BACKGROUND: Percutaneous arteriovenous fistulas have recently proven successful alternatives to surgical arteriovenous fistulas with encouraging initial results. The Ellipsys Endovascular Arteriovenous Fistula System utilizing ultrasound and thermal energy has recently received approval for use in the United States. At the University of New Mexico, we developed an integrated service between Vascular Surgery, Interventional Radiology, and Interventional Nephrology for percutaneous arteriovenous fistulas utilizing Ellipsys.

Patients with critical limb ischaemia have a poor life expectancy. Aggressive revascularization is accepted in order to preserve their independence in the final phase of their lives. Bypass surgery and more recently endovascular interventions with angioplasty and stenting have become the treatment of choice to prevent amputation and to resolve pain. Stent placement in the patent ductus arteriosus (PDA) is becoming an alternative treatment to improve cardiac function and quality of life in patients with ductal dependent cardiac conditions. Although, today most of these cardiac defects can be surgically corrected without the need of maintaining PDA patency for a long period of time, some countries lack the resources, and in other cases, surgical intervention is preferably delayed due to medical reasons. This report presents 2 cases where stents were placed in the PDA.

OBJECTIVES: Hemophilia is a sex-linked condition affecting about 1 of every 5000 males in the United States. The management of children with hemophilia can be improved with regular intravenous infusion of factor VIII or IX, thus preventing crippling and sometimes fatal hemorrhage. Maintaining this vital intravenous access is often hampered by gradual loss of superficial veins or repeated central catheter sepsis and thrombosis. By means of both experimental and finite element methods, we simulated three-dimensional unsteady flows through coronary bypass anastomosis. The host artery includes a stenosis shape located at two different distances of grafting. The inflow rates are issued from in vivo measurements in patients who had undergone coronary bypass surgery a few days before. We provide a comparison between experimental and numerical velocity profiles coupled with the numerical analysis of spatial and temporal wall shear stress evolution. The interaction between the graft and coronary flows has been demonstrated.

In an attempt to evaluate the effect of vein diameter on early patency and long-term durability of in situ lower limb bypasses, we evaluated 195 femoral-distal, popliteal, and/or tibial bypasses constructed in 189 patients (153 men, 36 women), consisting of tibial bypasses in 116 (60%), and popliteal in 79 (40%). One hundred and thirty-one reconstructions to the popliteal (n = 80), tibial (n = 26), and peroneal (n = 25) arteries were performed using glutaraldehyde (GA)-tanned human umbilical cord veins. Cumulative patency rates to 20 months, calculated by the standard life-table method were 84.8%, 74.0%, and 49.6% for popliteal, tibial, and peroneal reconstructions, respectively. BACKGROUND: Peripheral arterial disease (PAD) may cause occlusions (blockages) in the main arteries of lower limbs. We describe four patients in whom internal mammary arteries used as coronary artery bypass grafts demonstrated pathologic changes that could affect long-term patency. Two grafts showed evidence of arteritis, and two showed degenerative changes as seen in so-called cystic medial necrosis. In one of these two cases intraoperative dissection of the internal mammary artery and aorta occurred. Aside from the rare occurrence of atherosclerosis, we have found no previous reports of systemic vascular disease affecting this artery.
