In a prospective study of lung function of 34 patients taking amiodarone, 24 showed no functional changes but 10 developed a sustained fall in CO transfer factor (TLCO) exceeding 15 per cent. These patients had on average received a higher dose of drug in the first three months of treatment. Seven showed no clinical or radiographic changes and TLCO improved with reduction in drug dose. The other three patients developed florid clinical and radiographic features of amiodarone pulmonary toxicity. All three had impaired TLCO before receiving amiodarone. We explored the relationship between a number of respiratory symptoms or symptom complexes (e.g., cough, recent wheeze, byssinosis) and lung function in white cotton textile workers (CTW) and control subjects 45 yr of age or older. The CTW were studied in 1973 and in 1979 in Columbia, South Carolina, and the control subjects were studied in 1972 and in 1978 in Lebanon, Connecticut. The CTW had a high prevalence of byssinosis (18%) and other symptoms. Breathlessness, disability, and exercise tolerance were assessed in 26 patients with severe chronic airflow limitation (forced expiratory volume in one second (FEV1) less than or equal to 1 litre) divided into two groups--15 patients who were normocapnic (pressure of arterial carbon dioxide (Paco2) less than 5.5 kPa (less than 41.4 mm Hg)), and 11 patients who were hypercapnic (Paco2 greater than 6 kPa (greater than than 45.1 mm Hg)). This paper reports on 41 immature patients whose scoliosis was treated by fixation of ribs on the convexity. All had progressive scoliosis, the curve deteriorating at more than 10 degrees p.a., or the rib-vertebra angle difference being greater than 20 degrees. Dynamic spirometry and arterial blood gas analyses were performed in a series of 12 tetraplegic and 8 paraplegic patients.
