From Complete Heart Block to Torsades de Pointes: An Axis to Remember.

BACKGROUND: Complete atrioventricular block is a recognized cause of acquired QT prolongation and Torsades de Pointes. CASE SUMMARY: An 80-year-old woman with no known prior cardiac history presented to the emergency department with acute confusion and altered mental status. There was no reported history of syncope, palpitations or structural heart disease. Baseline electrocardiography demonstrated complete atrioventricular block with a stable junctional escape rhythm and marked corrected QT prolongation (QTc 545 ms). Continuous electrocardiography monitoring revealed sudden-onset polymorphic

Cooling-Triggered Release of Celecoxib from Implantable Alginate-Soluplus Composite Devices.

Currently, on-demand treatment of pain (both chronic and acute) is primarily achieved using opioids that are delivered systemically. Unfortunately, these drugs are highly addictive; over 5 people per hour die from opioid abuse in the US alone. A safer, nonsystemic mechanism for pain relief is therefore needed. Nonsteroidal anti-inflammatory drugs (NSAIDs) have been explored for this purpose; they are nonaddictive, provide excellent pain relief, and can be delivered locally to minimize dosage and systemic side effects. However, an on-demand release method is needed to make local delivery of the

Divergent Paths: A Comprehensive Review of Mechanisms and Clinical Implications in Atopic Dermatitis and Psoriasis.

Atopic dermatitis and psoriasis are two common cutaneous chronic inflammatory diseases. Although they share similar immunological mechanisms, their clinical implications in the patient are different. This review compares both diseases from different points of view such as genetics, natural history, triggers of disease, and skin barrier defects among other disease aspects. The lessons learned by translational research using targeted therapies have also contributed to clarifying differences.

[Perioperative acquired long QT syndrome: a case report].

Acquired long QT syndrome is a rare condition whose diagnosis is of vital importance given the risk of torsade de pointes and sudden death. This syndrome may be triggered by various events in patients with a genetic predisposition. Patients usually have a normal baseline QT interval. Some of the factors that may prolong the interval are exposure to common drugs such as antibiotics or agents used for general anesthesia. Diagnosis of the condition is essential, as is knowledge of how to manage anesthesia and prevent ventricular fibrillation, which is the most feared complication. We report the c

Uric acid as a danger signal in gout and its comorbidities.

Uric acid is a waste product of purine catabolism. This molecule comes to clinical attention when it nucleates to form crystals of monosodium urate (MSU) in joints or other tissues, and thereby causes the inflammatory disease of gout. Patients with gout frequently suffer from a number of comorbid conditions including hypertension, diabetes mellitus and cardiovascular disease. Why MSU crystals trigger inflammation and are associated with comorbidities of gout has been unclear, but recent studies provide new insights into these issues. Rather than simply being a waste product, uric acid could se

Expert statement for the management of hypovolemia in sepsis.

Hypovolemia is frequent in patients with sepsis and may contribute to worse outcome. The management of these patients is impeded by the low quality of the evidence for many of the specific components of the care. In this paper, we discuss recent advances and controversies in this field and give expert statements for the management of hypovolemia in patients with sepsis including triggers and targets for fluid therapy and volumes and types of fluid to be given. Finally, we point to unanswered questions and suggest a roadmap for future research.

Nonpharmacological approaches for the treatment of urological chronic pelvic pain syndromes in men.

Chronic nonbacterial prostatitis, or urological chronic pelvic pain syndrome (UCPPS), remains a common and often challenging disorder to evaluate and treat. Employing a more holistic approach, including urological therapy, physical therapy, and psychosocial perspectives, may be more appropriate for most patients. Growing evidence supports the use of biofeedback, myofascial trigger point release, prescribed exercise regimens, relaxation techniques, and supportive counseling to treat men with UCPPS.

Development and evaluation of a German Paediatric Trigger Tool (PAED-TT-GER) for detecting adverse events in hospitalized children.

UNLABELLED: Trigger tools (TTs) are highly recommended for structured detection of adverse events (AEs) through retrospective medical record review. However, available general tools require thorough adaptation to paediatric care and local documentation practices to ensure feasibility and reliable AE identification. The aim of this study was to develop a German Paediatric Trigger Tool (PAED-TT-GER) and evaluate its feasibility, applicability, and interrater reliability in a paediatric cardiology inpatient setting, and to describe the characteristics of AEs detected. We conducted a single-centre

Respiration triggered trans-spinal magnetic stimulation in healthy subjects.

Respiratory muscle dysfunction is usually observed in several neurological and pulmonary disorders. Consequently, it is essential to develop a clinically applicable strategy aimed at enhancing diaphragm excitability. The main objective of this study is to establish a respiration triggered trans-spinal magnetic stimulation protocol, and compare diaphragmatic motor evoked potentials during expiration-inspiration vs. inspiration-expiration transition. Bilateral diaphragm electromyograms were monitored in response to trans-spinal magnetic stimulation triggered by respiratory signals detected by th

Nrf2 deficiency aggravates Angiotensin II-induced cardiac injury by increasing hypertrophy and enhancing IL-6/STAT3-dependent inflammation.

BACKGROUND: NF-E2-related factor 2 (Nrf2) is a transcription factor playing cytoprotective effects in various pathological processes including oxidative stress and cardiac hypertrophy. Despite being a potential therapeutic target to treat several cardiomyopathies, the signaling underlying Nrf2-dependent cardioprotective action remains largely uncharacterized. AIM: This study aimed to explore the signaling mediating the role of Nrf2 in the development of hypertensive cardiac pathogenesis by analyzing the response to Angiotensin II (Ang II) in the presence or absence of Nrf2 expression, both in

[Episodes of apparent transient loss of consciousness: disorders a cardiologist should not ignore].

Affections such as psychogenic (functional) pseudosyncope and cataplexy are characterized by transient attacks without impairment of consciousness, but with loss of postural control and unresponsiveness. Therefore, these disorders should be differentiated from syncope and should not be ignored by the cardiologist, who is usually a reference point for patients with syncope or suspected syncope. Clinical findings that suggest psychogenic pseudosyncope include frequent attacks always in the presence of audience, a fall to the ground that may develop slowly enough to allow the patient to stagger a

Allergic manifestations of skin diseases--atopic dermatitis.

Atopic dermatitis (AD) is a chronic inflammatory skin disease which often becomes manifest in early infancy and is characterized by itchy eczematous lesions with characteristic localization. The cellular infiltrate of allergic eczematous skin diseases (i.e. AD, allergic contact dermatitis) is mainly composed of mononuclear cells. Whereas allergic contact dermatitis is always triggered by allergen-specific T cells, a number of allergic and nonallergic trigger factors appear to be relevant in AD. This article discusses data coming from immunological studies focusing on T-cell responses in AD. Th

Tactile Mechanisms and Afferents Underlying the Rat Pup Transport Response.

Juvenile rodents and other altricial mammals react with calming, immobility and folding up of feet to parental pickup, a set of behaviors referred to as transport response. Here we investigate sensory mechanisms underlying the rat transport response. Grasping rat pups in anterior neck positions evokes strong immobility and folding up of feet, whereas more posterior grasping positions have lesser effects on immobility and foot position. Transport responses are enhanced by slow (1Hz) and even more so by fast (4Hz) gentle shaking and translation of the pup, features consistent with parental trans

Impact of stent design on in-stent stenosis in a rabbit iliac artery model.

The purpose of this study was to evaluate the impact of stent design on in-stent stenosis in rabbit iliac arteries. Four different types of stent were implanted in rabbit iliac arteries, being different in stent design (crown or wave) and strut thickness (50 or 100 microm). Ten stents of each type were implanted. Each animal received one crown and one wave stent with the same strut thickness. Follow-up was either 12 weeks (n = 10 rabbits) or 24 weeks (n = 10 rabbits). Primary study end points were angiographic and microscopic in-stent stenosis. Secondary study end points were vessel injury, va

Enhancing pancreatic imaging in CT - prospective comparison of fixed versus individualized post-trigger delay in bolus tracking.

BACKGROUND: To evaluate if pancreatic computed tomography (CT) image quality benefits from using individualized vs. fixed post-trigger delay for contrast media timing. METHODS: In this prospective study CT of the pancreas was performed on one cohort using individualized post-trigger delay and one cohort using fixed post-trigger delay. Except for the post-trigger delays the CT-protocol, contrast media application and image reconstruction were identical in both cohorts. Objective (mean attenuation in HU) and subjective image quality (5-point Likert-scale) were evaluated in the pancreatic head a

Drug-induced Torsades de pointes: Insights from a canine model of chronic atrioventricular block.

Torsades de pointes is a potentially fatal ventricular arrhythmia that can arise when a patient receives medicines that adversely antagonize the potassium current governed by the Kv11.1 ion channel. Several drugs across numerous therapeutic classes, including antiarrhythmic drugs, can trigger this arrhythmia in susceptible individuals. Consequently, interest in the preclinical screening of medications, identification of at-risk patients, and the development of reliable arrhythmia biomarkers has significantly increased. Animal models of torsades de pointes are rare; however, the canine model of

Fibromyalgia: Pathogenesis, Mechanisms, Diagnosis and Treatment Options Update.

Fibromyalgia is a syndrome characterized by chronic and widespread musculoskeletal pain, often accompanied by other symptoms, such as fatigue, intestinal disorders and alterations in sleep and mood. It is estimated that two to eight percent of the world population is affected by fibromyalgia. From a medical point of view, this pathology still presents inexplicable aspects. It is known that fibromyalgia is caused by a central sensitization phenomenon characterized by the dysfunction of neuro-circuits, which involves the perception, transmission and processing of afferent nociceptive stimuli, wi

Spontaneous Pneumomediastinum in Rheumatoid Arthritis Associated Interstitial Lung Disease and Bronchiectasis: A Three-Component Pathophysiologic Framework.

UNLABELLED: Spontaneous pneumomediastinum is a recognized complication of connective tissue disease associated interstitial lung disease, most extensively described in dermatomyositis. Despite rheumatoid arthritis being the most common connective tissue disease with pulmonary involvement, spontaneous pneumomediastinum in rheumatoid arthritis associated lung disease has been reported only once previously. Rheumatoid arthritis uniquely causes two distinct forms of structural lung injury (interstitial lung disease and bronchiectasis) both linked to seropositive autoimmunity. An 83-year-old woman,
