[Tenosynovitis in chronic inflammatory joint diseases and its sequelae in the area of the hand].

The synovial lining of tendons may participate early in chronic rheumatic diseases. As a result of the inflammatory activity degenerative changes of tendons may occur. Increasing damage to tendons ends in their rupture. At 775 tenosynovectomies, 395 of them in chronic rheumatic diseases, in 68 patients (78 hands) 173 ruptures of tendons (14 flexors, 159 extensors) were found. The value of early tenosynovectomy as a preventive measure is emphasized.

Proximal and distal reconstruction of the extensor mechanism for patellar subluxation.

Proximal and distal reconstruction of the leg extensor mechanisms has been developed and performed in 346 cases over a period of 25 years. Proximal extensor mechanism reconstruction produces very satisfactory results in over 70% of cases and continues to be the basic principle of surgical treatment for patellar subluxation. In patients with Q angles (angle between line of the quadriceps force and the direction of the patellar tendon) greater than 10 degrees distal reconstruction (patellar tendon transfer) is done in addition to, not in lieu of, the dynamic proximal reconstruction. Treatment of…

Twenty-five-year follow-up evaluation of an active silicone/Dacron tendon interposition prosthesis: A case report.

A 19-year-old man sustained a severe avulsion wound of the dominant distal forearm, dividing the radial and ulnar arteries, median and ulnar nerves, and all flexor tendons. Initial treatment consisted of revascularization. Shortly thereafter he had sural nerve grafting of the median and ulnar nerves. This was followed by insertion of a silicone/Dacron tendon interposition prosthesis to reconstruct a 4-cm deficit in the flexor profundus tendons and the flexor pollicis longus tendon. Six weeks thereafter an opposition transfer using the extensor indicis proprius and a Brand type 2 intrinsic tran

[Minimally invasive treatment of congenital foot deformities in infants: new findings and midterm-results].

In the last decade treatment of foot deformities has changed from extensive surgery to casting and minimally invasive surgery. The Ponseti method has become the most preferred treatment for clubfoot deformities and early evaluations showed promising results. Mid-term results for idiopathic clubfoot revealed the need for additional surgery by anterior tibial tendon transfer in 11-32% of cases depending on the duration of bracing. Anterior tibial tendon transfer is the most important surgical procedure for relapses in the Ponseti concept. Casting, recasting in cases of relapses, bracing and ante…

Greater tuberosity osteotomy and teres minor transfer for irreparable superior rotator cuff tears.

The purpose of this study was to evaluate the mid- to long- term objective, subjective and radiographic results of patients who underwent anterior-superior transfer of remaining infraspinatus tendon and teres minor tendon for irreparable superior rotator cuff tears. Thirteen patients were identified who underwent infraspinatus tendon transfer to a more superior position on the humeral head between January 1, 1990 and December 31, 2001. Nine shoulders in eight patients were available for clinical examination, radiographs and questionnaire follow-up at an average of 83.5 +/- 31.4 months. Radiogr…

Transfer of extensor carpi radialis longus or brevis for opponensplasty.

For the restoration of thumb opposition many types of tendon transfer techniques have been described. The flexor digitorum superficialis (FDS) of the ring finger is commonly selected as a motor. On occasion, however, the quality of the flexor muscles of the fingers or wrist is not good enough for tendon transfer and another available muscle must be selected. In this situation, we have preferred to use an extensor carpi radialis longus (ECRL) or brevis (ECRB) transfer to restore opposition of the thumb. Follow-up examination, at an average 5 years and 10 months after operation, showed that the …

Combined rupture of the tibialis anterior and the extensor hallucis longus tendons--functional reconstruction.

BACKGROUND: Traumatic rupture of the tibialis anterior (TA) tendon represents a very rare foot injury. A combined injury of both the TA and the extensor hallucis longus (EHL) tendons has not yet been reported. Within the scope of this work we will prove that tendon transfers in cases of combined tendon injuries are a reasonable course of action in order to achieve the aim of a functional reconstruction. METHODS: A combined rupture of the tibialis anterior (TA) and the extensor hallucis longus (EHL) tendons was treated by suturing the EHL tendon to the distal TA tendon stump. The TA insertion …

Presurgical Perspectives and Post-treatment Evaluation of Achilles Tendon Injuries.

The Achilles is a commonly injured tendon, prone to degeneration and tear. Achilles tendon treatment ranges from conservative management to injections, tenotomy, open versus percutaneous tendon repair, graft reconstruction, and flexor hallucis longus transfer. Interpreting postoperative imaging of the Achilles tendon is a difficult task for many providers. This article clarifies these issues by presenting the imaging findings following standard treatments and depicts expected appearance versus recurrent tear and other complications.

A review of salvage procedures after failed Achilles tendon repair.

The most common complications that are associated with delayed repair of the Achilles tendon are rerupture, wound necrosis, infection, and inability to regain dorsiflexion. Wound complications can be avoided by using meticulous soft-tissue handling techniques, full-thickness flaps when necessary, and minimal pressure to the wound postoperatively. Ankle stiffness may be reduced by ensuring that the repair does not place undue tension on the tendon which requires the ankle to be placed in excessive plantarflexion. Good restoration of function can be obtained by surgical intervention. Regardless

Spontaneous rupture of the extensor pollicis longus tendon in a professional skier.

Spontaneous rupture of the extensor pollicis longus (EPL) tendon is uncommon in sports activities. We report a rare case of a professional downhill skier presenting with a rupture of the EPL tendon. Repetitive motion of the wrist joint appeared to cause the rupture. The patient was treated successfully with tendon transfer of the extensor indicis proprius.

Delayed rupture of the extensor pollicis longus tendon: a retrospective study.

Eight patients with delayed rupture of the extensor pollicis longus tendon are presented. Six had suffered from a fracture of the wrist without dislocation. This is in our opinion a predisposing factor in the etiology of delayed tendon rupture. Our treatment of choice is transfer of the extensor indicis proprius tendon. In all patients satisfactory thumb function was restored.

Flap coverage of dorsum of hand associated with extensor tendons injuries: A completely vascularized single-stage reconstruction.

This study reports results in 12 patients treated with "completely vascularized single-stage approaches," so defined because skin, tendon, and nerve are transferred as a compound flap, and all are vascularized. A free dorsalis pedis cutaneotendinous flap was used in 7 patients, while a radial forearm cutaneotendinous island flap was transposed in 5 patients. A dorsalis pedis flap provides four vascularized extensor tendons (extensor digitorum comunis tendons), and the radial artery flap permits the inclusion of one completely vascularized tendon (palmaris longus) and two "strips" of vasculariz

Macrolobium paulobocae (Leguminosae, Detarioideae), a new species from seasonally inundated black-water floodplain forests in the Brazilian Amazon.

BACKGROUND: A novel opposition tendon transfer, previously described by Snow and Fink in adults but not in children, can provide better functional results and cosmetic appearance than the currently preferred Huber transfer. METHODS: Thirty-one children were born either without a thumb (17) or with a hypoplastic thumb (14). Thirty-eight thumbs were classified according to the Blauth classification as stages II, IIIA, IIIB, IV, and V respectively. The opposition tendon transfer was performed between the ages of 4 and 10, using the expendable flexor digitorum superficialis (FDS) tendon from the …

A tendon graft weave using an acellular dermal matrix for repair of the Achilles tendon and other foot and ankle tendons.

Many techniques are available to repair tendon ruptures of the Achilles tendon as well as other foot and ankle tendons. The purpose of this article was to present a technique of tendon repair that is a combination of a tendon graft inlay with a Pulvertaft-type tendon weave using an acellular human dermal matrix graft with a resultant "strip and shoelace." A description of the technique, along with 3 case reports, is included. Tendon donor site morbidity problems associated with tendon transfer procedures are avoided by use of this dermal matrix weave. This procedure is felt to have enhanced st…
