Disease-related syncope. Analysis of a community-based hospital registry.

Syncope is characterized by sudden and transient loss of consciousness that follows a reduction or interruption of cerebral blood flow. The present study was designed to assess the prevalence of disease-related syncope in a wide sample of in-patients admitted for different diseases. A total of 16 809 patients (age range 18-99 years) were recruited from three hospitals in Florence in 1998. The community-based registry was reviewed to identify all patients suffering from complaints associated with syncope. Each disease-related syncope was matched with the number of patients suffering from that d…

Syncope due to autonomic insufficiency syndromes associated with orthostatic intolerance.

Recurrent syncope may be either a sign or a symptom and may occur due to a wide variety of very different causes. Extensive investigations into the nature of this disorder soon uncovered that it represents only one aspect of a broad, heterogenous group of disturbances of the autonomic nervous system (ANS) that can result in hypotension, orthostatic intolerance, and often syncope. Disorders of orthostatic regulation may be subgrouped into both primary and secondary forms. In primary autonomic failure syndromes, as opposed to the intermittent periods of hypotension seen in the reflex syncopes, p…

Sympathetic withdrawal and forearm vasodilation during vasovagal syncope in humans.

Our aim was to determine whether sympathetic withdrawal alone can account for the profound forearm vasodilation that occurs during syncope in humans. We also determined whether either vasodilating beta 2-adrenergic receptor or nitric oxide (NO) contributes to this dilation. Forearm blood flow was measured bilaterally in healthy volunteers (n = 10) by using plethysmography during two bouts of graded lower body negative pressure (LBNP) to syncope. In one forearm, drugs were infused via a brachial artery catheter while the other forearm served as a control. In the control arm, forearm vascular re…

[Recurrent syncope without angina: an uncommon presentation of coronary spasm].

We report the case of a 64-year-old patient admitted for repetitive syncope as an isolated clinical manifestation probably due to coronary artery spasm. The patient had no history of previous cardiac disease, and was studied because of two nonspecific syncopes. Long-term electrocardiographic monitoring showed many episodes of transient ST segment elevation, associated with premature ventricular beats and runs of ventricular tachycardia. Coronary angiography during ergonovine infusion was performed to confirm the diagnosis. We discuss the incidence of coronary spasm provoking syncope and the ne…

[Syncope in Lown IVb ventricular arrhythmias and mitral valve prolapse].

The indication for antiarrhythmic treatment in patients with the mitral valve prolapse syndrome is based mainly on symptomatology. In patients with a history of syncope in the presence of marked mitral valve reflux, recording of repetitive forms by means of Holter ECG and, when persistent ventricular salvos can be provoked by ventricular stimulation, long-term treatment with antiarrhythmic drugs should be considered. Especially in the case of troublesome palpitations resulting from ventricular extrasystoles, the substances of first choice are beta blockers. If these are not adequately effectiv…

Painless abruptio placentae associated with disseminated intravascular coagulation and syncope.

Abruptio placentae is a complication of pregnancy associated with significant morbidity and mortality for both fetus and mother. The presence of uterine pain and tenderness traditionally has been considered a useful criterion for distinguishing abruptio placentae from other causes of bleeding in pregnancy. We report the case of a 25-year-old woman who presented with painless abruptio placentae and coagulopathy during the second trimester. This case illustrates the importance of considering this diagnosis in pregnant patients with vaginal bleeding.

Empiric pacemaker compared with a monitoring strategy in patients with syncope and bifascicular conduction block--rationale and design of the Syncope: Pacing or Recording in ThE Later Years (SPRITELY) study.

AIM: This study seeks to identify the optimal management strategy for patients with syncope in the context of bifascicular block and preserved left ventricular systolic function. METHODS AND RESULTS: This multicentre, randomized, open label, parallel group pragmatic randomized trial will test the hypothesis that a strategy of empiric permanent pacemaker implantation in patients with syncope and bifascicular heart block improves future outcome more effectively than a strategy of therapy guided by prolonged monitoring with an implantable loop recorder (ILR). A total of 120 patients with bifasci…

Cardiac pacing in patients with neurally mediated syncope and documented asystole: effectiveness analysis from the Third International Study on Syncope of Uncertain Etiology (ISSUE-3) Registry.

AIMS: The randomized, double-blind Third International Study on Syncope of Uncertain Etiology (ISSUE-3) showed that dual-chamber permanent pacing was effective in reducing the recurrence of syncope in patients ≥ 40 years with severe asystolic, probably neurally mediated syncope (NMS), documented by implantable loop recorder (ILR). Analysis in ISSUE-3 was performed according to the intention-to-treat principle. In the present study, we performed an on-treatment analysis, which included additionally those non-randomized patients followed up in the ISSUE registry to evaluate in a better manner th…

Determinants of carotid sinus hypersensitivity in patients with suspected reflex syncope.

PURPOSE: To elucidate the pathophysiology of the carotid sinus reflex in a large cohort of patients with suspected reflex syncope. METHOD: We reviewed an electronic database of patients with suspected reflex syncope who underwent carotid sinus massage. The diagnostic protocol included both right and left massage, in supine and upright positions, followed by tilt testing. RESULTS: The cohort included 2249 patients (52% male), median age 73 years. The conventional abnormality threshold of a maximum RR interval ≥ 3000 ms was present in 14.7% of patients and of a systolic blood pressure fall > 5…

Aetiology and haemodynamic patterns of orthostatic hypotension in a tertiary syncope unit.

AIMS: Orthostatic hypotension (OH) is an important differential diagnosis in unexplained syncope. Neurogenic OH (nOH) has been postulated to differ from non-neurogenic OH (non-nOH), yet pathophysiological differences are largely unexplored. We aimed to investigate aetiology and tilt table test (TTT)-induced haemodynamic responses in symptomatic OH patients. METHODS AND RESULTS: We performed a retrospective study analysing patients referred for unexplained syncope or highly symptomatic orthostatic intolerance with TTT-verified classical OH (cOH). Medical records were analysed for the presumpti…

Catheter Ablation for Vasovagal Syncope: The Therapeutic Potential of Gateway Plexi.

Vasovagal syncope (VVS) is the most common cause of syncope, and significantly impacts quality of life despite its benign nature. For some patients, conventional management strategies such as lifestyle changes, pharmacotherapy and pacemaker implantation, fail to prevent recurrence. Cardioneuroablation (CNA), a novel intervention targeting the cardiac autonomic nervous system's ganglionated plexi, has shown promise in addressing refractory VVS. This review examines the therapeutic potential of CNA, exploring the anatomy and physiology of the cardiac autonomic nervous system, the role of ganglio…

The Pathophysiology of Vasovagal Syncope and New Approaches to its Pharmacological Treatment.

Syncope refers to the transient loss of consciousness and the most common type of syncope is vasovagal syncope (VVS), usually occurring when a person is in an upright position, and/or after exposure to intense stress. The sequelae of VVS is caused by an increase in sympathetic tone and heart rate combined with an underfilled left ventricular chamber that leads to stimulation of cardiac afferent C fibers ultimately leading to bradycardia and vasodilation causing a reduction in venous return, cerebral hypoperfusion and VVS. Several treatment options have been tested including physical counter-pr…

The wolf in sheep's clothing: vasovagal syncope in acute aortic dissection.

BACKGROUND: The presentation of acute aortic dissection can pose a challenge for emergency physicians, as it may occur without pain. Atypical presentations can lead to significant delays in diagnosis and increased mortality rates. CASE DESCRIPTION: Our case illustrates that isolated painless syncope can be a rare presenting symptom of acute aortic dissection type A. What is unique about our case is the limited extension of the dissection tear and the availability of Holter monitoring during the syncopal episode. CONCLUSION: This constellation provides insight into the pathophysiological mech…

Polymorphic Ventricular Tachycardia Detected by a Smartwatch in a Patient With Recurrent Syncope.

The diagnosis of vasospastic angina remains challenging. It may present with syncope and life-threatening arrhythmia. Patients with syncope usually require automatic modes of recording. We describe a patient with recurrent syncope and polymorphic ventricular tachycardia documented by a smartwatch. Eventually, the arrhythmia was attributed to vasospasm and effectively managed with pharmacotherapy.

An Investigation of Metabolic Risk Factors and Gut Microbiota in Unexplained Syncope.

BACKGROUND: The pathogenesis of many syncopal episodes remains unexplained. Intestinal dysbiosis could be involved in the pathophysiological mechanisms of syncope due to its connection with the central nervous system via the microbiota-gut-brain axis. This pilot study aimed to explore the specific cardiometabolic risk factors and gut microbiota in unexplained syncope (US), compared to other types of syncope, to assess their similarity or verify their different origins. METHODS: We studied 86 participants with syncope, who were divided into four groups: an orthostatic syncope group (OH, n = 24…
