[Sigmoid shaped ventricular septum causing hemodynamic deterioration during lumbar anesthesia: a case report].

A 73-year-old woman with a left femoral neck fracture underwent an operation for femoral caput replacement. Twenty-five minutes after induction of spinal anesthesia, ST depression emerged followed by shock. Although the ST depression indicated an ischemic event, Levine grade IV systolic murmur was noticed at the aortic valve area. Echocardiography revealed narrowing of the left ventricular outflow tract with a pressure gradient of 170 mmHg. Rapid fluid infusion (1,000 ml) stabilized her blood pressure and the patient recovered without adverse sequelae. Echocardiography and magnetic resonance i

Microvascular dysfunction causing myocardial ischemia in obstructive hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is characterized by myocardial hypertrophy and can lead to significant complications including left ventricular outflow tract obstruction (LVOTO) and myocardial ischemia. Microvascular dysfunction (MVD) is a less recognized but crucial cause of myocardial ischemia in HCM, contributing to myocardial injury in the absence of obstructive coronary artery disease. We presented a case of a 48-year-old male with retrosternal chest tightness radiating to the left arm for 3 h. Electrocardiography revealed left ventricular (LV) hypertrophy with ST-segment depression and

Double outlet right ventricle with intact atrial septum and restrictive ventricular septal defect: an analysis of two cases.

Two cases of double outlet right ventricle with restrictive ventricular septal defect are described. This is an uncommon presentation that causes left ventricular dysfunction because of left ventricular outflow tract obstruction. The presence of an intact atrial septum leads to severe pulmonary hypertension, which tends to aggravate the right ventricular output. In the presence of a normal left ventricle, the authors suggest the possibility of enlargement of the ventricular septal defect in order to perform a biventricular repair. The association of a supramitral valve ring in both cases, and

Hemolytic Anemia due to Right Ventricular to Pulmonary Artery Conduit Stenosis.

Hemolytic anemia is a well-recognized complication in patients with left-sided mechanical heart valves. It is rare to see hemolysis with a bioprosthetic valve in the right ventricular outflow tract. We report a 4-year-old-girl with history of truncus arteriosus status-post repair who developed hemolytic anemia as a result of a calcified and obstructed bioprosthetic right ventricular to pulmonary artery-valved conduit. The hemolysis was alleviated by replacing the obstructed conduit with a larger valved conduit.

Cause of normal pulmonic velocity in fetal tetralogy of Fallot.

In 10 fetuses with tetralogy of Fallot, 9 lacked high velocity across the obstructed right ventricular outflow tract, which we conclude was due to the parallel flow circulations created by the combination of ventricular septal defect and arterial duct. The normally high resistance in the fetal pulmonary vascular system substantially exceeds the resistance of the systemic vasculature, resulting in the maintenance of right-to-left ductal flow in all 10 fetuses who had only moderate pulmonary obstruction.

Morphology of ventricular septal defect associated with coarctation of aorta.

It is now well established that the morphology of the ventricular septal defect associated with interrupted aortic arch is such as to compromise blood flow to the ascending aorta. Though there is some evidence that a similar mechanism may be operative in coarctation with ventricular septal defect, we are unaware of any necropsy study of this hypothesis. We therefore studied 25 hearts from the Heart Museum of Children's Hospital of Pittsburgh with coarctation and ventricular septal defect. Four had a malalignment defect with left ventricular outflow tract obstruction such as is seen in cases wi

Unrepaired tetralogy of Fallot in the adult: an unexpected diagnosis.

A 40-year-old female is admitted for paroxysmal episodes of exertional dyspnoea, with associated cyanosis, improving with squatting, and a holosystolic murmur radiating to the interscapular area. Echocardiography showed a subaortic ventricular septal defect with left-to-right shunt and overriding aorta. The characteristic murmur prompted us to seek right ventricular outflow tract obstruction. Magnetic resonance was performed, confirming Tetralogy of Fallot, and corrective surgery was performed.

[Ross-Konnno Procedure for the Patient of Intractable Subaortic Stenosis after Operation of Double Outlet Right Ventricle with Coarctation of the Aorta and Subaortic Stenosis:Report of a Case].

Recurrent left ventricular outflow obstruction (LVOTO) after intraventricular rerouting (IVR) with ventricular septal defect (VSD) enlargement and aortic arch repair for double-outlet right ventricle (DORV), non-committed VSD and coarctation of the aorta was successfully performed by a Ross-Konno procedure at 1 year 6 months as the second reoperation. Ross-Konno procedure could be an effective option in treatment of recurrent LVOTO after IVR for DORV.

The Yasui procedure with a modified right ventricle-to-pulmonary artery connection utilizing autologous left atrial appendage as a free graft.

A 9-month-old girl born with an interrupted aortic arch type B, an arteria lusoria (aberrant right subclavian artery) and a multilevel left ventricular outflow tract obstruction underwent a Yasui completion after an initial interim palliation. After the Damus-Kaye-Stansel procedure was carried out and the Sano shunt had been established as a source of pulmonary flow, the reported procedure comprised closure of the ventricular septal defect and the intraventricular baffling of left ventricular outflow through a malaligned ventricular septal defect, incision and partial resection of a conal sept

Severe obstruction to systemic blood flow in congenitally corrected transposition (discordant atrioventricular and ventriculo-arterial connexions): an analysis of 14 patients.

The more common associated cardiac anomalies in any cohort of patients with discordant atrioventricular and ventriculo-arterial connexions (congenitally corrected transposition) include ventricular septal defect, left ventricular (subpulmonary) outlet obstruction, and displacement and/or dysplasia of the morphologically tricuspid valve. There is scant mention in the literature of severe obstruction to systemic blood flow at aortic valve level or beyond in these hearts. The present study reviews those pertinent morphological-clinical and follow-up data on 14 patients with congenitally corrected

Role of provocation and exercise imaging for the identification of candidates for cardiac myosin inhibitors.

AIMS: Left ventricular outflow tract obstruction (LVOTO) drives symptoms and functional limitation in obstructive hypertrophic cardiomyopathy (oHCM). Some patients may only show treatment-qualifying obstruction during exercise echocardiography, yet their clinical profile and response to cardiac myosin inhibition are not well defined. This study compared the characteristics and therapeutic response of patients requiring exercise echocardiography to establish eligibility for mavacamten versus those meeting criteria at rest or during Valsalva. METHODS AND RESULTS: A single-centre retrospective c

Tetralogy of Fallot with anomalous origin of left anterior descending coronary artery. Surgical options.

Anomalous origin of the left anterior descending coronary artery from the right coronary artery can interfere with the location of the usual ventriculotomy during repair of tetralogy of Fallot. The purpose of this study was to compare the results of two operative techniques: (1) a "tailored" right ventricular incision and outflow patch reconstruction and (2) placement of a conduit from the right ventricle to the main pulmonary artery. We reviewed the records of 416 patients who had complete repair of tetralogy of Fallot at the Mayo Clinic from 1973 through 1984. Twenty (5%) (median age 6.5 yea

[Hypertrophic cardiomyopathy treated by interventional technique of septal reduction].

A 75-year old woman with obstructive hypertrophic cardiomyopathy and class IV dyspnea refractory to medical management had relative contraindications for both pacemaker implantation and surgical approach. Percutaneous intervention was devised to perform the selective injection of absolute alcohol in the first septal branch of the left anterior descending coronary artery. This led to a limited septal infarction documented by ST elevation, enzymatic elevation and the appearance of complete right bundle branch block on the electrocardiogram. A baseline left ventricular outflow tract pressure grad

5-Year Outcomes of Anterior Mitral Leaflet Laceration to Prevent Outflow Obstruction.

BACKGROUND: Left ventricular outflow tract (LVOT) obstruction is a common, often fatal complication of transcatheter mitral valve replacement (TMVR). Laceration of the anterior mitral leaflet to prevent outflow obstruction (LAMPOON) was safe and effective at preventing LVOT obstruction at 30 days in the National Heart, Lung, and Blood Institute LAMPOON trial. OBJECTIVES: The authors report the 5-year outcomes of intentional anterior mitral leaflet laceration before SAPIEN 3 TMVR, in patients at risk of LVOT obstruction. METHODS: The National Heart, Lung, and Blood Institute LAMPOON trial was

Impact of hypertrophic pattern and obstruction on cardiovascular magnetic resonance myocardial strain and left atrioventricular coupling in hypertrophic cardiomyopathy: a retrospective study.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) exhibits complex phenotypic heterogeneity. Left ventricular myocardial strain and atrioventricular coupling hold significant clinical value in cardiovascular diseases; however, the impact of different left ventricular hypertrophy (LVH) distribution patterns on myocardial strain and atrioventricular coupling remains unclear in HCM. OBJECTIVE: To investigate left ventricular strain and left atrioventricular coupling in HCM patients with different LVH distribution patterns and obstruction using cardiac magnetic resonance feature tracking. METHODS: A

Senning plus arterial switch operation for discordant (congenitally corrected) transposition.

BACKGROUND: Congenitally corrected transposition of the great arteries is a complex cardiac lesion, usually associated with ventricular septal defect, left ventricular outflow tract obstruction, and tricuspid valve abnormalities. A subset of patients without left ventricular outflow tract obstruction have undergone Senning plus arterial switch operation in an attempt to place the left ventricle in the systemic circuit and the right ventricle in the pulmonary circuit. METHODS: Fourteen patients have had the operation performed since July 1989. Age and weight medians were 12 months (range, 0.5

Hybrid Melody Valve Implantation in Mitral Position in a Child: Usefulness of a 3-Dimensional Printed Model for Preprocedural Planning.

We present the case of a 4-month-old child with atrioventricular canal associated with severe left atrioventricular valve dysfunction who previously underwent 3 surgical valve reconstructions without significant improvement. A Hybrid Melody valve (Medtronic, Minneapolis, MN) insertion was planned. Because of the low weight, the risk of left ventricular outflow tract obstruction was significant and therefore evaluated preprocedurally using a cardiac computed tomography-derived 3-dimensional printed model. In vitro tests showed good anchorage of the valve without subaortic obstruction and the pr
