toxin injection. In this article, we report a case of idiopathic Harlequin syndrome and review the literature.

The influence of ATP and AMP injected into the lateral cerebral ventricle in doses of 50, 100 and 200 mug on behavior in rats was studied. ATP in the doses administered had no effect on behavior of rats. AMP in these doses enhanced motor activity of the rats. Chemical sympathectomy by intracerebral injection of 6-hydroxy-dopamine did not affect the action of either of the compounds on behavior of rats. AMP potentiated the action of intracerbrally administered noradrenaline on behavior of the animals.

Shown in the paper is a possibility of surgical correction of disturbances in the intestinal phase of gastric secretion in treating peptic ulcer and its complications which are encountered in an overwhelming majority of patients. Sympathectomy of relevant vascular trees is regarded as a pathogenetically validated procedure as is correction of duodenal motility combined with selective proximal vagotomy and excision of the ulcer. It is shown that truncal and selective vagotomy can aggravate the existing disorders of the intestinal phase of gastric secretion.

Based on two patients with vascular complications in thoracic outlet-syndrome, the anatomic and pathophysiologic principles prior to surgery are discussed. Causative therapy including rib resection and elimination of the embolic source in the subclavian artery is often supplemented by peripheral revascularization with bypass, lysis and/or sympathectomy. The transaxillary approach seems to be optimal, combining minimally invasive principles with a long exposure of the subclavian artery from segment 3 to the proximal axillary artery.

Treatment of a 55-year-old woman with Raynaud's phenomenon and orthostatic hypotension secondary to lumbar sympathectomy performed for Raynaud's phenomenon is described. Indomethacin increased the blood pressure but caused severe gastrointestinal bleeding. It was possible to treat the symptoms of Raynaud's phenomenon with drugs generally used to treat hypertension without causing an undue decrease in blood pressure and, at the same time, to combat orthostatic hypotension with drugs to increase blood pressure.
