Clinically inapparent invasive vulvar carcinoma in an area of persistent Paget's disease: a case report.

BACKGROUND: Invasive vulvar carcinoma is reported to occur in 5 to 20% of patients with vulvar Paget's disease. We report a case in which a clinically inapparent invasive lesion was discovered on reexcision of microscopically persistent vulvar Paget's disease. CASE: A 58-year-old woman presented with a diagnosis of vulvar Paget's disease. A wide local excision of the lesion was performed and pathologic analysis revealed microscopic Paget's disease at two of the margins. The patient returned for a follow-up 4 months later and a vulvar biopsy revealing persistent Paget's cells was obtained from

Neuroma of the clitoris after female genital cutting.

BACKGROUND: Nerve tumors of the clitoris and particularly neuromas are extremely rare. CASE: A 27-year-old infibulated African woman suffering from chronic vulvar pain increasing with sexual intercourse presented for gynecologic care. Examination revealed a painful clitoral tumor. The tumor was surgically excised. The diagnosis of amputation neuroma of the clitoris was made by microscopic examination. CONCLUSION: This is the first well-documented case of clitoral amputation neuroma occurring after female genital cutting. Considering the high number of genital cuttings practiced, these tumors

Vulval myxoid liposarcoma.

A 26 year old woman presented with a 4-year history of a gradually enlarging fluctuant mass on the left labium majus. Histologic examination following excision revealed myxoid liposarcoma. Following the diagnosis, further surgery was performed to ensure complete resection. Routine excision of draining lymph nodes is not advocated. Although rare, myxoid liposarcoma should be considered in the differential diagnosis of a vulval soft tissue mass.

Radiotherapy for liposarcoma of the vulva.

BACKGROUND: Primary liposarcoma of the vulva is extremely rare. We report a case of liposarcoma of the vulva which was treated with local excision and postoperative radiotherapy. CASE: A 21-year-old woman complained of a painless lump in her labium majus which she first noticed 3 years earlier. An initial diagnosis of lipoma of the vulva was made. The patient was treated by surgical removal. On examination of the surgical specimen, the final pathological diagnosis was well-differentiated liposarcoma and the stalk was not excised completely. Radiotherapy was initiated because of the uncertaint

Verrucous carcinoma of the vulva in a patient infected with the human immunodeficiency virus.

Verrucous carcinomas of the vulva are rare and have not been reported in women infected with the human immunodeficiency virus. We present such a case in a 32-year-old woman characterized by bladder involvement that failed therapy with 13-cis-retinoic acid and interferon-alpha and required anterior exenteration.

Liposarcoma (atypical lipomatous tumors) of the vulva: a clinicopathologic study of six cases.

Liposarcoma, the most common soft tissue sarcoma, usually arises in the limbs, trunk, or abdomen, but very rarely in the vulva. Six cases of liposarcoma of the vulva are described here. They were characterized by their occurrence in predominantly middle-aged women (median age 52), variable size, and focally infiltrative margin. The preoperative clinical diagnosis for all patients was of a benign lesion, with a diagnosis of lipoma in two. Four of six cases had the usual histologic appearance of a well-differentiated liposarcoma/atypical lipomatous tumor with variation in adipocyte size, adipocy

Epithelioid hemangioendothelioma of the clitoris: a case report with immunohistochemical and ultrastructural findings.

A 30-year-old woman was referred for evaluation of a small nodule of the clitoris. This was subsequently diagnosed as epithelioid hemangioendothelioma. This rare vascular tumor of intermediate malignancy has not been previously described in the vulva. The patient underwent a modified radical vulvectomy and bilateral inguinal lymph node dissection, and subsequently received photon therapy. She is alive with no evidence of disease 27 months after diagnosis.

Microbiological and Histopathological Features of Gynecologic Cancers: A Systematic Review of Current Evidence and Future Directions.

Gynecologic cancers are a diverse group of malignancies with distinct microbiological, histopathological, and molecular characteristics. This systematic review examined the available evidence on these features in gynecologic cancers and premalignant lesions. A total of 46 studies involving 18,742 patients or samples were included. Because the studies differed considerably in design, sampling methods, laboratory techniques, histopathological classification, and reported outcomes, the findings were synthesized narratively. The strongest and most consistent evidence was found for persistent high-

Vulvar Cellular Angiofibroma With Cytologic Atypia and Sarcomatous Transformation: A Clinicopathologic Analysis.

Cellular angiofibromas (CAFs) are benign mesenchymal neoplasms of the vulva and lower genitourinary tract. Although most cases are benign with excellent prognosis, data on CAFs with cytologic atypia (aCAF) and sarcomatous transformation (tCAF) is limited. We identified 13 vulvar CAFs comprising 4 aCAFs and 9 tCAFs. The median age at presentation was 49  yr (40-84). All tumors involved the subcutis with a median size of 4.75 cm (0.8-11.7). Vascular and stromal hyalinization was present in all cases. Fascicular growth pattern and chronic perivascular inflammation were seen in 10 cases, followed

[Cutaneous leiomyomas: a clinicpathological and epidemiological review].

BACKGROUND: Cutaneous, superficial and or suprafascial leiomyoma are divided into three variants: piloleiomyomas (PL), angioleiomyomas (AL) and genital leiomyomas (GL) that include the vulvar, scrotal and areolar forms. This study set out to establish the clinical and histological characteristics and incidence of each variant, and any likely associations with internal neoplasms. METHODS: A review was carried out of 255 cases of cutaneous leiomyomas diagnosed between 1982 and 2018 at the Pathology departments of three hospitals (Navarra and Alicante). Demographic, clinical, histological and im

Human papillomavirus-associated subsequent malignancies among long-term survivors of pediatric and young adult cancers.

Long-term survivors of pediatric and young adult (PAYA) cancers have a high incidence of subsequent neoplasms, but few risk factors other than cancer treatment have been identified. We aimed to describe the burden of human papillomavirus (HPV)-associated malignancies among survivors of PAYA cancers to assess whether HPV infections might be a reasonable area of future etiologic research on subsequent malignancies in this population. We used longitudinal data from 9 population-based registries of the Surveillance, Epidemiology, and End Results program collected between 1973 and 2010 to assemble

Magnetic resonance imaging and computed tomography appearance of asymptomatic paravaginal cysts.

The magnetic resonance imaging (MRI) and computed tomography (CT) appearance of 18 paravaginal cysts was reviewed. On MRI, serious cysts were hypointense on T1-weighted and hyperintense on T2-weighted images. Cysts presumed to contain proteinaceous material were hyperintense on T1-weighted and T2-weighted sequences while hemorrhagic cysts exhibited varying stages of hemorrhage. On CT, a high attenuation cyst was presumed to contain proteinaceous material or hemorrhage. MRI and CT examination may reveal paravaginal cysts; their varied appearance must be recognized to avoid confusion with other

HPV-related Vulvar Disease.

Human papillomaviruses (HPV) are the most common cause of sexually transmitted infections worldwide and can play a role in vulvar disease. This review provides a clinically relevant summary of our current understanding of the virology, epidemiology, and pathogenesis of HPV along with diagnosis and management strategies of condyloma acuminata, vulvar intraepithelial neoplasia (VIN), and invasive squamous cell carcinoma. Formal screening and management guidelines for vulvar disease, especially cancer, remain limited, highlighting the need for more robust evidence and consensus. Early recognition

[Epidemiology of vulvar intra-epithelial neoplasias].

The vulvar intraepithelial neoplasia has been identified as one of the 12 neoplasias whose incidence increases in the developed countries. The vulvar intraepithelial neoplasia (VIN) and invasive vulvar cancer incidence increases by 2.4% per annum; and this principally in young women. The VIN account for 57% of the vulvar neoplasias and are actually more frequent than invasive carcinomas. In the United States, between 1973 and 2000, the incidence of the VIN increased by 411% against 20% for invasive cancers. Similar figures were reported from Norwegian registers. The VIN have a different age di
