Bilateral vestibular hypofunction in children.

BACKGROUND: Vestibular hypofunction in children can lead to frequent falls and delayed motor development. Especially children with bilateral vestibular hypofunction (BVH) are most at risk for developing symptoms. These children might benefit from future therapies, like vestibular implants, to restore their vestibular system. This study aimed to describe the prevalence, etiology, motor development, and hearing status of pediatric patients with BVH. METHODS: A multicenter retrospective chart review of 492 children with sensorineural hearing loss was performed. Children with at least one bilater

Recurrent spontaneous vertigo with interictal headshaking nystagmus.

OBJECTIVE: To define a disorder characterized by recurrent spontaneous vertigo (RSV) of unknown etiology and interictal headshaking nystagmus (HSN). METHODS: We characterized HSN in 35 patients with RSV-HSN compared to that recorded in randomly selected patients with compensated vestibular neuritis (VN), vestibular migraine (VM), and Ménière disease (MD). RESULTS: The estimated time constant (TC) of the primary phase of HSN was 12 seconds (95% confidence interval [CI] 12-13) in patients with RSV-HSN, which was larger than those in patients with VN (5 seconds, 95% CI 4-5), VM (5 seconds, 95%

Ocular vestibular evoked myogenic potentials: frequency tuning to air-conducted acoustic stimuli in healthy subjects and Ménière's disease.

Ocular vestibular evoked myogenic potentials (oVEMP) in response to 250-, 500- and 1000-Hz air-conducted short tone bursts were studied in 22 healthy subjects and 37 Ménière's disease patients. The goal of this study was to investigate normal tuning characteristics of the oVEMP and the possible oVEMP changes with respect to frequency dependence in Ménière's disease. In unilateral Ménière's disease patients, a distinction was made between affected ears and unaffected ears. It was found that in normal subjects, the oVEMP tunes to a stimulus frequency of 500 Hz, with the highest amplitude and low

Hyperventilation-induced nystagmus in patients with vestibular neuritis in the acute and follow-up stages.

OBJECTIVE: Our purposes were to characterize hyperventilation-induced nystagmus (HVIN) in patients with unilateral vestibular neuritis (VN) through follow-up examinations and to determine the effects of hyperventilation on vestibular imbalance in patients with VN. MATERIALS AND METHODS: The horizontal eye movements in 35 patients with acute VN were recorded. The eye movements were analyzed and the maximum value of slow-phase eye velocity (SPV) was obtained during and after hyperventilation. Nineteen of 35 patients underwent follow-up examinations around 7 weeks later. When spontaneous nystagm

[The significance of the interaural latency difference of VEMP].

OBJECTIVE: To investigate the significance of the interaural latency (IAL) difference of the latency of VEMP and to raise the sensitivity of the test. METHOD: Vestibular evoked myogenic potentials (VEMP) were tested in 20 healthy subjects; 13 patients with acoustic neuromaor cerebellopontile angle occupying lesions and 1 patient with multiple sclerosis. IAL differences of the wave p13,n23 and p13-n23 (abbreviatd as /delta p13/, /delta n23/ and /delta p13-n23/, respectively) were analysed to determine the normal range and the upper limit of the norm data. Vertical eye-movement oscillation with a frequency double that of lateral linear acceleration in patients with long-standing unilateral vestibular loss.

OBJECTIVE: To examine our hypothesis that an unusual floating or tilting sensation claimed by patients with long-standing unilateral vestibular loss might be attributed to incomplete central compensation in the otolith system. METHODS: Seven patients who were with or without symptoms for 6-101 months after intratympanic gentamicin therapy for unilateral endolymphatic hydrops were sinusoidally exposed to lateral linear acceleration, and their compensatory eye movements were compared with those of 18 normal controls, using electro-oculography (EOG). The subjects, secured firmly in the chair of

Significance of head-shaking nystagmus in the evaluation of the dizzy patient.

In a series of studies, the phenomenon of head-shaking nystagmus (HSN) was assessed in 50 control subjects and 1364 consecutive dizzy patients who underwent formal electronystagmography (ENG) at the Toronto Hospital Center for Advanced Hearing and Balance Testing. HSN was compared in a series of 30 patients who underwent conventional electro-oculography (EOG) vs magnetic (scleral) coil eye movement recordings. Clinical correlation of HSN to other parameters of the ENG test battery was performed in another sub-series of 300 patients with known diagnoses. HSN was identified in 31.7% of dizzy pat

Panic disorder and vestibular disturbance: an overview of empirical findings and clinical implications.

The purpose of this article is to provide a general overview of the growing literature on the relationship between panic disorder and vestibular disturbance. This relationship has been examined from two distinct perspectives, including: (a) the assessment of vestibular dysfunction in patients with panic disorder; and (b) the evaluation of panic symptomatology in patients with vestibular disturbance. Consequently, this review focuses primarily on the literature pertaining to vestibular symptoms in patients with panic disorder and panic symptomatology in patients with vestibular complaints. Subs

[Electronystagmography aspects of peripheral and central vestibular syndromes].

This report deals with what is to be known in applying ENG in the everyday office. From vestibular, visual and proprioceptive informations central neural processing evolve to stabilize images on the retina. ENG can perform an analysis of these systems, alone or interacting. Anatomical, neurophysiological backgrounds and experimental pathophysiology results are first described. The methodic rule of anamnesis is emphasized and the non instrumental examination is outlined. Concerning the technical aspects and the ENG achievement, some nonlinearities are stressed. Next, procedure of eye movements

Cochlear-Vestibular Impairment due to West Nile Virus Infection.

OBJECTIVES: West Nile virus (WNV) has been spreading over the last 20 years. Human infection is asymptomatic in most cases. When the disease becomes clinically manifest, it may involve a range of issues, from a mild infection with flu-like symptoms to a neuroinvasive disease. Albeit rarely, WNV-associated sensorineural hearing loss (SNHL) has also been reported. Here we describe two new cases of SNHL and balance impairment caused by WNV infection. METHODS: The patients were investigated with repeated audiometric tests and, for the first time, videonystagmography was also used. RESULTS: Unlik

The Vestibular Effects of Repeated Low-Level Blasts.

The objective of this study was to use a prospective cohort of United States Marine Corps (USMC) instructors to identify any acute or long-term vestibular dysfunction following repeated blast exposures during explosive breaching training. They were assessed in clinic and on location during training at the USMC Methods of Entry School, Quantico, VA. Subjects received comprehensive baseline vestibular assessments and these were repeated in order to identify longitudinal changes. They also received shorter assessments immediately following blast exposure in order to identify acute findings. The m

Peripheral vestibular dysfunction in patients with primary ciliary dyskinesia: abnormal otoconial development?

HYPOTHESIS: Patients with primary ciliary dyskinesia (PCD) have absent or reduced otoconial function compared to the normal population. BACKGROUND: Investigations in zebrafish show that ciliation is important for the development of the otolith organs, but this has never been evaluated in humans. PCD is a congenital defect of ciliary structure. We undertook a pilot study to determine whether patients with PCD have absent or reduced otoconial function compared to the normal population. METHODS: Vestibular function testing, including utricular centrifugation (UCF) testing, vestibular evoked myo

Rotatory and collic vestibular evoked myogenic potential testing in normal-hearing and hearing-impaired children.

OBJECTIVES: Vertigo and imbalance are often underestimated in the pediatric population, due to limited communication abilities, atypical symptoms, and relatively quick adaptation and compensation in children. Moreover, examination and interpretation of vestibular tests are very challenging, because of difficulties with cooperation and maintenance of alertness, and because of the sometimes nauseatic reactions. Therefore, it is of great importance for each vestibular laboratory to implement a child-friendly test protocol with age-appropriate normative data. Because of the often masked appearance

[Early and late damage to the auditory and vestibular area after meningitis in childhood and adolescence].

Meningitis is still one of the serious infectious diseases in clinical practise. In a high percentage of cases with this disease process, the auditory and vestibular parts of the inner ear are damaged. In this study the auditory and vestibular systems of 53 children and teenagers between 1.7 and 18.7 years of age were examined following meningitis. The mean time-interval between meningitis and examination was 5.8 years. Eighty-one of 106 ears examined showed sensorineural hearing losses which varied between mild hearing loss and total deafness. Peripheral vestibular lesions were found in 60 of

Patterns of vestibular dysfunction in chronic traumatic brain injury.

BACKGROUND: Dizziness and imbalance are common following traumatic brain injury (TBI). While these symptoms are often attributed to vestibular dysfunction, the relative contribution of peripheral vs. central mechanisms is unclear. This study investigated the prevalence of semicircular canal and otolith abnormalities in a cohort of patients with chronic TBI and symptoms of dizziness or imbalance. The relationship between vestibular, oculomotor and posturography results was further explored. METHODS: Clinical records of patients attending the New Zealand Dizziness and Balance Centre from Januar

Effect of vestibular training on cognitive functions in people with multiple sclerosis: A randomized controlled trial.

BACKGROUND: Cognitive dysfunction is prevalent but inadequately treated in people with multiple sclerosis (PwMS). Central vestibular functions are associated with multiple cognitive domains in PwMS, even when controlling for disability status. To our knowledge, the effectiveness of vestibular training on cognition in PwMS has never been explored. The study's purpose was to determine the effect of vestibular training on PwMS's cognitive functions. METHODS: Forty PwMS with relapsing-remitting MS, primarily fatigued, were randomly divided into two equal groups. The control group (GI) was treated
