Cogan's syndrome: a rare systemic vasculitis.

A 4 year old girl presented with keratitis and ataxia. Over the next two months she developed profound hearing loss, arthritis, and polychondritis. A diagnosis of Cogan's syndrome was made. The literature on the condition is reviewed and the importance of early diagnosis to prevent hearing loss is highlighted.

Neuroradiology of central nervous system vasculitis.

Evaluation of the patient with suspected vasculitis affecting the nervous system requires consideration of a broad range of differential diagnostic possibilities. Appreciating pertinent neuroradiologic findings as well as understanding the capabilities and limitations of the imaging methods employed is a prerequisite for proper evaluation. Most importantly, integration of the imaging features with clinical and laboratory findings is necessary to maximize the chance of a correct diagnosis.

Necrotizing vasculitis--relapse despite cytotoxic therapy.

The pattern of relapses is described in a consecutive series of 150 patients with vasculitis. Relapse occurred in 42 out of 124 patients who survived for more than 3 months.

Nodular vasculitis associated with propylthiouracil therapy.

Cutaneous vasculitis is a rare complication of propylthiouracil therapy. We describe a patient who presented with lower extremity subcutaneous nodules, hemorrhagic bullae, and necrotic ulcers during treatment with this agent. Examination of a skin biopsy specimen revealed nodular vasculitis. This type of vasculitis has not been described in association with propylthiouracil therapy before. Nodular vasculitis not associated with the drug is characterized by a chronic protracted course, whereas in the patient reported the lesions rapidly regressed when use of the drug was discontinued.

[Necrotizing vasculitis with predominant kidney involvement].

We report five patients with necrotizing vasculitis with predominant renal involvement; the diagnostic and therapeutics was effected completely at regional hospital. We detach: a) the clinical presentation's forms with rapidly progressive renal failure with microscopic hematuria and minimal proteinuria; b) the biopsy of kidney with necrotizing vasculitis or necrotizing glomerulonephritis (microscopic polyarteritis). We review the necrotizing vasculitis with predominant renal involvement in its pathogenic, clinical and histological aspects, and we insist that the diagnostic and early treatment …

Activated, cytotoxic lymphocytes in systemic vasculitis.

Peripheral blood lymphocytes from some patients with rheumatoid vasculitis and giant cell arteritis were cytotoxic in vitro towards endothelial cells, but not fibroblasts. Use of cell surface markers and cell density showed that cytotoxicity correlated with numbers of HLA-DR bearing cells, but not with cells bearing CD3, the transferrin receptor, or less dense lymphocytes. We concluded that activated, cytotoxic lymphocytes were present in the peripheral blood of a sub-set of patients with vasculitis.

Churg-Strauss vasculitis and ascaris infection.

A patient with Churg-Strauss vasculitis presenting with mononeuritis multiplex, who developed obstructive jaundice, is described. On investigation the jaundice proved to be due to ascaris infestation. As the immune abnormalities associated with ascaris infection are also typical of those seen in the Churg-Strauss syndrome it is speculated that the vasculitis occurred because of a failure to regulate the anti-ascaris immune response.

Serum cytokine changes in systemic vasculitis.

Cytokines are known to alter a number of vascular tissue cell functions. The aim of this retrospective study was to determine serum cytokine levels in patients with vasculitis and to analyse the possible relation to the severity of the disease. Tumour necrosis factor alpha (TNF alpha), interleukin-1 (IL-1)beta, IL-2, interferon (IFN)- and IFN-gamma were assayed in 33 patients with polyarteritis nodosa (PAN) or Churg and Strauss angiitis (CSA), and three with Wegener granulomatosis (WG). Serum cytokine changes were observed in most patients with active disease, i.e. before treatment was started…

Rheumatoid vasculitis with intrarenal aneurysm formation.

A 58-year-old male with an 8-year history of seropositive erosive rheumatoid arthritis (RA) presented with acute abdominal pain and syncope; a diagnosis of vasculitis was made arteriographically by finding intrarenal saccular aneurysms. Patients with severe RA and a high titer of rheumatoid factor may develop a diffuse vasculitis, but clinical renal involvement attributable to vasculitis is uncommon. Our case demonstrates the coexistence of RA and renal vasculitis, documented by autopsy findings, and for the first time by arteriographic demonstration of intrarenal aneurysm formation.

Surgical aspects of systemic necrotizing vasculitis.

Thirty patients with systemic necrotizing vasculitis (SNV) of the polyarteritis nodosa type were reviewed. Eleven patients (36%) had only gastrointestinal manifestations of the disease. Four patients (13%) had only pulmonary involvement. Seven patients (23%) had both gastrointestinal and pulmonary manifestations of SNV. With use of an approved protocol from the National Institute of Allergy and Infectious Diseases of cyclophosphamide and prednisone, remission was attained in 29 of 30 patients (96%). However, 18 patients (60%) underwent major surgical procedures for either diagnosis or complica…
