Macroscopic features of knee synovitis in early untreated Behçet disease and psoriatic arthritis.

In a previous study, we found that synovial immunopathology differs between Behçet disease (BD) and psoriatic arthritis (PsA). The objective of this study is to describe the macroscopic features of early untreated knee synovitis in BD and PsA. Fourteen consecutive patients with active early knee synovitis (seven BD and seven PsA) undergoing rheumatologic arthroscopy were assessed. The following macroscopic synovial features were evaluated and scored by analyzing the video recordings of each procedure: capillary hyperaemia, morphology of synovitis, vascular pattern, fibrinoid membranes, and top…

Persistent synovitis in children with Lyme arthritis: two unusual cases. An immunogenetic approach.

UNLABELLED: We report on two patients with a persistent Lyme arthritis. In addition both had a peculiar disease history. The first patient had oligoarticular juvenile idiopathic arthritis in remission. Five months after an infected tick bite, she developed a relapse of arthritis in the same knee. We considered Lyme borreliosis as the possible trigger for this reactivation. The disease history of the second patient was that of a classical non-responder. After extensive antibiotic treatment osteolytic lesions became visible. MRI images suggested an erosive arthropathy and arthroscopy was used to

Remitting seronegative symmetrical synovitis with pitting edema following intravesical bacillus Calmette-Guérin instillation.

Remitting seronegative symmetrical synovitis with pitting edema (RS3PE) is a rare syndrome of undetermined etiology occurring in the elderly. We describe the first case of RS3PE in a HLA-B27 positive 65-year-old man following intravesical bacillus Calmette-Guérin (BCG) instillation for bladder carcinoma. He developed symmetrical arthritis and synovitis involving wrists, knees, ankles, and metatarsophalangeal joints, with marked pitting edema of the dorsa of both hands and feet, fever, and elevated acute phase reactants. Right knee effusion revealed nonspecific sterile inflammatory fluid. He re…

Primary chronic osteomyelitis associated with synovitis, acne, pustulosis, hyperostosis and osteitis (SAPHO syndrome).

Diffuse sclerosing osteomyelitis may indicate the mandibular localisation of the SAPHO syndrome. Twelve patients with diffuse sclerosis of the mandible were examined for symptoms of the SAPHO syndrome. Nine patients were found to have primary chronic osteomyelitis and eight of these represented a SAPHO syndrome. Results in this series support the hypothesis of an association between primary chronic osteomyelitis and the SAPHO syndrome.

Severe aseptic synovitis of the knee after biodegradable internal fixation. A case report.

Biodegradable rods made of polyglycolide (Dexon, Biofix) or lactide-glycolide copolymer (Vicryl) have been used for the past 5 years for internal fixation of a variety of fractures and osteotomies (Böstman et al. 1989, 1990b). Experience from using such rods for fixation of intraarticular osteochondral lesions seems to be less extensive. We report a case of severe synovial reaction to biodegradable rods used for fixation of osteochondritis dissecans of the knee and discuss possible reasons for the increased risk of foreign-body reactions when these rods are used intraarticularly.

Human T lymphotropic virus I, myelopathy, polymyositis and synovitis: an expanding rheumatic spectrum.

A black woman developed chronic myelopathy, polymyositis and synovitis. Although native of a nonendemic area the possibility of human T lymphotropic virus I association was considered. She tested positive and was serendipitously found to have been positive 16 years before. Risk factors were a transfusion and work as a serology technician. Sjögren's syndrome, alveolitis, gammopathy and immune dysfunction are other rheumatic manifestations of this virus.

The role of antimicrobial peptides in rheumatoid arthritis: From mucosal predisposition to chronic synovitis.

Antimicrobial peptides (AMPs) play many roles in immune regulation, integrating direct antimicrobial activity with potent immunomodulatory functions to coordinate host defence. However, these same signals can become maladaptive and contribute to the development and persistence of chronic inflammatory diseases such as rheumatoid arthritis (RA). In the inflamed joint, AMPs can disrupt tissue architecture, activate resident and infiltrating immune cells through multiple mechanisms, such as neutrophil extracellular trap formation, and may themselves serve as autoantigens, thereby amplifying pathog

When inflammation is not the driver: Pauci-immune synovitis and the stromal basis of refractory arthritis.

Approximately 5% to 20% of patients with rheumatoid arthritis (RA) fail to respond to sequential biologic and targeted synthetic disease-modifying antirheumatic drugs, representing a major unmet clinical need. Increasing evidence indicates that a substantial proportion of these patients exhibit a pauci-immune (fibroid) synovial pathotype, characterised by sparse immune-cell infiltration and enrichment of fibroblast, extracellular matrix, and cell adhesion pathways rather than inflammatory immune signatures. Notably, this pathotype is present in up to one quarter of treatment-naïve patients wit

The relationship of ALPK1, hyaluronic acid and M1 macrophage polarization in the temporomandibular joint synovitis.

M1 macrophage polarization and synovitis play an important role in the pathogenesis of temporomandibular joint osteoarthritis (TMJOA). Reduced molecular weight of hyaluronic acid (HA) in synovial fluid of patients with TMJOA. In addition, high molecular weight hyaluronic acid (HMW-HA) is often used clinically to treat TMJ inflammation. As a pattern recognition receptor of the cytoplasm, ALPK1 was found to be pro-inflammatory in a variety of diseases. However, the relationship of ALPK1, HA and M1 macrophage polarization in TMJ synovitis remains unclear. We aimed to investigate the role of ALPK1…

Localized Pigmented Villonodular Synovitis in the Shoulder: Report of a Case Treated through Arthroscopy.

Pigmented villonodular synovitis (PVNS) is rare in the shoulder, with few descriptions in the literature. We present the case of a 58-year-old female patient with no history of trauma. The patient reported pain for 2 months with no limb irradiation and presented lifting strength loss and progressive limitation of active and passive mobility. She underwent unsupervised physical therapy and there was no improvement in symptoms. A magnetic resonance imaging (MRI) scan of the shoulder showed an oval structure of approximately 2 cm in diameter in the anteroinferior region of the glenohumeral joint …

Magnetic Resonance Imaging (MRI) Findings in a Case of Pigmented Villonodular Synovitis of the Knee.

Pigmented villonodular synovitis (PVNS) is a rare benign condition of tenosynovial proliferation that mostly affects the knee joint. In this case report, we present a 39-year-old female with a ten-year history of gradual progression in the size of painful soft tissue swelling in her left knee. Our case report emphasizes the MRI's ability to provide detailed information on tendon sheath and synovium involvement, as well as extensive extra-articular involvement and hemosiderin deposition.

Clinical outcomes of patients with pigmented villonodular synovitis of the shoulder after arthroscopic synovectomy.

BACKGROUND: Shoulder pigmented villonodular synovitis (PVNS) is a severe clinical condition, while few studies have focused on this situation due to its rarity. This study was to investigate the efficacy of arthroscopic treatment of patients diagnosed with shoulder PVNS. METHODS: From Jan 1st, 2010 to Dec. 31st, 2019, 6 patients (5 females and 1 male) diagnosed with shoulder PVNS underwent arthroscopic synovectomy in our hospital and combined rotator cuff repair was performed in 3 of them. The outcomes of this study include Constant score, Visual Analogue Scale (VAS), University of California…

NLRC4-mediated activation of CD1c+ DC contributes to perpetuation of synovitis in rheumatoid arthritis.

The individual contribution of specific myeloid subsets such as CD1c+ conventional DC (cDC) to perpetuation of rheumatoid arthritis (RA) pathology remains unclear. In addition, the specific innate sensors driving pathogenic activation of CD1c+ cDC in patients with RA and their functional implications have not been characterized. Here, we assessed phenotypical, transcriptional, and functional characteristics of CD1c+ and CD141+ cDC and monocytes from the blood and synovial fluid of patients with RA. Increased levels of CCR2 and the IgG receptor CD64 on circulating CD1c+ cDC was associated with
