Salivary Duct Carcinoma With Rhabdoid and Signet-Ring Cell Features Mimicking Invasive Lobular Carcinoma of the Breast: A Case Report and Review of the Literature.

Salivary duct carcinoma is a rare, aggressive salivary gland malignancy that morphologically and immunophenotypically resembles breast ductal carcinoma. The rhabdoid variant is characterized by discohesive tumor cells with rhabdoid cytomorphology, frequent loss of E-cadherin expression, and morphologic overlap with pleomorphic invasive lobular carcinoma of the breast. We report a 74-year-old man who presented with an enlarging right buccal mass and trismus. Positron emission tomography-computed tomography demonstrated right cervical lymphadenopathy with no evidence of breast or lung primary. R…

StyleTubation Applied for Difficult Airway With Post-Radiation Fibrosis and Halo-Vest Stabilization.

Patients with nasopharyngeal cancer and following head/neck radiotherapy often experience a difficult airway during subsequent surgical treatment. Pre-operative airway examination usually reveals certain degrees of radiation-induced fibrosis over the neck, trismus, a higher Mallampati class, and an upper lip bite test grade. Available pre-operative transnasal videoendoscopic and radiological imaging findings might show vulnerable mucous membrane irregularities and anatomical abnormalities over the nasopharynx and larynx, and edematous or distorted epiglottis and vocal cords. Along with aforeme…

A hidden diagnosis in the tonsil: recognising and differentiating intratonsillar abscess.

OBJECTIVE: Peritonsillar abscess (PTA) and intratonsillar abscess (ITA) are distinct entities, but the latter is often overlooked due to its … y elevated in ITA, suggesting a lymphocyte-dominant response. Fever was more frequent in ITA, while trismus and local spread were more characteristic of PTA. CONCLUSIONS: ITA typically presents with smaller abscesses and limited systemic involvement, often responding to conservative treatment. Recognising ITA as a distinct clinical entity may improve dia …

Multidisciplinary management of severe odontogenic maxillofacial multi-space infection in a patient with HIV-syphilis coinfection and advanced immunosuppression: a case report.

BACKGROUND: HIV-syphilis coinfection poses a substantial challenge in the management of severe odontogenic infections, with few reports addressing maxillofacial multi-space infections in this setting. CASE PRESENTATION: A 50-year-old man developed severe left maxillofacial multi-space infection one week after molar extraction, presenting with respiratory distress, trismus, elevated C-reactive protein (235.20 mg/L), computed tomography evidence of gas-forming abscesses, reactive HIV antibody, positive syphilis serology, and a CD4+ T-cell count of 133.76 cells/μL. Emergency tracheostomy and inc…

Oncocytic carcinoma of the parotid gland: A rare malignancy with diagnostic and therapeutic challenges.

Oncocytic carcinoma of the parotid gland is an exceptionally rare malignancy, accounting for approximately 0.18% of all salivary gland cancers. Its diagnosis is often delayed due to overlapping clinical and radiologic features with benign oncocytic tumors such as oncocytoma and Warthin tumor. We report the case of a 64-year-old man who presented with a progressively enlarging right parotid mass over 4 months, accompanied by new-onset facial nerve palsy and trismus. MRI revealed a multilobulated, ill-defined, heterogeneously enhancing mass in the superficial lobe of the right parotid gland, wit…

Suspected Emphysematous Cellulitis of the Face Secondary to Untreated Nondisplaced Angle of the Mandible Fracture: A Case Report.

Facial cellulitis with palpable crepitus is a rare complication following a nondisplaced angle of mandible fracture. We report a case of a male in his mid-20s who presented with suspected emphysematous cellulitis of the face 3 days after an assault. Clinical examination revealed diffuse facial swelling with palpable subcutaneous crepitus over the left cheek and submandibular region. The patient had trismus and suppuration of the lower left third molar but no intraoral soft tissue injury. An orthopantomogram (OPG) showed a left nondisplaced angle of mandible fracture and multilocular bubble-lik…

Two Cases of Temporomandibular Synovial Chondromatosis Associated with Gli1 Gene Mutation.

Synovial chondromatosis (SC) is a rare benign disease involving multifocal generation of ectopic cartilage in the synovial tissue. Herein, we report two cases of SC in the temporomandibular joint: a 38-year-old woman (patient 1) and 39-year-old woman (patient 2). Both patients had trismus, jaw joint noises, and jaw-opening pain in the temporomandibular joint. Cone-beam computed tomography (CT) and magnetic resonance imaging (MRI) in patient 1 showed multiple calcified loose bodies around the right mandibular condyle. In addition, CT and MRI in patient 2 showed multiple calcified loose bodies a…

Effectiveness of irrigation with chlorhexidine after removal of mandibular third molars: a randomised controlled trial.

To evaluate the effect of postoperative irrigation with chlorhexidine on inflammatory complications after the extraction of lower third molars under local anaesthesia, we recruited 100 patients to participate in a controlled, single-blind, randomised clinical trial. They were assigned to one of two groups: the intervention group (postoperative irrigation of the surgical site with chlorhexidine for seven days) or the control group (postoperative chlorhexidine mouth rinse for seven days). The primary outcome variables were pain, swelling, trismus, infection, and alveolar osteitis. The secondary …

An immunohistochemical study on a tetanus fatal case using toxin fragment C (TTC). Should it be a useful diagnostic tool?

A 65-year-old man fell in his garden and sustained a right pre-radial cutaneous laceration associated with a Colles' fracture. His status for tetanus immunization was uncertain; so a course of antitetanus treatment was immediately started. Two days after admission the man suddenly developed severe nucal pain, rigidity and dysphagia. A brain CT scan was negative. His condition progressively worsened and then he developed trismus. Cultures from the wound were negative for Clostridium tetani; the CSF analysis was negative. On the 9th day after admission, the man died. A presumptive clinical diagn…

Physiotherapeutic Approach in Oral Submucous Fibrosis: A Systematic Review.

One of the most poorly recognized and inadequately managed diseases, oral submucous fibrosis progresses over time. Betel nut eating is the foremost cause of oral submucous fibrosis. One such condition is oral submucous fibrosis, which is characterized by severe trismus, disability, and a higher risk of cancer. The mouth opening gradually decreases, which is related to difficulty eating, altered gustatory sensation, and mouth dryness, leading to decreased oral intake. The main and beginning signs include decreased mouth opening, pain, difficulty eating, dry mouth, and blanching of the buccal mu…

Modified Gap Arthroplasty for Temporomandibular Joint Ankylosis Following Radiotherapy for Rhabdomyosarcoma: Report of an Unusual Case and Brief Literature Review.

Radiotherapy at the temporomandibular joint (TMJ) area often results in trismus, however, post radiation ankylosis is extremely rare and has not been previously reported in literature. Radiation is known to impact the vasculature of bony structures leading to bone necrosis with certain risk factors including surgical intervention, even teeth extraction, that could lead to osteoradionecrosis. Accordingly, gap arthroplasty for such case seemed rather challenging. In this report, we introduce for the first time, a rare case of temporomandibular joint ankylosis post radiotherapy for management of …

Mandibular metastasis as the first clinical indication of occult lung adenocarcinoma with multiple metastases: A case report.

RATIONALE: Although metastases to the oral and maxillofacial region (OMR) are rare, the lung is the most common primary site metastasizing to the OMR. PATIENT CONCERNS: An 83-year-old woman presented with reports of trismus, occlusal discomfort, swelling, and spontaneous pain in the right buccal region. Despite the absence of abnormal chest imaging findings, immunohistochemical analysis of biopsy specimens of the mandible and the thyroid indicated that the patient had multiple metastases from a lung poorly differentiated adenocarcinoma. DIAGNOSES: Metastases to the OMR and the thyroid from a…

Use of mid upper arm circumference for evaluation of nutritional status of OSMF patients.

BACKGROUND: Oral submucous fibrosis (OSMF) is always associated with juxtaepithelial inflammatory reaction followed by fibroblastic changes in lamina propria, with epithelial atrophy leading to stiffness of oral mucosa and causing trismus and inability to eat. Mid upper arm circumference (MUAC) is a useful tool for a fast assessment of the nutritional status. AIMS: The study was undertaken to evaluate the correlation of MUAC as a nutritional status indicator in OSMF patients. PATIENTS AND METHODS: The study group comprised 50 clinically diagnosed and histopathologically confirmed cases of OS…
