3: Community-acquired pneumonia.

Community-acquired pneumonia is caused by a range of organisms, most commonly Streptococcus pneumoniae, Mycoplasma pneumoniae, Chlamydia pneumoniae and respiratory viruses. Chest x-ray is required for diagnosis. A risk score based on patient age, coexisting illness, physical signs and results of investigations can aid management decisions. Patients at low risk can usually be managed with oral antibiotics at home, while those at higher risk should be further assessed, and may need admission to hospital and intravenous therapy. For S. pneumoniae infection, amoxycillin is the recommended oral dru

Inhibition by yeast killer toxin-like antibodies of oral Streptococci adhesion to tooth surfaces in an ex vivo model.

BACKGROUND: Monoclonal (KTmAb) and recombinant (KTscFv) anti-idiotypic antibodies, representing the internal image of a yeast killer toxin, proved to be microbicidal in vitro against important eukaryotic and prokaryotic pathogens such as Candida albicans, Pneumocystis carinii, Mycobacterium tuberculosis, Staphylococcus aureus, S. haemolyticus, Enterococcus faecalis, E. faecium, and Streptococcus pneumoniae, including multidrug-resistant strains. KTmAb and KTscFv exerted a strong therapeutic effect in well-established animal models of candidiasis and pneumocystosis. Streptococcus mutans is the

Pulmonary paragonimiasis: clinical features, diagnosis and treatment of 39 cases in Manipur.

Thirty-nine cases of pulmonary paragonimiasis due to Paragonimus westermani were identified in Manipur, India. Recurrent haemoptysis, often increased on exertion, was the most common symptom. The clinical features and radiological appearances simulated pulmonary tuberculosis and 23 cases were treated for tuberculosis for varying periods before the final diagnosis. Young males aged 11 to 30 years comprised two-thirds of the cases. The disease was acquired by eating raw or improperly cooked crabs (Potamon deham). Intradermal tests (IDT) employing a saline extract of adult Paragonimus were a simp

CROI 2022: tuberculosis and infectious complications in persons With HIV.

Early treatment of anal high-grade squamous intraepithelial lesions compared with active monitoring reduced the risk of anal cancer by 57% in persons with HIV in a landmark randomized trial of 4446 participants. In a multi- country randomized trial, an entirely oral combination regimen consisting of bedaquiline, pretomanid, linezolid, and moxifloxacin for 24 weeks outperformed the World Health Organization-recommended 36- to 96-week standard of care regimen for multidrug-resistant tuberculosis (TB), ushering in a new era of shorter multidrug-resistant TB treatment. These and other studies of T

Herpetic ulcer of the eyelids in an immunocompromised patient.

A 24-year-old black female (HIV-positive) was referred to our clinic with a 4-week history of an ulcerative lesion of the right upper and lower eyelids. She was on treatment for pulmonary tuberculosis and had been admitted to a secondary level hospital. She had no other ocular symptoms or signs. A tissue biopsy of the lesion revealed multinucleate squamous cells with ground glass viral nuclear inclusion bodies, indicative of herpes simplex virus (HSV) infection. The ulcer healed with oral and topical acyclovir therapy, confirming a herpetic origin. There is only one other reported case of this

Improving Tuberculosis Treatment Outcomes in Somalia: A Narrative Review of Strategies, Challenges, and Policy Recommendations.

Tuberculosis (TB) remains a major public health concern in Somalia, driven by prolonged conflict, population displacement, fragile health systems, and adverse social determinants of health. Although Somalia has made progress in expanding TB services and achieving favourable treatment outcomes for drug-susceptible TB, the country continues to experience a high TB burden and emerging drug-resistant tuberculosis (DR-TB). Persistent challenges include delayed diagnosis, treatment interruption, catastrophic patient costs, paediatric TB care gaps, and reliance on external donor funding. This narrati

Self-emulsifying Delivery System for Improving Biopharmaceutical Performance of a Novel Antitubercular Agent: Formulation Development, Biological Activity and Pharmacokinetic Evaluation.

A nitroimidazole-derived lead (IIIM-019) was developed as a part of our institutional drug discovery program in the area of tuberculosis. This molecule suffers from poor aqueous solubility and to overcome this limitation, lipid-based self-emulsifying drug delivery system (SEDDS) was developed. The SEDDS was characterized for emulsification efficiency, droplet size, stability and drug release. The morphology of SEDDS was investigated by transmission electron microscopy (TEM). The cellular uptake of formulation was assessed in Caco-2 cells and anti-tubercular activity was evaluated in M. tubercu

In vitro investigation of Datura innoxia phytocompounds against Mycobacterium tuberculosis H37Ra strain in association with in silico studies.

Tuberculosis (TB) is a recurrent and progressive bacterial disease caused by Mycobacterium tuberculosis (Mtb), posing a significant challenge globally due to its drug resistance. This study focuses on identifying natural phytocompounds from the plant Datura innoxia (leaves), which is well known for its biologically active metabolites. Initially, the current study employed in vitro analysis of 20 phytocompounds, revealing that the natural compound 9, o-vanillin, exhibited the best minimal inhibitory concentration (MIC) which was 12.5 µg/mL, and minimal bactericidal concentration (MBC) was 50 µg

Dual Burden of MDR-TB and COVID-19 in a Previously Treated Tuberculosis Case: Diagnostic and Therapeutic Dilemmas.

The presence of both MDR-TB and COVID-19 complicates diagnosis and treatment, as their symptoms can overlap, resulting in possible delays in receiving the appropriate care. This study aimed to investigate whether COVID-19 plays a role in the initiation or progression of latent or current tuberculosis (TB) infection, especially MDR-TB, through immunosuppression or lung injury. On May 19, 2022, a retired black African 40-year-old woman was admitted to the emergency room. She had a history of persistent cough, fever, muscle weakness, and weight loss. SuFEx-based antitubercular compound irreversibly inhibits Pks13.

Mycobacterium tuberculosis (Mtb) remains the world's deadliest bacterial pathogen1. There is an urgent medical need to develop new drugs that shorten the treatment duration to combat widespread multi-drug-resistant and extensive-drug-resistant Mtb. Here, we present a preclinical covalent compound, CMX410, that contains an aryl fluorosulfate (SuFEx)2 warhead and uniquely targets the acyltransferase domain of Pks13, an essential enzyme in cell-wall biosynthesis. CMX410 is equipotent against drug-sensitive and drug-resistant strains of Mtb and efficacious in multiple mouse models of infection. In

Macrophage-targeted versus free calcitriol as host-directed adjunct therapy against Mycobacterium tuberculosis infection in mice is bacteriostatic and mitigates tissue pathology.

Host-directed therapy (HDT) with vitamin D in tuberculosis (TB) is beneficial only if the subject is deficient in vitamin D. We investigated pulmonary delivery of 1,25-dihydroxy vitamin D3 (calcitriol) in mice infected with Mycobacterium tuberculosis (Mtb). We made two kinds of dry powder inhalations (DPI)- soluble particles or poly(lactide) (PLA) particles. We compared treatment outcomes when infected mice were dosed with a DPI alone or as an adjunct to standard oral anti-TB therapy (ATT). Mice infected on Day 0 were treated between Days 28-56 and followed up on Days 57, 71, and 85. Neither D

Multidrug-resistant Burkholderia pseudomallei pneumonia misdiagnosed as tuberculosis: A case report.

Burkholderia pseudomallei is a Gram-negative bacterium commonly found in coastal regions and is usually contracted through contact with contaminated water or soil. This report describes a case of chronic Burkholderia pseudomallei pneumonia in a 54-year-old male, initially misdiagnosed as pulmonary tuberculosis at an external hospital and resistance to treatment based on in vitro drug susceptibility testing. The patient experienced recurrent cough and pulmonary nodules for over 1 year, with symptoms temporarily alleviated by oral anti-infective drugs but recurring repeatedly. In April 2023, the

Nodular posterior scleritis associated with presumed ocular tuberculosis: A multimodal imaging case report.

PURPOSE: To describe a patient with nodular posterior scleritis associated with presumed ocular tuberculosis (TB). OBSERVATIONS: A 25-year-old Caucasian man reported metamorphopsia in the right eye (OD). He had lost the vision in his left eye when he was 15 years old. The visual acuity (VA) levels were 20/20 and light perception in the right and left eyes, respectively. Wide-field color fundus photography, fluorescein and indocyanine green angiography, optical coherence tomography, and ultrasound suggested an inflammatory condition associated with an elevated lesion in the choroid/sclera. Strongyloides Hyperinfection and Miliary Tuberculosis Presenting with Syndrome of Inappropriate Antidiuretic Hormone Secretion in a Malnourished Patient.

Strongyloides stercoralis (S. stercoralis) is an intestinal nematode endemic to tropical regions. An accelerated infection, known as a hyperinfection, occurs in immunocompromised patients, most commonly those treated chronically with glucocorticoids or those who have human T cell leukemia virus-1 (HTLV-1) infection. We describe a 67-year-old Hispanic female who presented with complaints of decreased oral intake and fatigue since three months. Hyponatremia on initial presentation was attributed to syndrome of inappropriate antidiuretic hormone (SIADH) secretion and managed with fluid restrictio
