Tendon mechanical properties are enhanced via recombinant lysyl oxidase treatment.

Tendon mechanical properties are significantly compromised in adult tendon injuries, tendon-related birth defects, and connective tissue disorders. Unfortunately, there currently is no effective treatment to restore native tendon mechanical properties after postnatal tendon injury or abnormal fetal development. Approaches to promote crosslinking of extracellular matrix components in tendon have been proposed to enhance insufficient mechanical properties of fibrotic tendon after healing. However, these crosslinking agents, which are not naturally present in the body, are associated with toxicit…

[Clinical results after refixation of proximal hamstring tendon ruptures comparing different anchor systems].

BACKGROUND: Injuries to the hamstring muscles and tendon complex are among the most frequent muscular injuries with proximal hamstring tendon avulsion being a particularly severe form. The surgical treatment of these injuries is superior to conservative management with respect to patient satisfaction, recovery of muscle strength and function as well as return to sport rates. It is therefore the method of choice. OBJECTIVE: Presentation and comparison of the surgical results after treatment with titanium, polyether ether ketone (PEEK) and all suture anchors. MATERIAL AND METHODS: A systematic

[Imaging of wrist injuries in athletes].

Wrist imaging is currently used for diagnosis of sport injuries as part of a global strategy of rapid recovery. Standard x-rays are the first step in this procedure. Although arthrography is still the reference for the diagnosis of intrinsic ligament and cartilaginous lesions, MRI can sometimes be sufficient. Ultrasonography is a dynamic process and is accurate in detecting tendon injuries. Wrist sport injuries are frequent and often asymptomatic. Here we review the usual aspects of bone, ligament, and tendon lesions encountered in each sport, while providing advice on the most appropriate ima…

Bilateral Open Achilles Tendon Complete Lacerations Following Interpersonal Violence: A Case Report.

Introduction: Interpersonal violence victims account for a sizable proportion of trauma cases encountered in South African trauma centers. Open Achilles tendon injuries are more prevalent in male patients, accounting for a high number of all cases. Case Report: A 45-year-old male was brought in a trauma casualty by a paramedic after being referred by a local clinic in severe pain on both ankles, unable to walk or stand, and dressed bilaterally with blood stains. On examination, he demonstrated the triad of Achilles tendon injuries, including Matle's sign, Thompson's test, and a gap on palpatio…

Neglected Patellar Tendon Rupture Treated by Trans-Osseous Encirclage Wire and Turn-O-Flap: Case Report.

INTRODUCTION: Although patellar tendon injuries are common but chronic patellar tendon rupture are rare injuries but severely disabling. Problems associated with this injury are scar formation, atrophy and contracture of the quadriceps, poor quality of remaining reabsorbed tendon, inadequate soft tissue, proximal patellar migration and prolonged post operative immobilization. Hence these injuries are technically difficult to repair with invariably poor functional outcome. We report such a case of neglected patellar tendon injury in a leprotic patient successfully treated with trans-osseous enc…

Arthroscopic anterior cruciate ligament reconstruction with quadriceps tendon-patellar bone autograft.

BACKGROUND: Surgical reconstruction of the anterior cruciate ligament (ACL) is indicated in the ACL-deficient knee with symptomatic instability and multiple ligaments injuries. Bone patellar tendon-bone and the hamstring tendon generally have been used. In the present study, we describe an alternative graft, the quadriceps tendon-patellar bone autograft, by using arthroscopic ACL reconstruction. METHODS: From March of 1996 through March of 1997, a quadriceps tendon-patellar bone autograft was used in 12 patients with ACL injuries. RESULTS: After 15 to 24 months of follow-up, the clinical out

Olympic Elbow Comprising Lateral Epicondylitis, Cubital Tunnel Syndrome, and Distal Biceps Tendon Rupture.

There is a wide range of elbow injuries including chronic injuries such as lateral epicondylitis, medial epicondylitis, and cubital tunnel syndrome, or acute injuries such as elbow fracture-dislocations and distal biceps tendon ruptures. Combinations of acute and chronic elbow injuries have been reported including country club elbow and terrible triad of the elbow which are important to identify to properly treat. Thus, we report for the first time a new elbow injury triad termed Olympic elbow in a 65-year-old man comprising lateral epicondylitis, cubital tunnel syndrome, and a distal biceps t

Nerve injury complicating multiligament knee injury: current concepts and treatment algorithm.

Multiligament knee injuries account for <0.02% of all orthopaedic injuries, and 16% to 40% of these patients suffer associated injury to the common peroneal nerve (CPN). The proximity of the CPN to the proximal fibula predisposes the nerve to injury during local trauma and dislocation; the nerve is highly vulnerable to stretch injury during varus stress, particularly in posterolateral corner injuries. CPN injuries have a poor prognosis compared with that of other peripheral nerve injuries. Management is determined based on the severity and location of nerve injury, timing of presentation, asso

Adult native knee extensor mechanism ruptures.

Extensor mechanism rupture is a serious event requiring prompt diagnosis and treatment. Patella fractures are reportedly six times more frequent than soft tissue injuries such as quadriceps or patella tendon ruptures. Classically quadriceps and patella tendon ruptures are seen more in males, with those over 40 predominantly suffering from quadriceps tendon ruptures, often associated with an underlying condition, whereas patella tendon ruptures are mostly associated with sport injuries and are commonly seen in the under 40s. Almost all types of extensor mechanism ruptures benefit from early man

A systematic review of tibialis anterior tendon rupture treatments and outcomes.

Tibialis anterior (TA) tendon rupture is a relatively rare injury that has been documented primarily in case reports. This article is the first large systematic review of the literature on treatment techniques for subcutaneous rupture of TA tendons. Studies for review were identified through a PubMed search. Eligible studies involved cases of closed tendon rupture. Of the 87 cases in the study, 72 were treated with surgery, 15 with conservative measures. Mean age was 63.9 years (surgery group) and 72.4 years (conservative treatment group). Primary repair was used most often for newer injuries,

A Site-Based Analysis of Relationship Between Clinicopathological Factors and Their Influence on Locoregional Recurrence in Oral Squamous Cell Carcinoma.

BACKGROUND: There are only a few studies on the functional outcome of anterior cruciate ligament reconstruction using quadriceps tendon (QT) autograft. The available evidence highlights many advantages of this technique. The graft is thicker and stronger with less donor site morbidity and there is much less kneeling pain as compared to patellar tendon grafts. It is very versatile and allows harvesting as an all-soft tissue or bone-tendon graft, thereby making it suitable for various groups of patients and various surgical techniques. OBJECTIVES: To evaluate the functional outcome associated w

Effect of wrist position on extensor mechanism after disruption separation.

A cadaveric study was performed on extensor tendon separation and excursion after extensor zone I (distal interphalangeal [DIP]) joint and III (proximal interphalangeal [PIP]) level lacerations and treatment. An average 1.5 mm tendon gap with 30 degrees lag following laceration at the DIP joint level and a 1.1 mm average tendon gap with 18 degrees (10 degrees-25 degrees) extension loss at the PIP level was demonstrated. Splinting zone I injuries with the DIP joints in 5 degrees hyperextension and zone III injuries with the PIP joint in 0 degrees extension eliminated tendon separation in all pa

[Sport injuries of the extensor mechanism of the knee].

Injuries of the extensor mechanism of the knee occur frequently during sport activity. For a successful treatment they must be diagnosed early. Besides osseous structures the patellar tendon, the patella, the quadriceps muscle and tendon, retinacula and bursae can be affected. After initial clinical examination there are different noninvasive imaging modalities available for assessment of bone, cartilage, ligaments, tendons and soft tissue. Conventional radiographs are still the basic imaging tool for the clinician. Additional information about the osseous status is provided by computed tomogr

The effects of overuse combined with intrinsic or extrinsic alterations in an animal model of rotator cuff tendinosis.

An in vivo animal model was used to evaluate overuse and overuse plus intrinsic tendon injury or extrinsic tendon compression in the development of rotator cuff injury. Forty-four male Sprague-Dawley rats were divided into groups of 22. Each left shoulder received an intrinsic or extrinsic injury plus overuse (treadmill running), and each right shoulder received only overuse. Eleven rats from each group were sacrificed at 4 and 8 weeks. Supraspinatus tendons were evaluated histologically or geometrically and biomechanically. Ten rats constituted a cage-activity control group. Both supraspinatu
