Moderate hyperhomocysteinemia and immune activation.

Moderate hyperhomocysteinemia is associated with an increased risk of atherosclerosis, thrombosis and neurodegenerative diseases. Homocysteine accumulation in the blood can be due to many underlying causes, which may interact with each other, e.g. genetic disposition and B-vitamin status. The role of the sulfur-containing amino acid homocysteine in the pathogenesis of diseases remains unclear, even if many studies suggest a causal relationship between homocysteine-mediated processes like oxidative stress, NO-inactivation and endothelial deficiency and atherogenesis. Proposed mechanisms of acti

Folate and Vitamin B12 Status in Pediatric Hematopoietic Stem Cell Transplantation Patients.

BACKGROUND/OBJECTIVES: Vitamin B12 and folic acid (FA) are crucial for children's hematopoiesis after hematopoietic stem cell transplantation (HSCT). This study evaluates the B12 and FA level changes before and after HSCT. METHODS: We retrospectively collected data from 125 patients who underwent HSCT between March 2019 and February 2024. B12 and FA levels were measured at three time points: before transplantation, one month after, and six months after. Sixty-two patients had complete data. B12 deficiency was defined as levels < 200 pg/mL and insufficiency as 200-300 pg/mL. Folate deficiency

Vitamin B12 deficiency and anemia in 140 Taiwanese female lacto-vegetarians.

BACKGROUND/PURPOSE: Lacto-vegetarians (LVs) tend to have vitamin B12 deficiency (B12D). This study assessed whether 140 female LVs, including 16 B12D/LVs and 124 non-B12D/LVs, had significantly higher frequencies of microcytosis, macrocytosis, and of blood hemoglobin (Hb), red blood cell (RBC), and serum vitamin B12 deficiencies than 140 healthy control subjects (HCSs). METHODS: The complete blood count and serum vitamin B12 level in 140 female LVs and 140 female HCSs were measured and compared. RESULTS: We found that 8.6%, 4.3%, 22.9%, 20.0%, and 11.4% of 140 LVs had microcytosis, macrocyto

Anemia, hematinic deficiencies, and hyperhomocysteinemia in gastric parietal cell antibody-positive and -negative burning mouth syndrome patients.

BACKGROUND/PURPOSE: Our previous study found the serum gastric parietal cell antibody (GPCA) positivity in 12.3% of burning mouth syndrome (BMS) patients. This study assessed whether GPCA-positive BMS (GPCA+BMS) patients had significantly higher frequencies of macrocytosis, anemia, hematinic deficiencies, and hyperhomocysteinemia than healthy control subjects or GPCA-negative BMS (GPCA-BMS) patients. METHODS: The mean corpuscular volume, blood hemoglobin (Hb), and serum iron, vitamin B12, folic acid, homocysteine, and GPCA levels were measured and compared between any two of three groups of 1

Atrophic glossitis: Etiology, serum autoantibodies, anemia, hematinic deficiencies, hyperhomocysteinemia, and management.

Atrophic glossitis (AG) is characterized by the partial or complete absence of filiform papillae on the dorsal surface of the tongue. AG may reflect the significant deficiencies of some major nutrients including riboflavin, niacin, pyridoxine, vitamin B12, folic acid, iron, zinc, and vitamin E. Moreover, protein-calorie malnutrition, candidiasis, Helicobacter pylori colonization, xerostomia, and diabetes mellitus are also the etiologies of AG. Our previous study found the serum gastric parietal cell antibody (GPCA), thyroglobulin antibody (TGA), and thyroid microsomal antibody (TMA) positiviti

Relationship between cobalamin and folate deficiencies and anemia in dogs.

BACKGROUND: Megaloblastic, nonregenerative anemia is a well-known consequence of cobalamin or folate deficiencies in humans but is not recognized in hypocobalaminemic or hypofolatemic dogs. Establishment of relationships between hypocobalaminemia or hypofolatemia and hematologic disease would encourage vitamin B testing, and potentially supplementation, in anemic dogs. OBJECTIVES: To determine the prevalence of anemia in hypocobalaminemic or hypofolatemic dogs and to report the prevalence of hypocobalaminemia and hypofolatemia and nonregenerative anemia, macrocytosis, and anisocytosis in anem

The relationship between serum trace elements, vitamin B12, folic acid and clinical parameters in patients with myofascial pain syndrome.

BACKGROUND: Myofascial pain syndrome (MPS) is characterized by myofascial trigger points in a palpable taut band of skeletal muscle. OBJECTIVE: We aimed to investigate serum trace elements, vitamin B12, folic acid levels and their correlations with clinical findings and functional status in patients with MPS. METHODS: Thirty eight patients with at least one trigger point located on shoulder muscles, and at least 6 months duration, were included in this study. The demographic data, disease duration of patients were noted. Serum copper, zinc, magnesium and iron levels, vitamin B12 and folic ac

Vitamin B-12 status is associated with socioeconomic level and adherence to an animal food dietary pattern in Colombian school children.

Vitamin B-12 is related to neurocognitive function in school-age children, yet sociodemographic and dietary correlates of vitamin B-12 status in this age group are not well characterized. The prevalences of vitamin B-12 or folate deficiencies in Colombia are unknown. We conducted a cross-sectional study in a representative sample of 2800 low- and middle-income children aged 5-12 y from Bogotá's public schools. Plasma vitamin B-12 and erythrocyte folate concentrations (mean +/- SD) were 327 +/- 106 pmol/L and 858 +/- 256 nmol/L, respectively. The prevalence of vitamin B-12 deficiency (<148 pmol

The treatment of patients at risk of developing Wernicke's encephalopathy in the community.

AIM: To review the process of identifying alcohol-dependent patients at risk of developing Wernicke's encephalopathy (WE) in the community, and prophylactic treatment options. METHODS: Non-systematic literature review of the diagnosis of thiamine deficiency and of its treatment in the community. The role of supplementation of beer and bread with thiamine was evaluated. RESULTS: The diagnosis of thiamine deficiency is not always made, and treatment apparently may sometimes be inadequate. CONCLUSIONS: Alcohol-dependent patients in the community who are at risk of developing WE should be given

Diagnosing vitamin B12 deficiency, a common geriatric disorder.

Vitamin B12 deficiency in the elderly is a common disorder associated with an increased morbidity if it goes undetected, as often happens. Its diagnosis can be enhanced if the clinician recognizes the associated clinical features of nonspecific symptoms, glossitis, and dermatologic and neuropsychiatric abnormalities, and realizes the limitations of various tests (serum B12 assay, parietal cell and intrinsic factor antibody, mean corpuscular volume, and Schilling tests). Available data indicate it is sufficient to prescribe replacement B12 injections three or four times a year.

An assessment of serum 57Co cyanocobalamin as an index of vitamin B12 absorption.

A technique has been developed for accurately assaying serum and urine (57)Co cyanocobalamin during the conventional Schilling test. Serum and urine radioactivity in a large number of patients has been correlated and the precise normal limits have been determined by statistical analysis. The implications of this for investigating conditions other than pernicious anaemia and the malabsorption syndromes are stressed.

Bicytopenia with haemolysis: Think severe vitamin B12 deficiency-associated thrombotic microangiopathy.

Bicytopenia is not uncommonly encountered in clinical practice; it may reflect either marrow failure or a consumptive process such as haemolysis with thrombotic microangiopathy (TMA). We describe a case of a patient with bicytopenia caused by severe Vitamin B12 deficiency, where there is laboratory evidence that is suggestive of haemolysis in what is known as pseudo-TMA phenomenon. Lack of awareness of this entity may lead clinicians to misdiagnose such cases as microangiopathic haemolytic anaemia (MAHA) or thrombotic thrombocytopenic purpura (TTP), which could lead to inappropriate immunomodu
