PURPOSE: We evaluate our experience using the laparoscopic Palomo varicocele ligation (LPV) technique in male children and adolescents with varicoceles.
MATERIALS AND METHODS: Between September 1994 and September 2002, 122 varicoceles were treated using LPV with either mass cord vascular clip application (68) or LigaSure (Valleylab, Boulder, Colorado) vascular sealing (54). Antegrade sclerotherapy has been used since 1987 to treat patients with varicocele. Carried out to date in 825 patients at our clinic, the method has proved to be easy to perform, safe, economical, and effective. Fifty men underwent testicular biopsy at the time of varicocele ligation. The biopsies were scored and also a portion from each biopsy was subjected to meiotic analysis. All men were followed up (mean follow-up 19.3 months). There were no consistent histologic or meiotic abnormalities, and there was no evidence that the varicocele side was more defective than the contralateral side. Thirteen pregnancies were recorded, and these occurred only when the mean Johnsen score from one or other testis was greater than 6.0.

Varicocele was surgically induced in monkeys to study the long-term effects of the condition on seminal and testicular parameters. Sperm motility was depressed but improved after varicocelectomy. Venography and coil embolization of the left internal spermatic vein and/or right internal spermatic vein was performed in 75 patients with clinical or subclinical varicocele. Venograms of clinical varicocele were compared with venograms of subclinical varicocele. A major difference between the two groups was the degree of reflux. However, in terms of spermatologic improvement and restoration of fertility, embolization treatment appeared to be about equally effective in clinical and subclinical varicocele.

A case of metastatic adenocarcinoma from the left kidney to the veins of the pampiniform plexus is described following initial presentation with an acute varicocele. A systematic study of a series of thirty bilateral biopsies of testes was performed using electron microscopy. Observations showed how frequently there were bilateral changes within the Sertoli cells. These included a sometimes extensive vacuolization of the endoplasmic reticulum which may cause germ cells to exfoliate. Bilateral vascular lesions including modifications of the capillary endothelium which becomes thick and rich in pinocytotic vesicles and arteriolar spasms with an increase in number of microfilaments in the endothelial cells were constantly observed.

The DNA fragmentation index (DFI) is a newer and potentially more reliable marker for male factor infertility, with higher values indicating poorer sperm genetic quality. This study assessed the effect of varicocele embolization on DFI in men with infertility.
